Prevalence of Helicobacter pylori infection and risk of upper gastrointestinal ulcer in patients with rheumatoid arthritis in Japan.

We evaluated the prevalence of Helicobacter pylori infection and the association of H. pylori infection and/or nonsteroidal anti-inflammatory drug (NSAID) use with upper gastrointestinal (UGI) ulcers in a cohort of Japanese patients with rheumatoid arthritis (RA). Using the clinical database of the cohort of RA patients and the serum titers of H. pylori antibody, 1815 patients were analyzed. Clinical data were successfully collected for 1529 patients over 2 years, and the history of NSAID use and the occurrence of newly diagnosed UGI ulcer were ascertained by patient self-reports and confirmed by their medical records. A total of 871 patients (49.3%) were H. pylori antibody-positive. Rates of positivity for H. pylori in patients with and without NSAID use were 47.5% and 54.7%, respectively (odds ratio = 0.75, 95% confidence intervals [CI]: 0.58-0.96). The incidence of newly diagnosed UGI ulcer was 0% in the H. pylori-/NSAID- group, 1.24% in the H. pylori-/NSAID+ group, 1.06% in the H. pylori+/NSAID- group, and 3.46% in the H. pylori+/NSAID+ group. The odds ratios of H. pylori infection and NSAID for the occurrence of new UGI ulcers after adjusting for age and sex were 2.97 (95% CI: 1.19-7.38) and 4.31 (95% CI: 0.57-32.4), respectively. Although the prevalence of H. pylori antibody was low in patients with RA compared with that in healthy Japanese individuals, H. pylori infection was a significant risk factor for UGI ulcer in patients with RA.

Recto-peritoneal fistula following transperineal prostate biopsy.

A recto-peritoneal fistula is an extremely rare complication after prostate biopsy. We report herein on a peritonitis arising from a recto-peritoneal fistula 5 days after undergoing prostate biopsy. To our knowledge, this is the first case of recto-peritoneal fistula following transperineal needle biopsy of the prostate in the published literature.

Superficially spreading cholangiocarcinoma.

Cholangiocarcinoma of a raised, elevated type, not an infiltrative type, may be accompanied with superficial spread in the contiguous mucosa. We report a case of an extrahepatic cholangiocarcinoma that showed a specific cholangiographic finding of extensive superficial spread. The patient was a 62-yr-old man. Cholangiography revealed an elevated lesion in the middle of the bile duct, which was surrounded by irregularity of the bile duct wall extensively. We accurately diagnosed the tumor extent by percutaneous transhepatic cholangioscopy and achieved radical resection without hepatic resection. The margins were tumor-negative by microscopy. In conclusions, it is important to accurately diagnose the extent of superficial spread preoperatively by cholangioscopy and biopsy, and decide the resecting lines to make margins tumor-negative.

Ankle skeletal injury predictions using anisotropic inelastic constitutive model of cortical bone taking into account damage evolution.

The most severe ankle skeletal injury called pilon fractures can cause long term disability and impairment. Based on previous experimental studies, the pilon fractures are regarded as caused by a high-energy compressive force in the ankle joint and affected by a muscular tension force generated by emergency braking. However, quantitative injury criteria for the pilon fractures are still unknown. More accurate prediction of bone fractures in the distal tibia using a FE model of human lower leg can help us know the quantitative injury criteria. Therefore we newly proposed an anisotropic inelastic constitutive model of cortical bone including damage evolution and then implemented it to a FE code, LS-DYNA. The proposed model successfully reproduced most of anisotropy, strain rate dependency, and asymmetry of tension and compression on material and failure properties of human femoral cortical bone. However, the simplified model using an isotropic elasto-viscoplastic material, which has been used in previous studies, did not reproduce the characteristic features of the cortical bone. Two series of validation on axial impact cadaver tests for the foot and ankle indicate that the proposed model predicts the pilon fractures more accurately than the simplified model. Parametric studies on footwell impacts and pedal impacts for the foot using the proposed model show that the severity of the pilon fractures increases when the foot sustains normal and heel impacts with the impact velocity of 5 m/s and the pedal hits the forefoot with the impact velocity of 3 m/s regardless of the muscular tension force.

Urinary 8-hydroxy-2'-deoxyguanosine excretion as a biomarker for estimating DNA oxidation in patients undergoing external radiotherapy and/or brachytherapy.

