Attention disorders in schizophrenia.

In order to clarify the feature of attention disorders in schizophrenia, two tasks (X and AX) of the Continuous Performance Test (CPT) were given to 36 schizophrenic patients and 25 healthy controls. The schizophrenic patients performed considerably less well than normal controls. Performances were expressed in indices such as omission and commission errors, reaction time and discriminability. Among errors, omission in both the X and AX tasks was found to be an index that varied in reaction time, indicating difficulty in sustaining attention. Omission was found to act as a state-dependent index of schizophrenia in relation to inattentiveness in clinical settings and emotional disturbance in interpersonal situations. Commission, in contrast, differed between the two tasks, acting as an index of disinhibition of reaction in the X task, and of disturbance of the capacity and allocation of attention in the AX task. Commission was also thought to be an index related to thought disorder during episodes of acute schizophrenia, that is, it reflects an essential pathology in schizophrenia.

Quality of life and lifestyle among city dwellers participating in the medical programs.

In order to offer health education to the general population, taking into account not only their physical health but also their QOL (quality of life), it is necessary to clarify what guidance should be provided in the sight of QOL. In this study, the relationship between QOL and lifestyle was investigated in 615 Japanese (402 males, 213 females; ranging in age from teens to nineties; mean age, 43.4 +/- 16.3 years) undergoing medical checkups. After informed consent was obtained, subjects completed a questionnaire incorporating the Medical Outcome Study 36-item Short Form Health Survey as a QOL scale in addition to questions concerning individual and lifestyle factors. The results showed that most individual factors and lifestyle factors evaluated were related to QOL. In particular, the physical health QOL of elderly subjects, females, and single subjects, was lower than that of other subjects. Moreover, subjects who ate 'irregular meals' and had 'less than 6 h sleep' reported lower mental health QOL scores than those who ate 'regular meals' and had '6 h or more sleep'. Smoking and alcohol consumption were not as significantly related to QOL as anticipated. Taking these findings into account, it is clear that it is necessary to offer medical and health services focusing on meals and sleep for medical program attendants in order to improve QOL.

[Reliability and validity of the functional assessment of chronic illness therapy-spiritual (FACIT-Sp) Japanese version in cancer patients--a preliminary study].

The reliability and validity of the Functional Assessment of Chronic Illness Therapy-Spiritual well-being (FACIT-Sp), Japanese version, which was developed in the United States for a simple survey of QOL related to spirituality, was evaluated. The survey simultaneously covered FACIT-Sp and HADS, an anxiety and depression scale, using 64 cancer patients as the subjects of the study. In the evaluation of its intrinsic conformity, the Cronbach alpha reliability coefficient of the sub-scale ranged from 0.80 to 0.91 and the evaluation of validity by factor analysis indicated satisfactory factorial appropriateness. The correlation coefficients for the sub-scales were all below 0.7, indicating that each scale is independent. The correlation with HADS depression and anxiety scales was -0.54 and -0.56, respectively, showing that the emotional and spiritual scales are independent of each other. The findings suggest that The Japanese version of FACIT-Sp is associated with satisfactory reliability and validity and is an effective tool in the study of spirituality among cancer patients.

Significant linkage to chromosome 22q for exploratory eye movement dysfunction in schizophrenia.

A genome-wide scan for a locus responsible for exploratory eye movement (EEM), which is quantitative and can be disturbed in association with schizophrenia, was performed. A 10-cM resolution genome-wide linkage analysis of the EEM disturbance with 358 highly polymorphic microsatellite markers in 38 nuclear families with 122 members (38 probands, 47 sibs, and 37 parents) including 58 sib-pairs yielded the suggestive linkage to the GCT10C10 marker on chromosome 22q11.2 (LOD = 2.48). Dense mapping with additional markers around the GCT10C10 marker yielded evidence for significant linkage between EEM disturbance and markers D22S429 and D22S310 on chromosome 22q12.1 (LOD score of 4.63) with suggestive evidence for the chromosome region 22q11.2-q12.1. Our findings suggest that a relatively small number of loci may control the schizophrenia-related quantitative EEM trait. We believe that identifying gene(s) on chromosome 22q associated with the EEM phenotype may forward our understanding of the etiology of schizophrenia.

