Acquired intravesical ureteral diverticulum: an unusual late complication of ureteroneocystostomy.

An unusual case of late ureteral obstruction following a Politano-Leadbetter ureteroneocystostomy performed 11 years previously is reported. Fibrosis of the distal ureter associated with vascular compression of the extravesical ureter presumably led to development of a ureteral intramural diverticulum. This case illustrates the necessity of careful ureteral placement and emphasizes the importance of close followup for many years, even in asymptomatic patients.

[Ureteral perfusion test in the urodynamic evaluation of the upper urinary tract: critical review and case study].

The authors report on their clinical experience with the use of Whitaker test in 38 patients (27 men, 5 women; age 3 months-66 years). Clinical applications of ureteral pressure-flow test are extensively reviewed. A standardized method with simultaneous recording of bladder pressure and combined video-pressure-flow study are basic points to be considered in order to obtain more objective data.

The morphological basis of ureteral peristalsis. An ultrastructural study of the rat ureter.

The ultrastructure of the normal rat ureter has been studied in order to correlate the morphological findings with the function of the organ. Electron microscopy demonstrates that the ureteral musculature behaves as a functional syncytium which permits the spread of electrical excitation from cell to cell. A scanty number of nerve endings without evidence of autonomic ganglia was found throughout the ureteral layers. These morphological data support the hypothesis that ureteral peristalsis is an essentially myogenic phenomenon, the influence of the autonomic nerve supply being limited to modulating peristalsis and influencing ureteral tonus.

Agenesis of the bladder: a case report and review of the literature.

Agenesis of the bladder is a rare congenital anomaly, recorded most often in stillborn infants. Renal agenesis and other severe malformations are frequently associated. An apparent failure of the infraureteral tissue of the mesonephric duct to develop into trigone and proximal urethra seems to be the cause of this anomaly. We report a case of bladder agenesis with anal atresia, left renal dysplasia, and bicornuate uterus.

Congenital polyp of the prostatic urethra: report on 2 cases.

Congenital polyps of the prostatic urethra are an uncommon cause of obstructive uropathy, infection and/or hematuria in male children. A filling defect localized in the posterior urethra on the voiding cystourethrogram represents the peculiar diagnostic finding. Transurethral resection is the treatment of choice, according to the size of the polyp. Two cases of congenital posterior urethral polyps are reported and the main clinical and radiological features are discussed. This lesion has to be considered in the differential diagnosis of the voiding dysfunction in young boys.

Therapeutical aspects of intrarenal artery aneurysms.

The relative infrequency of aneurysms of the branches of the renal artery produces some indecision as to the surgeon's therapeutical choice. The purpose of this study is to carefully examine the indications for surgical treatment and to select precise therapeutical criteria on the basis of 8 patients in whom aneurysms of the main renal artery or of its branches were diagnosed from 1978 to 1986. In 1 patient, the disease was bilateral with a ruptured main artery aneurysm. 5 aneurysms were treated surgically (the ruptured one by nephrectomy, 2 by ex situ revascularization and 2 by in situ revascularization). In the remaining cases, only periodical controls were performed. In 4 surgically treated patients, angiographic follow-up demonstrated a regular renal revascularization and in 1 patient a thrombosis on the site of the anastomosis. On the basis of personal experience, surgical treatment is required for aneurysms larger than 1.5 cm in size without or with partial calcification, aneurysms occurring in pregnant women or in patients likely to conceive in the future, expanding aneurysms, and renin-mediated hypertension. The introduction of microsurgical techniques and renal preservation makes it possible for the urologist to chose between various therapeutical means for the treatment of intrarenal aneurysms. Reconstruction of anatomical continuity of the arterial supply avoiding unnecessary operative demolishment is feasible.

Sarcomatous carcinoma of the kidney presenting as spontaneous retroperitoneal hemorrhage. Report of a case with immunocytochemical study.

A case of spontaneous perinephric hematoma due to a renal cell sarcomatous carcinoma in a 46-year-old woman is reported. Immunocytochemical analysis for the intermediate-sized filament proteins allowed a specific diagnosis of pseudosarcomatous renal carcinoma. Clinical, radiographic, sonographic and CT findings of spontaneous perinephric hematoma are discussed and the therapeutical modalities are briefly outlined.

[Verrucous carcinoma of the penis. 2 cases and review of the literature].

The Authors report general experience with two cases of penis verrucous carcinoma. Such a neoplasia, slowly evolving and not infiltrating the surrounding tissues, needs deep biopsies for a correct diagnosis. Surgical excision of the tumor is the choice treatment at an early stage: at an advanced stage demolitive therapy consisting in a partial or total penectomy is necessary. Recent findings of frank malignant squamous cell carcinoma arising in verrucous carcinoma raise serious concerns on the use of conservative therapies.

[Psychogenic retention of urine in women: diagnostic and therapeutic problems].

Psychogenic urinary retention is rare event. 6 cases are reported: diagnostic urologic and neurologic work-up is outlined. The final statement is the exclusion of rare or minimal neurogenic causes (silent disk prolapse, early multiple sclerosis and so on) and the evidence of cause-effect relationship between psychic trauma and starting of urinary symptoms are compulsory to join the diagnosis of psychogenic urinary retention.

[Bladder instability in patients with prostatic hypertrophy. Urodynamic study].

A urodynamic study was conducted in 37 patients with benign prostatic hypertrophy before and five months after transvesical prostatic adenomectomy. Prior to the operation, 17 patients had cystometric signs of unstable bladder which persisted after the operation in only two patients. Normal detrusor function was restored in the other 15 patients. The pathophysiological and clinical aspects of bladder neck obstruction in patients with prostatic hypertrophy are discussed.
