A case-control study of the relationships among silica exposure, gastric cancer, and esophageal cancer.

BACKGROUND: We examined the effect of silica exposure on gastric and esophageal cancer mortality using a cancer control series in a population setting. METHODS: Cases and controls were restricted to male subjects and were drawn from death certificates in the Tobi area of Japan. A control group was selected from a series of deaths due to colon cancer, and cancers of other organs. The Japanese death certificate system is comprehensive because all deaths must be reported to the local office and death certificates are written by medical doctors. Age and smoking habits adjusted the Mantel-Haenszel odds ratios were estimated. RESULTS: For gastric cancer, the age-, smoking-adjusted odds ratios were 1.22 (95% CI 0.74-2.01) for colon cancer and the other cancer control for silica exposure work, and 1. 36 (95% CI 0.76-2.43) for silicosis. For esophageal cancer, the age- and smoking-adjusted odds ratios were 1.53 (95% CI 0.59-3.96) for the cancer control for silica exposure, and 2.33 (95% CI 0.87-6.23) for silicosis, respectively. CONCLUSIONS: The results suggest that gastric and esophageal cancer were related to silica exposure and silicosis in the study area, although they did not reach a statistically significant level because of the small sample size. The estimated odds ratios were higher for esophageal cancer and silicotic patients.

Sustained release of basic fibroblast growth factor and angiogenesis in a novel covalently crosslinked gel of heparin and alginate.

The construction of organs by tissue engineering and regenerative engineering, using an artificial extracellular matrix, is an innovative method that is expected to replace artificial organs and organ transplantation. We have produced an artificial extracellular matrix of alginate and demonstrated that the matrix stimulated the regeneration of skin, nerve, and bone. In this report, the new matrix, which consists of heparin and alginate covalently crosslinked with ethylenediamine, was produced to stabilize and control the release of growth factors. Heparin content of the new matrix was confirmed by toluidine blue absorption, elementary analysis, and Fourier transform infrared spectrum. In vitro experiments showed that the new matrix significantly suppressed the initial burst of basic fibroblast growth factor, which is a representative member of heparin-binding growth factors, and released biologically active basic fibroblast growth factor for 1 month under physiological conditions. Obvious cellular infiltration and angiogenesis were shown to occur in the new matrix which was implanted subcutaneously in the dorsal area of rat with 1 microg of basic fibroblast growth factor for 2 weeks. This new matrix may be useful for not only the construction of transplantable blood vessels of small diameter, but also the induction of angiogenesis in regenerated skin constructed by tissue engineering.

Effects of chemotherapy on invasion and metastasis of oral cavity cancer in mice.

BACKGROUND: Using an orthotopic implantation model in which oral cancer invasion and metastasis can be reproduced, we investigated the inhibitory effects of anticancer agents on invasion and metastasis. METHODS: A highly invasive and metastatic human oral squamous cell carcinoma cell line, OSC-19, was implanted into the oral floor of nude mice, and cisplatin or peplomycin was administered to the mice 7 or 14 days after implantation. The effects of each anticancer drug and different administration timings on cancer invasion and metastasis were investigated. RESULTS: Tumor size and the ratio of proliferating cell nuclear antigen-positive cells was significantly reduced. In the control group, the tumors showed grade 4C mode of invasion, whereas in the groups treated with anticancer drugs, grade 3 was observed in 77.3% of the mice, with an inhibitory effect on tumor invasion being observed. The rate of metastasis in the cervical lymph node was significantly decreased in the groups treated with the cisplatin or peplomycin on day 7 after implantation. The tumor stage progression in the metastatic lymph nodes was also inhibited. CONCLUSIONS: Chemotherapy is effective not only for tumor diminution but also for inhibiting invasion and metastasis. In light of these effects, administration of anticancer drugs may be clinically useful in this regard.

Smoking and mental health: cross-sectional and cohort studies in an occupational setting in Japan.

BACKGROUND: The relationship between smoking and mental health remains unclear. METHODS: We carried out a cross-sectional study and a cohort study on the possible association of smoking and mental health in 782 workers. Using a questionnaire including the 30-item General Health Questionnaire (GHQ-30) and items related to the smoking state, the association between smoking and mental health was evaluated separately in males and females. The subjects were classified into smokers and nonsmokers, and changes in the GHQ score during a 2-year follow-up period were evaluated. To control potential confounding factors, multiple regression analyses were performed. RESULTS: The cross-sectional study showed no difference in the GHQ score between smokers and nonsmokers among males but a significantly higher GHQ score for smokers than nonsmokers among females. This difference among females was confirmed to be significant by multiple regression analysis. The 2-year cohort study showed a decrease in the GHQ score in each group and no reduction in the difference in the GHQ score between smokers and nonsmokers among females. CONCLUSIONS: No difference was observed in mental health between smokers and nonsmokers in males. However, in females, smokers showed poorer mental health than nonsmokers, and this difference remained unchanged even after 2 years.

