Cloacogenic carcinoma is a rare tumor, originating from epithelium of the anal transition zone. We report a 63-yr-old man with pathologically proven cloacogenic carcinoma which caused a rapidly progressive paraparesis and changes in mental status. These were related to extramedullary deposits around the spinal cord and cauda equina and intramedullary deposits in the brain. This pattern of neural involvement with anal canal carcinoma has not been reported previously.
Three kinds of generalized residuals based on probability integral transformation are defined and their roles in graphical goodness of fit testing are discussed. Although they are useful in fitting parametric distributions, their value is questionable, however, in fitting Cox's proportional hazard rate (PHR) models. They almost always appear to give a good fit because of (a) almost non-parametric nature of the PHR models; (b) heavy right-hand censoring in epidemiological data; and (c) scale adjustment in graphical presentation. Thus, the apparent overall fit has little inferential meaning, but the residuals are useful in exploratory analysis. A simple method, based on non-parametric stratified residual plots, for selection of risk factors is discussed. It is, in fact, equivalent to comparison of several empirical survival functions, one for each set of values (stratum) of a covariable under consideration. It is also suggested that for the preliminary PHR model, the continuous variables should be discretized and indicator variables used in the model to check analytically the graphical perception of the possible functional form of the contribution of each covariable to the PHR model.
Antibodies to specific human immunodeficiency virus (HIV) polypeptides are important laboratory markers of HIV infection. We have used an antibody to the major structural gag protein p24 of HIV-1 virus to immunochemically localize this capsid antigen in lymphoid cells from seven of eight patients at risk for HIV infection and who presented with parotid lymphadenopathy and lymphoepithelial cysts of the parotid gland. A clinicopathological assessment of these two manifestations as they relate to HIV infection is also presented.
The passage of the Chernobyl plume over the United Kingdom in May 1986 led to the deposition of radionuclides on the ground by wet and dry deposition processes. Here we analyse rainfall during the passage of the plume and the published monitoring data obtained afterwards, and show that levels of deposited 137Cs can be closely related to rainfall intercepting the plume. 137Cs was present in the atmosphere mostly as particulate species with wet deposition mechanisms dominating. In contrast, 131I was present as particulate and vapour phase material, and reported levels on grass and in cow's milk show that both wet and dry deposition mechanisms were important. 131I on grass and in cow's milk therefore shows a different geographic pattern to 137Cs, and is not so closely related to rainfall. We have calculated washout factors for locations where there are data on deposition, rainfall and air concentrations during the passage of the Chernobyl plume. From these factors and interpolated concentrations in the atmosphere, the total deposition of 137Cs has been estimated at each of 4,000 rain gauge stations in the United Kingdom. The results are presented as deposition contours and have been compared with measurements in parts of the country. Estimates of the total deposition of 131I and 137Cs show that less than or equal to 1% of the estimated total releases from Chernobyl were deposited on the United Kingdom.

The case presented illustrates an unusual entity. The differential diagnosis of degenerative spinal stenosis is voluminous, although incomplete without including oxalosis. Oxalosis as a cause of spinal stenosis has not yet been reported in the orthopedic literature.
Fifteen partial parotid salivary gland resection specimens interpreted as benign lymphoepithelial lesion (BLL) were accessioned by our surgical pathology service between January 1983 and December 1986. Twelve of the specimens were removed from 11 prison inmates referred to our hospital, a patient subgroup constituting the source of only 2% of surgical pathology specimens in general. All prison inmates with BLL had a history of intravenous drug use, had developed unexplained lymphadenopathy concurrently with the parotid gland enlargement, and had suspected human immunodeficiency virus (HIV) infection (serologically confirmed in two). Histopathologic features of the parotid gland specimens included atypical follicular hyperplasia and follicular involution, resembling lymph node changes of HIV infection. The clinical and pathologic findings in these cases suggest an association between HIV infection and BLL, and support a previously proposed mechanism for the development of BLL through progressive enlargement of intraparotid lymph nodes.

We found that fetal bovine serum supplementation of culture medium provided limited quantities of linoleic acid, an essential fatty acid, to cells grown in culture (2.8 +/- 0.3% of total fatty acids in 12 lots). Supplementation of the medium with additional linoleic acid resulted in altered phospholipid acyl composition in cells of two established lines, A549, a putative model of the pulmonary Type II epithelial cell, and SIRC, a line derived from rabbit corneal epithelium. In particular, linoleic acid supplementation induced a relative increase in disaturated choline phosphoglycerides of 33 and 36%, respectively, in cells of the two lines. This observation may be relevant to design of media for primary culture of Type II cells, in which disaturated phospholipid synthesis used as an index of differentiated function (surfactant production). Linoleate supplementation did not alter growth or size (protein content) of cells of either line and caused a slight increase in accumulation of neutral lipid, in the form of cytoplasmic droplets, in A549 cells. Supplementation of cell cultures with equivalent concentrations of the nonessential fatty acids palmitic and oleic acid did not significantly alter the growth, morphologic appearance, or lipid composition of the cells. However, it was demonstrated in cells of one line that palmitic acid supplementation temporarily stimulated synthesis of disaturated choline phosphoglyceride from radiolabeled choline.
