Evaluation of a patient with central retinal vein occlusion.

Patients suffering a central retinal vein occlusion should be evaluated for conditions that promote and/or contribute to the formation of a thrombosis in the central retinal vein. These conditions include alterations of blood flow, altered viscosity of the blood, abnormalities of coagulation, and abnormalities of vessel walls. Patients with either a partial or complete central retinal vein occlusion have similarities in the age of onset, associated systemic findings, and laboratory data. At the time of onset, 90% of patients are older than 50 years. Seventy-four percent of patients will have symptoms or signs of associated systemic vascular disease (ie, hypertension, arteriosclerotic heart disease, and diabetes mellitus.) The most common laboratory findings include elevated blood lipids, abnormalities of glucose tolerance testing, and abnormalities of protein chemistries. Evaluation should include both a complete ocular and medical examination with appropriate laboratory tests and special studies. Assessment of central retinal venous pressure may be helpful in identifying those patients who have a severe, complete thrombosis and are at significant risk to develop anterior segment neovascularization.

Sympathetic ophthalmia.

A 29-year-old woman developed severe ocular manifestations of sympathetic ophthalmia on the day after enucleation of a blind, painful eye, and four weeks after a penetrating ocular injury. She was observed for one year with fundus photography, fluorescein angiography, and electrophysiologic tests. Nystagmus, a rare systemic manifestation of sympathetic ophthalmia, was noted early in the course of her disease and was accompanied by vertigo, truncal ataxia, and cerebrospinal fluid pleocytosis. Enucleation and intensive corticosteroid treatment resolved the process; however, she has experienced recurrences in the two-year follow-up period.

Localization of suspected intraocular foreign bodies with a modified Delta 2020 scanner.

Metallic foreign bodies were localized in seven patients using a modified Delta 2020 scanner. In six of these seven, we compared the localization obtained by sector CT scan with ultrasound. The two methods were found to be complementary, with the sector CT scan giving more positive and more easily interpreted localizations, and ultrasonography giving more detailed soft tissue diagnosis. The new sector CT scanner greatly raised the level of confidence in the localization of metallic foreign bodies; however, we recommend the combined use of both techniques for a complete evaluation.

Macular and paramacular detachment of the neurosensory retina associated with systemic diseases.

We have reviewed 168 cases of angiographically proved SRD of the macular region of unknown etiology that were seen at The Cleveland Clinic Foundation. Thirty-seven percent of these patients had documented coincident systemic diseases. Of that group, patients with autoimmune disease appeared to have ocular symptoms during the active phase of the systemic illness. Nine cases are reported characterizing the coincidence of exacerbation of systemic disease with recurrent serous detachment of the neurosensory retina. It is our hypothesis that in patients with autoimmune disease, the choroiditis is secondary to the damage caused by immune complexes. Thus, in these patients, SRD was a manifestation of the systemic illness.

Silicone sponge implant in combination with episcleral implant for retinal detachment surgery.

The results of retinal detachment surgery performed in 50 patients utilizing a silicone sponge episcleral implant combination placed beneath an encircling band have been reviewed. Anatomical reattachment has been achieved in 48 (95%) procedures and complications associated with this technique have been infrequent. The modification described emphasizes the stability and permanency of the sponge episcleral implant combination placed beneath the encircling band without the placement of additional buckling or anchoring sutures. This procedure is used for the treatment of communicating radial folds and fishmouth tears in primary operations or reoperations.

The natural course of central retinal vein occlusion.

Thirty-five patients with central retinal vein occlusion had an extensive medical and laboratory evaluation to identify possible etiologic factors. We describe the natural course of the two forms of retinal venous occlusive disease, hemorrhagic retinopathy and venous stasis retinopathy, by observing a group of 25 untreated patients from one to eight years. Systemic vascular disease was most commonly associated with retinal venous occlusive disease. The visual prognosis is favorable in venous stasis retinopathy and extremely poor in hemorrhagic retinopathy. However, two of ten patients with venous stasis retinopathy developed hemorrhagic retinopathy with poor visual outcome.

The natural course of active choroidal lesions in the presumed ocular histoplasmosis syndrome.

Eighty-two patients and 106 eyes with the POHS have been followed for one year or longer. Untreated active choroidal lesions were divided into groupings based on the distance from the center of the CFZ and the reltionship to the border of the CFZ. Twenty-three percent of untreated active choroidal lesions that were located inside the CFZ achieved 20/20 to 20/40 vision. Sixty-three percent of untreated active choroidal lesions that were located outside of the CFZ achieved a vision of 20/20 to 20/40. The active choroidal lesions with the best visual prognosis were 0.25 to 0.5 disc diamterers, demonstrated no growth, and had little or no associated choroidal bleeding. Activation of choroidal lesions in the fellow eye at the site of preexisting atrophic scars was documented in 21% of the eyes. One fellow eye developed a de novo active choroidal lesion. Eighteen percent of the cases had structurally congested nerve heads characterized by slight elevation of the disc and no physiologic depression; 3% of the eyes had drusen of the optic nerve. A relationship may exist between peripapillary scarring in the POHS and these nerve head changes.

Dural arterio-venous malformations involving the cavernous sinus: a case report.

A case of dural arteriovenous malformation involving the cavernous sinus is reported. The patient was successfully treated with selective embolisation of the fistula. These patients constitute a distinct neuro-ophthalmological syndrome which may be very difficult to diagnose clinically because of the subtle signs and symptoms they present with. Angiography is necessary to make diagnosis and to differentiate these patients from those with the more common carotidcavernous fistulae.