Urinary 8-hydroxy-2'-deoxyguanosine (8-OHdG) is considered to be a biomarker of cellular oxidative stress and to be associated with carcinogenesis. To estimate the oxidative stress caused by radiotherapy, we used the enzyme-linked immunosorbent assay (ELISA) to monitor urinary 8-OHdG levels in 72 patients undergoing radiotherapy. Subjects consisted of 23 breast cancer patients undergoing post-operative external radiotherapy for breast conserving therapy, 12 tongue and seven prostate cancer patients undergoing interstitial radiotherapy, 19 esophageal cancer patients undergoing external radiotherapy and chemotherapy, and 11 cervical cancer patients undergoing external radiotherapy and brachytherapy with or without chemotherapy. Before radiotherapy, cervical cancer patients showed a higher urinary 8-OHdG level (16.0+/-8.8 ng/mg) than breast cancer patients (5.3+/-5.5 ng/mg, p=0.001). In the other three groups, urinary 8-OHdG levels were in the medium range (prostate cancer patients: 6.6+/-6.3 ng/mg; esophageal cancer patients: 8.8+/-11.1 ng/mg; tongue cancer patients: 10.2+/-6.7 ng/mg). Radiotherapy did not cause changes in the excretion level of urinary 8-OHdG in patients with breast, esophageal and tongue cancer. However, radiotherapy reduced 8-OHdG excretion levels in patients with cervical cancer, whereas interstitial radiotherapy transiently increased these levels in patients with prostate cancer.

Pelvic irradiation-induced eosinophilia is correlated to prognosis of cervical cancer patients and transient elevation of serum interleukin 5 level.

PURPOSE: To determine the significance of eosinophilia and serum interleukin 5 (IL-5) levels induced by radiotherapy in patients with gynecological cancer. MATERIALS AND METHODS: First, we examined changes in the number of eosinophils before and during radiotherapy in 145 patients with cervical cancer who underwent external radiotherapy and brachytherapy. Eosinophilia was defined as > or =10% eosinophils or > or =500 eosinophils/mm3. Next, we determined serum IL-5 levels prospectively in 20 patients, comprising 18 cases of cervical cancer, one of uterine corpus cancer, and one of vaginal cancer. RESULTS: Eosinophilia was seen in 88 patients (61%). Patients with advanced disease stage IV showed 32% eosinophila, lower than those of stage I (63%), II (69%), and III (65%) disease patients (p=0.03). The 5-year cause-specific survival rate was 76% for patients with eosinophilia (+) and 53% for patients without eosinophilia (p=0.0003). Thus, eosinophilia correlates to stage classification and a good prognosis. Of the 20 patients studied prospectively, the six with elevated serum IL-5 level at one week (6/20=30%) all showed eosinophilia (100%), whereas only six of the other 14 patients had eosinophilia (43%, p=0.04). CONCLUSION: Radiotherapy induced transient elevation of serum IL-5 levels and is related with eosinophilia, which is correlated to prognosis.

Long-term hormone replacement therapy delays the age related progression of carotid intima-media thickness in healthy postmenopausal women.

OBJECTIVES: Carotid intima-media thickness (IMT) is an appropriate intermediate end point to investigate clinically relevant effects on atherogenesis. The study objective was to clarify whether long-term hormone replacement therapy (HRT) modifies the progress of age-related IMT in healthy postmenopausal Japanese women. METHODS: One hundred and eighty-eight healthy postmenopausal women aged 42-69 years were recruited into the retrospective study. IMT was measured by B-mode real-time ultrasound in the following three groups of patients. One hundred and fifteen women who were prescribed estrogen plus progestin or estrogen alone were classified into two groups according to the HRT treated period: short-term (<2 years of treatment, n = 52) and long-term (> or =2 years, n = 63) HRT groups. The third group consisted an age-matched women (n = 73), who were never treated with HRT (non-HRT group) as a control. RESULTS: Each group was divided into three subgroups according to age: < or =49 years, 50-59 years and 60 years or older. IMT in patients of age > or =60 years in the non-HRT group was 0.607 +/- 0.064 mm and was significantly higher compared with that in the other two age subgroups of non-HRT patients (< or =49 years: [0.495 +/- 0.051 mm; 50-59 years: 0.505 +/- 0.068 mm) (P < 0.05). In the short-term HRT group, IMT of > or =60-year-old-subjects (0.588 +/- 0.074 mm) was also significantly higher compared with that in the other two age subgroups (< or =49 years: 0.480 +/- 0.034 mm; 50-59 years: 0.511 +/- 0.062 mm). However, in the long-term HRT group, IMT was not significantly different among the three age subgroups. There was a significant relationship between IMT and age in non-HRT (r = 0.594, P < 0.0001) and short-term HRT (r = 0.542, P < 0.001) groups, but no significant relationship was observed in the long-term HRT (r = 0.195 , P = 0.1266) group. CONCLUSIONS: In long-term HRT, more than 2 years may delay the age-related increase in IMT in healthy postmenopausal Japanese women.

