[Pseudo-obstruction of the colon (Ogilvie's syndrome) and its significance in the differential diagnosis of ileus of the large intestine].

The authors submit an analysis of a group comprising 33 patients who were in the course of two years (1991-1992) hospitalized as emergency cases on account of ileus of the large bowel. All patients were subjected urgently in addition to a basic clinical examination and a native X-ray of abdomen also to irrigographic examination. On its basis in three patients the diagnosis of pseudoobstruction of the large intestine-Ogilvie's syndrome was established. These three patients were successfully treated by a conservative approach. The author summarizes data from the literature pertaining to this disease and confronts them with his own clinical observation. He draws attention to the fundamental importance of irrigography for the differential diagnosis in patients admitted with sings of obstruction of the colon.

[Peroperative glove damage as a possible risk for the surgical team].

With regard to the relative absence of data in our as well as the world literature on possible peroperative injuries of the surgical team, the authors investigated damage of surgical gloves during operation. They divided operations by types and the structure of the surgical team. It was found that on average 45.5% of the gloves used during operations were damaged.

[Use of the Convatec Wound Manager and Stomahesive flanges in local treatment of external gastrointestinal fistulas].

The authors used in 1989-1992 for local treatment of postoperative external gastrointestinal fistulas the Wound Manager of Convatec Co. This aid ensures perfect protection of the skin, a good access to the fistula and dehiscent wound and accurate removal and measuring of the secretion. The authors used as an alternative method Stomahesive Convatec flanges with permanent suction. In this way during the above mentioned period 16 patients were treated at the Surgical Clinic of the Institute for Postgraduate Medical Training, Faculty Hospital Bulovka in Prague. Thirteen patients were treated by conservative methods only, but also in the remaining three prolonged conservative treatment made the operation possible. The total lethality was 25%. The authors recommend these aids as an integral part of comprehensive treatment of postoperative enterocutaneous fistulas.

[Perianal pyoderma].

Perianal pyoderma (hidradenitis suppurativa-acne conglobata) is a chronic disease which in its progressive form cannot be treated by conservative methods. Extensive suppurative foci must be radically excised incl. all systems of fistulas right down to the fascia and the defects which develop are allowed to heal per secundam or after several weeks they can at a different period of time be covered by a skin transplant. The authors present their 30-year experience with surgical treatment of 39 patients. Treatment was in all instances successful after radical surgical excision. All these patients have fully recovered as regards work capacity as well as social aspects.

[Laparostomy as a part of intensive surgical care].

The presented paper is based on an analysis of 24 cases where as part of comprehensive surgical care the laparostomic procedure was used. In nine patients advanced diffuse peritonitis was involved, nine times the method was applied in patients with haemorrhagic necrotizing pancreatitis, four times in intestinal ischaemia and twice as a temporary closure of an extensive defect of the abdominal wall. In all instances a zipper of Ethicon Co. was used. Eight patients died i.e. lethality was 33%. The main contribution of the method is according to the authors the possibility of easy repetition of the surgical intervention in the abdominal cavity with elimination of septic foci and effective decompression of the abdominal cavity which has a positive impact on ventilation.

[Primary resection of the large intestine].

The authors evaluate their own group of 49 patients subjected in 1982-1990 to primary resection of the large intestine with an immediately established anastomosis. 32 patients (65.31) recovered per primam and were discharged on average after 14.9 days. A disorder of the anastomosis was recorded in 5 patients, i.e. on 10.2%. In the conclusion the authors express conditions essential for the safety of these operations.

[Our attitude to the so-called extended surgical procedures for colorectal carcinoma].

The authors present their experience with 29 extended operations performed on account of tumours of the sigmoid, rectosigmoid and rectum. These operations accounted for 13.4% of all operations on account of operable tumours of the colon and rectum in 1984-1988. In addition to the basic operation one organ was removed 11 times, two organs twice, three organs four times and four organs were removed en bloc twice. Complications occurred in 61% of the patients, the mortality rate was 6.9%. The authors submit an account of recent views published in the literature on the given subject with emphasis on en bloc resection, and conclude that when certain principles are respected, these extended operations are justified.

[Spinocellular carcinoma as a rare complication in pilonidal cyst].

The authors present two cases where on the basis of the pilonidal sinus spinocellular carcinoma developed. Both patients were successfully operated. The first one has been for three years, the second one for eight months without relapse. From the paper ensues that cancerization of the pilonidal sinus must be considered a very serious complication and must be prevented by early surgery.

[Sexual disorders after colorectal operations].

After extensive colorectal operations there are sexual dysfunctions in the patients in 16-100%. In our cohort of 36 men the post-operation sexual disorders were reported in 21.3%. The authors present an anatomical survey of nerve supply of the organs influencing sexual activity emphasizing tactical and technical measures during the operations which could prevent the damage of important nervous structures.