Regional cerebral blood flow in depressed patients with white matter magnetic resonance hyperintensity.

BACKGROUND: Functional neuroimaging studies have consistently demonstrated decreased regional cerebral blood flow (rCBF) or metabolism in the frontal lobe, temporal lobe, or anterior cingulate gyrus of depressed patients. On the other hand, white matter hyperintensity as defined by magnetic resonance imaging (MRI) has been the most consistently replicated finding in structural neuroimaging studies on depression; however, these functional and structural neuroimaging findings of depression have not been well integrated. We aimed to clarify the possible associations of MRI-defined subcortical hyperintensities with rCBF changes in depressed patients. METHODS: Twelve depressed patients with subcortical hyperintensities defined by MRI, 11 depressed patients without MRI hyperintensities, and 25 healthy volunteers underwent 99mTc ECD SPECT. Group comparisons of their rCBF and correlation analysis between MRI hyperintensity and rCBF in patients were performed with a voxel-based analysis using statistical parametric mapping (SPM) software. RESULTS: Depressed patients showed decreased rCBF compared with control subjects in the frontal lobe, temporal lobe, and anterior cingulate gyrus whether subcortical hyperintensity existed or not; however, the patients with MRI hyperintensity showed decreased rCBF in the thalamus, basal ganglia, and brainstem in addition to cortical areas. Further, the score for white matter hyperintensity correlated negatively with rCBF in subcortical brain structures, including the thalamus and right basal ganglia. CONCLUSION: Our study indicates that depressed patients with MRI hyperintensities may have dysfunction in subcortical brain structures in addition to dysfunction in the fronto-temporal cortical structures.

Brain perfusion SPECT in patients with corticobasal degeneration: analysis using statistical parametric mapping.

We sought to determine the characteristics of brain perfusion in patients with corticobasal degeneration (CBD) using single photon emission computed tomography (SPECT) and statistical parametric mapping (SPM). Thirteen patients with CBD and 10 age-matched healthy volunteers were examined using brain perfusion SPECT and (99m)Tc-ethyl cysteinate dimer (ECD). Regional cerebral blood flow (rCBF) measurements were performed noninvasively using a graphic analysis method. Both the absolute rCBF data (raw data) and the adjusted rCBF data (global CBF normalized to 50 ml/100 g/min) were used in the SPM analysis. A significant decrease in the absolute rCBF was observed in extended areas of the brain in CBD patients compared to the control group. These areas included the frontal, parietal, and temporal cortices; basal ganglia; thalamus; and pontocerebellar regions. Our results suggest that the extent of the reduced rCBF region in CBD patients is more widespread than previously reported.

Exploratory eye movements during the Benton Visual Retention Test: characteristics of visual behavior in schizophrenia.

In order to investigate the relationship between the behavioral patterns and clinical symptomatology in schizophrenia, exploratory eye movements of schizophrenic subjects and healthy controls during the Benton Visual Retention Test were examined using an eye-mark recorder. The results were as follows: (i) with card 1, the number of eye fixations of schizophrenic subjects was fewer, and the total and mean eye scanning lengths of schizophrenic subjects were shorter than those of healthy controls; (ii) with card 3, almost none of the schizophrenic subjects looked at the blank area opposite the peripheral figure on the right; (iii) with cards 3, 5, 6 and 9 there were some schizophrenic subjects who did not look at the peripheral figures; (iv) with card 6, many of the schizophrenic subjects made stereotypical movements; (v) with card 9, schizophrenic subjects used a narrow vertical gaze to look at the large figure on the right. Based on these characteristics among the schizophrenic subjects themselves, factors such as longer eye scanning length, looking at peripheral figures without fail, and not making stereotypical movements were reflected directly in the results of the Benton test, while there was no relationship between the width of the vertical gaze and the Benton results. Correlations between visual behavior and some psychiatric symptoms were observed. The visual behavioral patterns of schizophrenic subjects were various according to the characteristics of the Benton figures, while those of normal subjects were always almost the same. It was suggested that these results were caused by disturbances of the mental attitude of schizophrenic subjects toward objects or environments.