An optimization algorithm of dose distribution using attraction-repulsion model (application to low-dose-rate interstitial brachytherapy).

PURPOSE: To optimize dose distribution for prostate cancer in low-dose-rate interstitial brachytherapy, we have developed a new algorithm named the Attraction-Repulsion Model. The purpose was to find the optimal source configuration. METHODS AND MATERIALS: The Attraction-Repulsion Model is used to optimize the dose distribution by finding the best seed configuration. We arranged grids at intervals of a certain space inside and established target and critical organs as areas of interest. We can make an attribute for grids, and the grids show attraction or repulsion depending on dose delivered from source. Source position is changed by the forces that the grids impose to the sources. A calculation was done repeatedly until the attraction and repulsion forces reached a balance. The optimal configuration was established when the sources reached a stable distribution in time. To evaluate the optimization plan, dose-volume histograms were used. RESULTS: Source configuration can be optimized automatically. The calculation time was approximately 5 min. The V100, V150, V200, and D90 of the target were 95%, 39%, 9%, and 157 Gy, respectively. V150 of the urethra and V80 of the rectum were 2% and 0%, respectively. CONCLUSION: This method can optimize the dose distribution objectively.

Outcome of duodenum-preserving resection of the head of the pancreas for intraductal papillary-mucinous neoplasm.

BACKGROUND: The standard surgical procedure for intraductal papillary-mucinous neoplasm of the pancreatic head is pylorus-preserving pancreatoduodenectomy. A less extensive resection may be justified because most intraductal papillary-mucinous neoplasms are benign or of low-grade malignancy. AIMS AND METHODS: The outcome of duodenum-preserving pancreatic head resection with preservation of the main bile duct was evaluated retrospectively in 13 patients with a branch-type intraductal papillary-mucinous neoplasm in the head of the pancreas and with a median (range) follow-up of 60.0 (0.3-99.5) months. RESULTS: Post-operative complications included anastomotic leakage (n=3), bile duct perforation (n=1), intra-abdominal bleeding (n=3), delayed gastric emptying (n=2) and death (n=2). All the resection margins were clear of tumour on histological examination. Ten of 11 patients maintained over 90% of their pre-operative body weight. Glucose tolerance improved in 4 of 11 evaluable patients, was unchanged in 6 and worsened in 1 patient. Biliary scintigraphy showed that bile flow was delayed compared with that before surgery (8.8 +/- 1.1 vs. 19.6 +/- 4.6 min; p = 0.03). Neither recurrence nor metastasis was observed. CONCLUSION: The results of duodenum-preserving pancreatic head resection for branch duct-type intraductal papillary-mucinous neoplasm were satisfactory and provided a good quality of life.

Lymph node metastasis of early oral tongue cancer after interstitial radiotherapy.

PURPOSE: To examine the prognostic factors for lymph node metastasis after brachytherapy for early (T1-T2N0M0) oral tongue cancer. METHODS AND MATERIALS: We reviewed the records of 571 patients (500 low dose rate and 71 high dose rate) treated at Osaka University Hospital between 1967 and 1999. RESULTS: Patients with lymph node metastasis had tumor with an average diameter of 26 +/- 8 mm and a thickness of 9 +/- 5 mm; for patients without lymph node metastasis, the corresponding dimensions were 23 +/- 8 mm and 7.5 +/- 4 mm (p = 0.0004 and 0.001, respectively). After 5 years, the ulcerative (48%) and indurative/infiltrative (39%) types showed a higher ratio of nodal involvement than the exophytic (31%) and superficial (19%) types (p <0.0001). Multivariate analysis showed ulceration (p = 0.006) and a thickness of <or =6 mm (p = 0.04) to be statistically significant predisposing factors for lymph node metastasis. The lymph node control rate was 68% in 1967-1979, 71% in 1980-1990, and 66% in 1990-1999; the corresponding successful salvage rates for lymph node metastasis were 43%, 33%, and 58% (p = 0.04). CONCLUSION: The appearance of the tumor, especially the presence or absence of ulceration and the diameter and thickness, are useful prognostic indicators for lymph node metastasis. Although the rates of lymph node metastasis did not change, the salvage outcome for recurrence after interstitial radiotherapy has recently improved.