Electroretinogram in the diagnosis of endophthalmitis after intraocular lens implantation.

PURPOSE: To determine whether single flash electroretinograms can assist in the diagnosis of eyes with endophthalmitis after intraocular lens implantation. METHODS: Sixteen eyes with postoperative endophthalmitis treated with vitrectomy or intraocular lens removal were studied. The preoperative electroretinogram findings, duration until the onset of endophthalmitis, culture results, and postoperative visual acuity were analyzed. RESULTS: Four eyes with a b-wave/a-wave (b/a) ratio less than 1.0 and early onset of endophthalmitis within 1 week had worse postoperative visual acuity (no light perception to 20/200) than the other 12 eyes (20/100 to 20/15) (P =.0035). Pseudomonas aeruginosa and Enterococcus faecalis were cultured in these 4 eyes. CONCLUSION: The combination of a b/a ratio less than 1.0 and early onset of endophthalmitis may indicate poor prognosis in a patient with postoperative endophthalmitis.

Implantation metastasis of head and neck cancer after fine needle aspiration biopsy.

One possible complication of the aspiration biopsy of malignant tumors is dissemination of tumor cells along the needle track. However, a search of the literature revealed few definite reports of implantation metastases of head and neck tumors after fine needle aspiration biopsy (FNAB). Here we report two cases of skin metastasis of head and neck cancer after FNAB, including a patient with papillary adenocarcinoma of the thyroid and one with adenoid cystic carcinoma of the submandibular gland. Surgical treatment prevented the spread of the tumor in both cases and there have been no evidence of recurrence to date. This report should alert head and neck surgeons to the possibility of implantation metastasis after FNAB.

Incidence of beta-lactamase production and antimicrobial susceptibility of anaerobic gram-negative rods isolated from pus specimens of orofacial odontogenic infections.

The incidence of beta-lactamase production in anaerobic gram-negative rods isolated from 93 pus specimens of orofacial odontogenic infections and the antimicrobial susceptibility of these isolates against 11 antibiotics were determined. A total of 191 anaerobic gram-negative rods were isolated from the specimens. Beta-lactamase was detected in 35.6% of the black-pigmented Prevotella and 31.9% of the nonpigmented Prevotella. However, no strains among the other species isolated produced beta-lactamase. Ampicillin, cefazolin and cefotaxime showed decreased activity as regards beta-lactamase-positive Prevotella strains, whereas the activity of ampicillin/sulbactam, cefmetazole, and imipenem continued to be effective against such strains. All tested beta-lactam antibiotics were effective against Porphyromonas and Fusobacterium. Erythromycin showed decreased activity against nonpigmented Prevotella and Fusobacterium. Clindamycin, minocycline and metronidazole were powerful antibiotics against which anaerobic gram-negative rods could be tested. The present study showed that beta-lactamase-positive strains were found more frequently in the Prevotella strains than in any of the other species of anaerobic gram-negative rods. The effectiveness of adding sulbactam to ampicillin was demonstrated, as well as the difference in cephalosporin activity against beta-lactamase-positive strains.

Expression of matrix metalloproteinase-2 and -9 in synovial fluid of the temporomandibular joint accompanied by anterior disc displacement.

The purpose of this study was to determine whether or not matrix metalloproteinases (MMPs) in synovial fluid are helpful in the biochemical diagnosis of temporomandibular joint (TMJ) disorder (TMD). We examined the synovial fluid from 38 TMD patients with disc displacement and 20 volunteers by gelatin zymography and immunoblotting analysis to clarify the involvement of the joint pathology from the viewpoint of expression of MMPs. Two gelatinolytic enzymes, MMP-2 and -9, were detected in the samples. The incidences of expression, except for pro-MMP-2, in anterior disc displacement (ADD) without reduction (ADD w/o R) were significantly higher than in ADD with reduction (ADD w R) (P<0.05). Quantitative analysis showed that the degree of MMP-2 and -9 expression in ADD w/ o R were higher than in ADD w R. These data suggest that the presence or absence of disc reduction is a major turning point in the process of joint destruction, and these MMPs are useful as biochemical markers for TMD diagnosis.

A model for temporomandibular joint disc repositioning surgery.