Relationship between attention and arousal level in schizophrenia.

The purpose of the present paper was to clarify the link between the attention and arousal level that supports the basis of the cognitive dysfunction in schizophrenia, by investigating the relationship between the simple reaction time and the closed-eye eye movements in 30 patients with schizophrenia and 20 healthy controls. In terms of closed-eye eye movements during the simple reaction time test, healthy controls showed an increase of s-type (small and slow) eye movements after the end of the preparatory interval (PI) in both regular and irregular series, while the patients with schizophrenia, particularly those in whom the cross-over phenomenon was observed, showed no changes and maintained a hyperarousal level during the regular PI test. These results indicate that the patients with schizophrenia could not maintain appropriate attention during the burden tasks and their hyperarousal level persisted. It is therefore suggested that there is a close relationship between attentional deficit and hyperarousal among patients with schizophrenia.

Influence of arousal level for functional magnetic resonance imaging (fMRI) study: simultaneous recording of fMRI and electroencephalogram.

Simultaneous recording of functional MRI (fMRI) and electroencephalogram (EEG) has been applied to several clinical fields, making it possible to monitor the arousal level of the subject during a cognitive task. The study confirmed that activated cerebral areas were different between high and low arousal levels during the smooth-pursuit eye movement task. When arousal level was high, activations in the parietal eye field, frontal eye field (FEF), supplementary eye field (SMA), visual fields (V1) and occipito-temporal junction (V5) were found. In contrast, when arousal level was low, activations were found only in V1 and FEF. The results indicate that the monitoring of the arousal level of subjects using fMRI and EEG recordings simultaneously is crucial for detecting cortical activations during a cognitive task.

Early detection of postoperative delirium and confusion in a surgical ward using the NEECHAM confusion scale.

The early detection and prevention of postoperative delirium and confusion has become an important issue in surgical ward management. With the aim of determining an objective technique for early detection of delirium, 64 patients admitted to a surgical ward before surgery were examined using the NEECHAM confusion scale. On the 2nd postoperative day they were tested again and divided into 3 groups on the basis of their NEECHAM scores: high risk, low risk, and no risk, for confusion. The trends of the NEECHAM scores in the 3 groups were compared, and the relationship between the NEECHAM scores and suspected clinical risk factors for delirium was investigated. Use of NEECHAM scaling enabled medical staff to identify cases of possible confusion early, indicating that the NEECHAM confusion scale should be useful for the detection of postoperative delirium and confusion in the surgical ward.

Psychological state, quality of life, and coping style in patients with digestive cancer.

The purpose of this study was to assess the relationships between psychological characteristics such as anxiety and depression, quality of life (QOL) and coping style among patients with digestive cancer. The subjects were 85 in-patients who were scheduled to undergo initial surgery for gastrointestinal cancer. The following psychological tests were administered: Japanese versions of the Hospital Anxiety and Depression Scale, Zung's Self-Rating Depression Scale, the European Organization for Research and Treatment of Cancer (EORTC) QLQ C30 and the Coping Inventory for Stressful Situations. The first 3 tests were performed on three occasions: before surgery, before discharge and 6 months after discharge. The results showed that there was no change over the 3 test administration days for the average scores of anxiety and that the scores of depression increased from before surgery to before discharge and did not return to presurgery levels at 6 months after discharge. Changes in each subscale score of the EORTC QLQ C30 across the 3 days displayed two typical trends. Relationships between the abovementioned trends and individual coping styles showed that the higher the score of "emotion-oriented coping style," the greater the deterioration in QOL subscales. It was suggested that focusing on a patient's coping style, particularly emotion-oriented coping style, is important and that patients likely to adopt a more emotion-oriented coping style should receive special consideration.