Ischemic gastropathy after distal pancreatectomy with celiac axis resection.

PURPOSE: Stomach-preserving distal pancreatectomy with en bloc resection of the celiac, common hepatic, and left gastric arteries is a radical operation performed for locally advanced cancer of the pancreatic body. However, it is not known whether the collateral pathways that develop immediately from the superior mesenteric artery to the gastroduodenal and hepatic arteries provide sufficient blood flow to support the hepatobiliary system and the stomach. This article examines the ischemic gastropathy that can occur after this procedure and identifies the predisposing conditions. METHODS: Between 1997 and 2001, nine patients underwent stomach-preserving distal pancreatectomy with en bloc resection of the celiac, common hepatic, and left gastric arteries. Concomitant resection of the right gastric artery or gastroduodenal artery was performed due to cancer infiltration in three patients. RESULTS: Irregular, shallow, and wide ulcerations thought to be ischemic in origin developed in these three patients, but all the ulcerations healed in 1-2 weeks with antiulcer medication. None of the other six patients had evidence of gastric ischemia. CONCLUSIONS: Ischemic gastropathy is rare after distal pancreatectomy with celiac axis resection alone; however, division of additional arteries supplying the stomach may predispose to ischemic gastropathy.

Role of hepatectomy in the treatment of hilar bile duct carcinoma.

PURPOSE: To clarify the role of hepatic resection in the surgical treatment of hilar bile duct carcinoma. METHODS: Between 1980 and 1997, 68 patients underwent surgery for hilar bile duct carcinoma. The patients were divided into a hepatectomy group ( n = 40) and a nonhepatectomized group ( n = 28) depending on whether they underwent resection of the bile duct confluence in combination with hepatectomy, or alone, respectively. Background data, operative morbidity and mortality, and survival were retrospectively compared between the two groups. RESULTS: There were no significant differences in morbidity and mortality, or in postoperative survival between the two groups (the 5-year survival rates being 20.6% in the hepatectomized group and 7.1% in the nonhepatectomized group; P = 0.0806). However, patients who underwent curative resection had significantly better postoperative survival than those who underwent noncurative resection ( P = 0.048). Hepatectomy provided a significantly better cancer-free margin than bile duct resection alone ( P = 0.0296). CONCLUSIONS: Although a countermeasure must be taken to decrease mortality, the introduction of hepatectomy with bile duct resection would provide a better cancer-free surgical margin than bile duct resection alone for hilar bile duct carcinoma. Curative resection contributed to long-term survival in this series.

Gastric tube-preserving pancreatoduodenectomy after esophagectomy and reconstruction using the gastric tube: report of a case.

We herein present the case of an intraductal papillary mucinous tumor of the pancreas resected by a gastric tube-preserving pancreatoduodenectomy in a patient who underwent an esophagectomy and reconstruction using the gastric tube. To preserve the gastric tube, the gastroduodenal artery and right gastroepiploic vein must be successfully skeletonized and preserved. In the present case, the vascular anatomy was clarified not only two-dimensionally with the use of selective abdominal angiography, but also three-dimensionally with the use of helical computed tomography. After precisely identifying the location of the tumor in relation of the blood vessels, it was confirmed that a gastric tube-preserving pancreatoduodenectomy was feasible. A detailed analysis of the vascular anatomy should be carried out by helical computed tomography preoperatively. When the radicality of tumor resection is not compromised, a gastric tube-preserving pancreatoduodenectomy can be recommended, due to the fact that it is considered to be less invasive.

Embolization of the replaced common hepatic artery before surgery for pancreatic head cancer: report of a case.

We report the case of a patient with pancreatic head cancer, whose replaced common hepatic artery (RCHA) arose from the superior mesenteric artery (SMA). We performed preoperative embolization of the RCHA, after which the liver blood flow was well maintained by the left gastric artery. The patient underwent a radical operation involving en bloc resection of the RCHA without any serious complications.

Classification of systemic sclerosis in the Japanese population based on rapid progression of skin thickening.