PURPOSE: The present study establishes a model of reversible anterior disc displacement using the rabbit craniomandibular joint (CMJ) and examines the period required for successful disc repositioning surgery. MATERIALS AND METHODS: The left CMJ was exposed surgically in 27 animals and a suture positioned at the posterolateral portion of the disc was pulled in an anterior direction causing 7 mm of anterior disc displacement. The suture was then fixed to the zygomatic arch. The changes in the CMJ were observed microscopically in 3 rabbits each at 2, 3, 5, 8, and 24 weeks. Another 12 rabbits underwent secondary disc repositioning surgery either the second and third week after disc displacement, and then 3 rabbits from each group were assessed microscopically at 5 to 6 weeks and 21 to 22 weeks after the first operation. Ten rabbits had a sham operation, and 2 served as unoperated controls. Histomorphometric evaluation included changes in morphology, cartilage, and disc condition. RESULTS: Progressive osteoarthritic change was induced by disc displacement. Articular disc perforation at 5 weeks after the first operation was accompanied by subchondral bone exposure. The CMJ morphology was almost normal in rabbits that underwent secondary disc repositioning 2 weeks after disc displacement. However, osteoarthritic changes and perforated discs similar to those in rabbits that did not undergo secondary disc repositioning were seen in animals in which repositioning was done in the third week. CONCLUSION: Artificial disc displacement in the rabbit causes osteoarthritic changes are reversible when the displaced disc is restored within after 2 weeks but not at 3 weeks.

Plate fixation after mandibular osteotomy.

Skeletal asymmetry in patients who undergo orthognathic surgery is frequently associated with postoperative temporomandibular joint (TMJ) disorders caused by condylar rotation. This study was designed to elucidate the relation between changes in the condylar long axis and TMJ function after bisagittal split osteotomy (BSSO). A total of 42 patients with mandibular prognathism underwent BSSO. Split osseous fragments were secured by standard titanium plates in 22 patients; bent titanium plates were used to secure fragments in 20 patients. The angle of the bent plates was adjusted to avoid displacement of the condyle after osteotomy, as compared with condylar position on preoperative submental-vertical (S-V) cephalograms. The postoperative position of the condyle was assessed on X-ray film, and changes in TMJ function were evaluated. The condylar long axis differed significantly on X-ray film between patients using a standard titanium plate and those using a bent plate (P<0.05), and no sign of TMJ functional impairment was noted in the bent-plate group. Although the use of bent plates requires further study, maintenance of a suitable position of the condyle relative to the condylar axis is one of the conditions for a successful outcome of BSSO.

Somatosensory-evoked potential to evaluate the trigeminal nerve after sagittal split osteotomy.

OBJECTIVE: The purpose of this study was to assess the onset period of trigeminal nerve hypoesthesia during bilateral sagittal split osteotomy, with the use of the trigeminal somatosensory-evoked potential (TSEP). STUDY DESIGN: Subjects were 10 patients with mandibular prognathism who underwent setback surgery. Intraoperative TSEP was measured 4 times: just before the commencement of the surgery, immediately after the induction of general anesthesia; after the medial periosteal dissection of the alveolar neurovascular bundle; after the sagittal split of bone; and after semirigid fixation with a titanium miniplate with the use of a monocortical screw. Postoperative TSEP was measured after the start of postoperative weeks 2, 4, 12, and 24. Normal records of the TSEP wave comprised peak (13 ms), trough (16 ms), peak (22.7 ms), and trough (36 ms) (N1, P1, N2, P2, respectively). Trigeminal hypoesthesia was assessed by the latency of P1 and N2, because they indicated the most reproducible waveforms. RESULTS: The change in shape of the spectra indicated that prolonged latency was initiated on medial periosteal dissection and was extended further after the sagittal bone split and fixation. However, latency recovery was observed relatively quickly after the start of postoperative weeks 2 and 4. CONCLUSIONS: Trigeminal hypoesthesia appeared to be induced by direct injury to the alveolar nerve during the bone split or by compression injury after fixation. Accordingly, the occurrence of a long-lasting postoperative trigeminal sensory hypoesthesia seemed to depend on nervous involvement of the split surface, the manner of fixation, or the intraoperative care.

Soybean receptor-like protein kinase genes: paralogous divergence of a gene family.

Receptor-like protein kinases (RLKs) in plants play major roles in cellular processes and stress responses. Three soybean (Glycine max) orthologs of Arabidopsis thaliana RLK were isolated and designated GmRLK1, GmRLK2, and GmRLK3. GmRLK1, GmRLK2, and GmRLK3 are similar in sequence, with GmRLK2 and GmRLK3 being nearly identical. The deduced amino acid sequences of GmRLK1, GmRLK2, and GmRLK3 possess characteristics of a transmembrane leucine-rich repeat RLK, AtCLV1. DNA fingerprinting and PCR analyses of a bacterial artificial chromosome library identified five GmRLK contigs (I-V): three for GmRLK1 (I, II, and V), one for GmRLK2 (III), and one for both GmRLK2 and GmRLK3 (IV). Phylogenetic analysis of the soybean RLKs together with other plant RLKs indicates that soybean and A. thaliana CLV1s generate a CLV1 branch, while soybean, A. thaliana, and rice RLKs generate an RLK branch. Thus, the AtCLV1 orthologs may have evolved later than the other pathogen-, environmental stress-, plant hormone-, and development-associated RLKs. A common ancestral GmRLK gene may have duplicated to give rise to GmRLK1, GmRLK2, and GmRLK3, or GmRLK2 and GmRLK3 may have resulted from a recent duplication event(s). Several amino acid replacements in the kinase domain of GmRLK1 compared with those of GmRLK2 and GmRLK3 may reflect evolutionary divergence of individual family members.

