Group therapy of psoriasis. Duo formula group treatment (DFGT) as an example.

Between 1978 and 1982 an experiment on group treatment for and by psoriasis sufferers was conducted, based on a pyramid of previous investigations since 1968. Each group was facilitated by a duo consisting of a fellow sufferer and a physician, both having trained together. The subjects practiced the procedure described, directed toward self-care and mutual aid facilitated and supported by the duo, in a series of ten 2-hour weekly sessions. A pretest/posttest control group design was used. Comparing the results of the quantitative analysis of the experimental groups with those of the control groups, the experimental groups showed significant change. The results of the qualitative analysis showed that decrease of anxiety correlated with mastering of the skills involved. The key element affecting outcome was the balance in cooperation within the duo, the expertise of the patient lying in his personal experience of the condition, and the expertise of the physician in his knowledge and skills. One of the method's main characteristics was the intrinsic complementarity of the duo partners.

[Group treatment of psoriasis with the duo formula].

Group treatment for and by psoriasis patients was the subject of a trial. For 12 sessions over a period of 3 months, a fellow sufferer as co-helper cooperated with a professional specialist as co-worker according to the so-called duo formula. When the results were compared with those in a control group, the patients showed a change in the dimensions examined, as well as in their illness behaviour and insight into the value of problem-solving in groups. Therefore, it is recommended that dermatologist/co-helper teams be instructed for group treatment of chronic skin diseases, because this method stimulates self-activity in patients.

Keratosis palmoplantaris nummularis ("hereditary painful callosities"). Clinical and histopathologic aspects.

Three families are described. A number of the members of these families showed nummular keratoses on the plantar pressure points. The lesions usually appeared when the children began to walk. They progressed slowly and were often accompanied by pain. The hands showed only minor lesions, which frequently followed manual labor. Extrapalmoplantar lesions were rare. The histopathologic findings in our patients were those of focal epidermolytic hyperkeratosis. The classification of this type of dominant hereditary focal palmoplantar keratosis, for which the name keratosis palmoplantaris nummularis is suggested, is discussed.

Intermittent topical treatment of psoriasis with betamethasone dipropionate 0.05 percent cream.

Seventy-three patients with psoriasis were treated in an open multicenter study with betamethasone dipropionate 0.05 percent cream (twice daily application for four days) alternated with the cream base (twice daily application for three days). The therapeutic response at the end of the treatment period of three weeks was very satisfactory: 39 patients had marked improvement or recovery, 16 patients had moderate improvement, and 14 patients had slight improvement. These results did not differ from those of a study on continuous treatment with betamethasone dipropionate cream in the treatment of psoriasis. Side effects (folliculitis) were only noted in one patient.

The association of cleft lip and palate with basal cell nevus syndrome.

The literature concerning the association of the basal cell nevus syndrome with cleft lip and/or palate is surveyed. Seventeen cases, including two of our own patients, are discussed. Fifteen of the patients were males. Orofacial clefting appears to be an uncommon but definite symptom of the basal cell nevus syndrome. The male-female ratio of clefting in this syndrome is statistically not different from the ratio in the normal population.

Dermatotillomania.

A clinical and histopathological description is given of a number of lesions in 4 patients, mainly caused by picking at the skin. The name dermatotillomania is proposed for this group of lesions.

Dystrophia unguium mediana canaliformis.

An attempt is made to give a sort of classification of the median nail dystrophies. 6 own cases are reported. As there are many morphological transitional forms, it is for the present recommended to call all those forms dystrophia unguium mediana canaliformis.

Juvenile plantar dermatosis.

Juvenile plantar dermatosis seems to be a new skin disease found mainly in children between 3 and 14 years old. The main symptoms are: redness, shiny hyperkeratosis, lamellar scaling and painful fissure formation in weight-bearing parts of the soles and toes. Sometimes the hands also seem to be affected. External factors, occlusive socks and footwear, possibly together with an atopic constitution, are believed to be the cause of the disease, which is very difficult to treat. The findings in 157 published and 23 own cases are discussed.