In general, patients with systemic sclerosis (SSc) have been classified in two clinical subsets (diffuse and limited) based on the extent of skin thickening, and the extent of skin fibrosis is closely related to the severity of organ involvement and mortality rates. In addition, there is a rapid progression of skin thickening in diffuse cutaneous SSc (dcSSc). In this study, we classified a novel subset of SSc based on the rate of skin thickening progression, and evaluated the relationship of the subset to disease severity and laboratory markers. Thirty-three patients with dcSSc were included in the study. Participants who had a modified Rodnan's total skin thickness score higher than 15 points within a year from the first symptoms were defined as having rapidly progressive SSc (RPSSc; group 1), while all the other dcSSc patients were defined as having non-RPSSc (group 2). The frequencies of interstitial lung disease and renal crisis were significantly higher in group 1 than in group 2. The frequencies of anti-topoisomerase I antibody (anti-topo I) and anti-SS-A antibody were significantly higher in group 1 than in group 2. Conversely, the frequency of anticentromere antibody was significantly higher in group 2 than in group 1. This study demonstrates the clinical significance of a new subset of SSc (defined as RPSSc), which is characterized by a markedly rapid progression of skin thickening. This new classification may be useful for predicting the prognosis of SSc at an early stage, so that patients who should receive aggressive therapy with immunosuppressants might be identified.

Forty consecutive resections of hilar cholangiocarcinoma with no postoperative mortality and no positive ductal margins: results of a prospective study.

OBJECTIVE: Our objective was to perform a prospective study of surgical treatment of hilar cholangiocarcinoma according to newly established guidelines for performing safe and curative resections. SUMMARY BACKGROUND DATA: The poor survival rate after resection of hilar cholangiocarcinoma is considered to be mainly the result of in-hospital death and positive ductal margins. METHODS: Between July 1999 and December 2002, 40 of 42 surgically explored patients with hilar cholangiocarcinoma underwent resection. They were managed with preoperative biliary decompression, portal embolization, cholangiographic evaluation, and a choice of surgical procedures and techniques. RESULTS: Hospital or 30-day mortality and morbidity rates were 0% and 48%, respectively. Hepatic failure was not encountered. Histopathologic examination revealed no positive ductal margins in all 40 patients, but 2 showed positive separation margins from the right hepatic artery. The overall 3-year survival rate and median survival time were 40% and 27 months. Survival of patients with Bismuth type III or IV tumors or of patients who underwent right hepatectomy was significantly better. Survival of patients who underwent concomitant vascular resection was similar to survival of those who did not. Univariate analysis indicated the type of hepatectomy, histopathologic grade, Bismuth classification, concomitant hepatic artery resection, and International Union Against Cancer stage as significant prognostic factors. CONCLUSIONS: No postoperative mortality and no positive ductal margins were achieved according to the above guidelines in a high-volume expert center. Long-term results, however, have not been significantly improved. A survival analysis of the patient series with homogeneous conditions derived from a short study period suggests the need for additional strategies including right hepatectomy for Bismuth type I or II tumors.

Validation of a Japanese version of the Stanford Health Assessment Questionnaire in 3,763 patients with rheumatoid arthritis.

OBJECTIVE: To develop and validate a self-administered instrument for measuring functional status in Japanese-speaking rheumatoid arthritis patients. METHODS: We translated the Stanford Health Assessment Questionnaire (HAQ) into Japanese (original HAQ), and then made a tentative Japanese version of the HAQ (J-HAQ) with culturally appropriate modifications of the arising, eating, and reach category questions. The questionnaire was then administered to 3,763 RA patients (82.6% female; mean age 58.0 years; mean onset age 47.4 years; mean disease duration 10.5 years). RESULTS: This instrument showed excellent internal reliability (Cronbach's alpha = 0.927), with a mean interitem correlation of 0.60. For the arising category question, the J-HAQ asks about arising from a futon in addition to a bed because futons are still common in Japanese culture. Arising from a futon is generally more difficult for disabled individuals than is arising from a bed, so the arising score was higher in the J-HAQ (mean score 0.82) than in the original HAQ (0.48). The average scores for the eating and reach categories were virtually identical for the original HAQ and the J-HAQ, with correlation coefficients of 0.979 and 0.926, respectively. Thus, the overall disability index (average of the scores for all functional areas) was higher in the J-HAQ (0.81) than in the original HAQ (0.76), although the correlation coefficient was high (0.993). The test-retest reliability value (0.92), studied at a 1-week interval, revealed identical disability index scores measured on the 2 occasions. CONCLUSION: The final version of the J-HAQ is a valid and reliable instrument for measuring functional status in Japanese-speaking RA patients.
