[Color-coded duplex sonography (angiodynography) in comparison with phlebography].

To verify the suspected diagnosis of deep thrombosis in the pelvic and/or the leg veins or to clarify the causes of pulmonary embolism, 84 patients were examined prospectively by color-coded duplex-sonography. The findings in the 103 legs examined were subsequently compared to those of an ascending phlebography. The deep veins from the region of the vena iliaca communis down to the lower leg were visualized, and blood flow, lumen and compressibility were checked. This procedure enabled the identification of totally occluding thrombi as well as partial thrombi with the circumfluent blood. The sensitivity of thrombus identification was 92.9%, the specificity was about 97% in the different segments. The results show that color-coded duplex sonography is a sensitive diagnostic instrument for follow-up in conservative therapy. Also, in postthrombotic changes, fresh clots can be identified for the most part. Color-coded duplex sonography can replace phlebography in most cases, if special attention is paid to regions which are difficult to visualize, such as the pelvic veins, the Hunter's channel and the veins of the lower leg.

[Renovascular hypertension: invasive percutaneous diagnosis and therapeutic possibilities].

Under the current conditions of improved pharmacological therapy of arterial hypertension there is a danger that the physician may overlook considering the possibility of underlying renovascular causes for the hypertension. However, the diagnosis and treatment of these conditions are simple and easily accessible. Digital subtraction angiography is a simple technique which enables the diagnosis of renal artery stenosis or stenosis of the aorta to be made as a cause of hypertension in more than 90% of cases. Dependent on the type, extent and location of obliteration in the renal arteries, percutaneous transluminal-renal dilatation with a balloon-catheter is quite frequently the intervention of choice. In fibromuscular dysplasia the results are excellent, with a cure rate exceeding 90%, the patients being predominantly younger and females dominating. In stenosis of atherosclerotic origin, 40-60% show improvement or even normalization of blood pressure. A sudden recurrent increase in blood pressure during long-term follow-up requires angiography in order to perform either dilatation again or surgical reconstruction. In all cases of renovascular hypertension, interdisciplinary cooperation between surgeons and interventional radiologists is essential and all patients with renal insufficiency require close contact to a dialysis centre.

Indications and results after Strecker-stent-application in iliac and SFA.

We report on a study which has been performed during two consecutive time intervals on patients with complicated arterial obliterations, who were treated with percutaneous endoprostheses in order to stabilize the results obtained by the preceding balloon angioplasty. Eighty patients participated at this study, and 136 stents were implanted. A detailed analysis of the reocclusions respectively the stenoses shows that the precise placement of the stents and also the length of the stent in relation to the lesion is of utmost importance, in order to avoid reocclusions. The Strecker stents were lege artis placed in both time groups, and the increased occurrence of reocclusions, especially in the femoropopliteal region, was mainly due to the fact that only one stent length was available. Restenoses in the second group are, thus, considerably lower between 6 and 12 months, as the use of an adequate stent length was possible by then. The medical supplementary treatment of lege artis placed stents in the iliac arteries has no additional influence on the patency rate, while the medication with anticoagulants seems to have a rather positive effect on the patency rate in the femoro-popliteal region. The results, which have been obtained up to now, show that definite improvements are to be achieved by the use of stents, provided that their indication is being handled with utmost care.

[Delayed adverse reactions following the intravascular administration of a nonionic x-ray contrast medium].

Side effects experienced by patients up to 7 days after intravasal administration of nonionic contrast media were recorded with the help of questionnaires. These were completed by the patients themselves and compared with data recorded in patients examined without the use of contrast media. Of the 1000 questionnaires distributed, 499 were returned (271 in the contrast media group, 228 in the other group). A statistically significant difference between the two groups was found with regard to local pain at the injection site. The reactions correlated with the time of intervention. Another significant side effect was found in increased diuresis, which did not lessen during the 7 days of our survey. Increasing side effects were noted with higher contrast medium dosage and advancing age.

[What is the contribution of PET in tumor diagnosis today?].

