[Urinary tract infection by Staphylococcus saprophyticus in a child].

UNLABELLED: Staphylococcus (S.) saprophyticus belongs to the coagulase-negative staphylococci and is the cause of urinary tract infections, especially in young women, rarely in children and in elderly men. Probably the real incidence of urinary tract infections due to these organisms is higher than usually accepted. We report on a 11 3/12-year-old girl, who developed symptoms of a urinary tract infection (dysuria, flank pain), but without fever and elevated CRP. In the bladder aspirate we found leukocytes and S. saprophyticus (cotrimoxazole-resistant). Ultrasound detected intrarenal doubling, but no typical anomalies, disposing for urinary tract infections. The treatment with cefuroxim i.v. and clindamycin p.o. was successful. CONCLUSION: Coagulase-negative staphylococci cultivated in urine represent contamination (mostly S. epidermidis--except in patients, who have undergone manipulation or catheterization of the urinary tract) or they are the cause of an urinary tract infection (mostly S. saprophyticus).

[Dentogenic infection due to Streptococcus constellatus in a child].

An 8-year-old boy was admitted with a pain-full swelling at the right mandible and symptoms of septicemia. The illness was caused by a dental infection due to Streptococcus constellatus proved by isolation from blood culture, the origin was a decayed tooth. Treatment comprised suitable antibiotics, extraction of the tooth and local irrigations with chlorhexidine solution. General condition improved rapidly, the local situation rather slowly, however.

[Infection caused by viridans streptococci in children with malignant hematologic diseases].

This is a review of the symptoms and signs of children with malignant diseases and septicemia due to viridans streptococci, treated during a 6-year-period (1990-1995) in the Departments of Pediatrics or Pediatric Surgery, University of Leipzig. All 11 children suffered from leukemia. Streptococcus mitis was the most frequently isolated streptococcal species. All patients had fever and malaise, in most cases we could find inflammatory signs of the respiratory tract. In two children we saw a severe course of the disease, one child had symptoms and signs of ARDS, the other died on septicemic shock. All 11 patients had neutropenia and a central venous line, 10 of them were treated by cytarabine before the streptococcal infection was diagnosed. Like others we could note during the last years an increase of systemic infections due to viridans streptococci in neutropenic patients with malignant diseases. Possibly there is an association between streptococcal infection and cytarabine therapy. The empiric antibiotic therapy in neutropenic patients with malignant diseases should cover also streptococcal infections.

[Recurrent subacute endocarditis caused by Streptococcus mutans in a child].

Bacterial endocarditis belongs to the rare diseases in childhood. It occurs usually as a single episode and almost exclusively in children with congenital heart disease. In recent years, however, an increased number of renewed endocarditis after the first episode were reported, especially in drug addicts. We present a case of renewed subacute infective endocarditis 3 years and 9 months after complete recovery from the first one. Furthermore, using the available literatures, the role of risk factors, the change in spectrum of the infecting organisms, the diagnostic and therapeutic approach in cases with renewed infection are discussed.

[Pharmacokinetic studies of cefotiam in late pregnancy].

Examining blood level values and urinary concentrations taken from pregnant women during the third trimester and nonpregnant female volunteers, the pharmacokinetic dates of cefotiam (a second generation's cephalosporin) have been evaluated. We used a computer program of an open two-compartment model for intravenous injection. We found an increase of some pharmacokinetic parameters (elimination half-life, distribution volume, clearance) during pregnancy. Because the serum levels were yet effective and nearly equal 6 hours after intravenous injection we state that it is not necessary to change the dosage and the application interval of cefotiam during pregnancy.
