[Atrophic zoniform nevus].

The first, in our knowledge, case of naevus atrophicus zoniforme in italian literature is here reported. A 13 year old girl had a thirteen years history of involved numerous depressed areas of skin of the right anterior region of the chest and of the right upper arm. The lesions were round, grey-bluish, smooth, depressed, completely asymptomatic. No inflammatory changes were noticed in affected patches nor in surrounding normal skin. The histologic changes were compatible with dermal atrophy: epidermis was quite normal; dermis war very thin; the thickness of hypodermis was increased. Some large nervous structures were visible within deep dermis. Histochemical changes mainly involved elastic fibers, whose thickness was considerably increased. The diagnosis of naevus atrophicus zoniforme is discussed, in comparison to other idiopathic atrophies. In conclusion, the main characters of naevus atrophicus zoniforme, according to literature data, are confirmed in our case: 1) congenital appearance 2) zoniforme distribution 3) dermal atrophy 4) thickening of elastic fibers 5) occurrence of large nervous structures in the dermis.

Generalized eruptive histiocytoma. A clinical, histologic, and ultrastructural study.

A case of adult self-healing histiocytosis, classified as generalized eruptive histiocytoma, is reported. Ultrastructural investigation of the cytoplasm of tumor cells revealed the presence of many dense and laminated bodies. The origin and possible importance of these bodies are described and discussed, and the need for quantitative study of cytoplasmic markers in histiocytic proliferations of the skin is emphasized.

Retinol binding protein in normal human skin and in cutaneous disorders.

Circulating vitamin A is transported by a carrier termed retinol binding protein (RBP). Autoradiography and immunofluorescence studies showed that, in normal human skin, RBP is confirmed exclusively to the dermis. Following oral vitamin A loading, a large increase in dermal RBP but only a very small rise of serum RBP was found. Serum RBP was slightly decreased in eczema patients and markedly decreased in patients with porphyria cutanea tarda. Values in other dermatoses, including psoriasis, were normal.

Darier's keratosis follicularis: an ultrastructural study during and after topical treatment with retinoic acid alone or in combination with 5-fluorouracil.

An ointment containing 0.05% retinoic acid (RA) was applied to the anterior surface of the chest in a patient suffering from Darier's keratosis follicularis. A mixture of two ointments containing equal amounts of 5% 5-fluorouracil (5-FU) and 0.05% RA was used on the dorsum of the same patient. Biopsies were taken 30 days and 70 days after treatment started. Retinoic acid gave rise to an initial disorganisation of desmosomes; the "corps ronds" and "grains" disappeared, the lacunae gradually decreased in size, and the epidermis was practically normal when the clinical manifestations of the skin had disappeared. The damage induced with the RA + 5-FU mixture was more serious, but when the skin lesions relapsed they did so later and in a less severe way than the recurrences observed after treatment with RA alone.

Fine structure of granular cell tumor of Abrikosov.

The case report illustrates the microscopical and ultrastructural findings obtained in a case of Abrikossoff's tumor. On the basis of the light and ultrastructural data a neural histogenesis is suggested.

Studies on dapsone induced haemolytic anaemia. I. Methaemoglobin production and G-6-PD activity in correlation with dapsone dosage.

The present study was undertaken on the hypothesis that methaemoglobin production and haemolytic anaemia following dapsone administration could be ascribed to an impairment of glucose-6-phosphate dehydrogenase-enzymatic activity. Analysis of the kinetic parameters of the G-6-PD (Vmax and KM) was performed in ten patients, normal with respect to G-6-PD, suffering from various dermatoses. It was concluded that haemolytic anaemia after dapsone therapy is not due to a functional impairment of the enzyme. The close relationship between dapsone dosage, methaemoglobin production and anaemia make reasonable the hypothesis that a toxic dapsone derivative (DDS-NHOH) could be responsible for the methaemoglobin formation and the haemolytic anaemia.

Immunohistochemical study of dermal cell popoulation in psoriatic skin.

As demonstrated by means of an immunoperoxidase technique, the great majority of the infiltrating mononuclear cells located in the superficial and middle dermis of psoriatic skin consisted of T-lymphocytes. On the contrary, immunoglobulin bearing cells, investigated by means of both immunoperoxidase and immunofluorescence methods, were scarce; IgA and IgG cells were predominant, whilst IgM cells were scarcely represented. Macrophages and monocytic cells were observed in the superficial dermis by means of the alpha-naphtyl-acetate-esterase method, in frozen sections. The role of the above mentioned cells and the significance of their reciprocal relationships still remains in discussion.

In situ identification of mononuclear cells infiltrating cutaneous carcinoma: an immuno-histochemical study.

The present study was undertaken in order to investigate the exact nature of the cells surrounding cutaneous tumours. Formalin-fixed, paraffin-embedded sections were tested with a horse antihuman T lymphocyte serum and with IgG, IgA and IgM rabbit antihuman serum. All the sections were respectively treated with rabbit anti-horse or swine antirabbit peroxidase-labelled serum. The advantages offered by the immunoperoxidase technique are briefly discussed. T cells are in overwhelming proportion in comparison with IgG, IgA and IgM bearing cells. This seems a further demonstration that mononuclear infiltrate surrounding cutaneous carcinoma mainly represents a cell-mediated immune response.