Positron emission tomography (PET) permits qualitative and quantitative measurements of tumor metabolism in the patient. In the clinical setting, the present studies suggest that tumors with a potential for proliferation can be differentiated from scar tissue, soft tissue and benign lesions. Accumulation of tracer by lymph nodes identifies them as metastatic. On the basis of a quantification of the accumulation of tracer and its metabolites, information can be provided about the biological activity of a tumor, and presumably also about the prognosis. The response of a tumor to treatment can be monitored, in particular in combination with CT. Possible selection of resistant parts of the tumor can be determined, so that modification of treatment is possible. Modes of application can also be individualized by identifying pre-treatment advantages of intraarterial or systemic administration; recurrent tumors can be detected with a high degree of accuracy. However, to date, only limited numbers of cases are available, so that, in view of the very promising aspects of this modality, more widespread application followed by the evaluation of larger numbers of patients is needed.

[Contrast medium clearance as a measure of kidney function in patients with renal artery stenoses].

The quantitative estimation of the characteristic radiation from 127I following external stimulation by 241americium photons makes it possible to estimate iodine concentration. The plasma concentration of non-ionic contrast media handled by glomerular filtration was measured in 16 patients with renal artery stenosis. This was done during diagnostic angiography or percutaneous dilatation, either at an early stage (one to two hours) or at a late stage (five to eight hours). Assuming an exponential excretory function, a clearance value could be calculated. The results were compared with the clearance values of a tubular excreted radiopharmaceutical (99m technetium-MAG3). Despite the different excretory mechanisms, there was a statistically significant correlation between the scintigraphic and contrast clearances at the later time (r = 0.85, p is less than 0.001). At the earlier stage, there was no signification correlation (r = 0.46, p is greater than 0.05). The correlation between the later values and the serum creatinin level was moderately significant (r = 0.55, p less than 0.05).

[Catheter dilatation of arterial stenoses of supra-aortic vessels and late results].

Arterial stenoses of supra-aortic vessels, particularly of the subclavian and vertebral arteries, can be successfully treated by percutaneous transluminal angioplasty (PTA). In 125 patients primary success was achieved in 93% by PTA; 84 patients have been followed up for an average of 46 months and a clinical cure rate of 98.8% has been achieved. Apart from one re-stenosis all patients were symptom free and no other re-stenoses could be demonstrated. PTA of occlusions of supra-aortic vessels has less chances of success.

[Radiologic diagnosis of erectile dysfunction].

Possibilities of radiological cooperation in diagnostics and therapy of erectile dysfunction are demonstrated. Besides SKAT-testing and pharmaco-duplex-sonography, pharmaco-cavernosometry and cavernosography procure the decisive confirmation of venous leakage. The veins which are not compressable during erection can eventually be obliterated under radiological control with the help of mini-coils. Pelvic and penile arteriography are mandatory for the localization of arterial obliterations which can be treated either surgically or by angioplasty. The value of interventional radiological measures in the arterial region can be clearly substantiated in common iliac and internal iliac stenoses. However, the results obtained in the venous region by occluding interventional measures have yet to be evidenced by systematic examinations, as the surgical treatment has not shown much success up to now.

Fine structure of mitochondrial helical filaments revealed by computer image analyses.

In order to clarify the fine structure of the helical filaments appearing in the outer compartment of the mitochondria, Sprague-Dawley strain male rats were given 30% ethanol in drinking water for 90 days. The hepatic tissues of these animals were fixed with perfusion of glutaraldehyde via the portal vein followed by immersion in OsO4 and were then routinely processed for preparation of thin sections. Transmission electron micrographs of the sections were used for computer image analyses of the intramitochondrial helical filaments. With the application of the image analysis, it was revealed that the helical filaments show a right-hand rotating helix of 4.3 nm in thickness, 13.2 nm in diameter of the helix, and 15.7 nm in pitch. Also, with the multivariate statistical analysis and classification method of the IMAGIC image processing system, it was suggested in the classified images that the helical filament has substructures of rod-shaped particles of 4.3 nm in diameter. These particles are considered to be connected to each other and forming the helical structure of the filament.