Generation of a highly pathogenic avian influenza A virus from an avirulent field isolate by passaging in chickens.

Highly virulent avian influenza viruses can arise from avirulent strains maintained in poultry, but evidence to support their generation from viruses in wild birds is lacking. The most likely mechanism for the acquisition of virulence by benign avian viruses is the introduction of mutations by error-prone RNA polymerase, followed by the selection of virulent viruses. To investigate whether this mechanism could apply to wild waterfowl, we studied an avirulent wild-swan virus that replicates poorly in chickens. After 24 consecutive passages by air sac inoculation, followed by five passages in chicken brain, the avirulent virus became highly pathogenic in chickens, producing a 100% mortality rate. Sequence analysis at the hemmaglutinin cleavage site of the original isolate revealed a typical avirulence type of sequence, R-E-T-R, which progressed incrementally to a typical virulence type of sequence, R-R-K-K-R, during repeated passages in chickens. These results demonstrate that avirulent viruses maintained in wild waterfowl in nature and bearing the consensus avirulence type sequence R-E-T-R have the potential to become highly pathogenic while circulating in chickens.

Biodegradation of long-chain n-paraffins from waste oil of car engine by Acinetobacter sp.

Microorganisms that degrade long-chain n-paraffins from used car engine oil were isolated from soil. For the screening, a fraction of n-paraffin prepared from car engine oil was applied as the sole carbon source. The strain was identified as Acinetobacter sp. The ability of the strain to assimilate long-chain n-paraffins was assessed and characterized. The strain mineralized long-chain n-paraffins (0.1% w/v) in the minimal medium after cultivation for 96 h and also reduced the weight of the waste oil added (1% w/v) by 20% after 72 h without an extracellular biosurfactant. When n-hexadecane was fed as substrate, 1-hexadecanol and 1-hexadecanoic acid were detected as the intermediates by gas chromatography/mass spectrometry. This indicates that the long-chain n-paraffins were metabolized via the terminal oxidation pathway of n-alkane.

Methylcellulose-immobilized reversed-phase precolumn for direct analysis of drugs in plasma by HPLC.

We evaluated a new restricted access media (RAM) precolumn for direct analysis of drugs in plasma using a column switching HPLC system. The new RAM material was prepared by the modification of the external surface of porous silica with hydrophilic methylcellulose (MC), followed by modification of the internal surface with octadecylsilane (ODS). The external surface of the MC-immobilized ODS silica material (MC-ODS) suppressed the adsorption of proteins, while the internal surface of MC-ODS retained various types of drugs, such as ketoprofen, propranolol, caffeine and atenolol in plasma samples. In addition, MC-ODS allowed direct analysis of drugs in a 1000-microL plasma sample to monitor trace amounts of analytes contained. Reduced efficiency and clogging of the MC-ODS precolumn and/or the analytical column were not observed even after the repetitive injection of plasma sample up to 40 mL. Our results indicated that the MC-ODS precolumn could be used in pharmacodynamic and clinical studies.

[Evaluation of RI scintiscanning to parotid gland tumors].

We retrospectively evaluated preoperative radioisotope (RI) scintiscans in 275 patients (283 glands) with parotid tumors who undergoing surgery between 1987 and 2000. Technetium-99m-pertechnetate (Tc) scintigraphy was conducted on 201 glands. Oncocytoma and Warthin's tumor showed a high positive rate. In evaluating Warthin's tumor with Tc scintigraphy, sensitivity was 75% and specificity 88%; above the age of 40 years, specificity exceeded 90%. Gallium-67-citrate (Ga) scintigraphy was conducted on 207 glands. Malignant parotid tumors showed high positivity (85%), but the specificity of positive findings was quite low (38%) because 44% of benign pleomorphic adenoma, which occupied major material, showed Ga-positive findings. Ga-negative findings indicated high probability (95%) of benign tumors. Thus, Tc scintigraphy is effective in diagnosing Warthin's tumor, while Ga scintigraphy is effective in diagnosing benign tumors.

Characteristics of dorsal lingual papillae of the Bactrian camel (Camelus bactrianus).

The morphology of dorsal lingual papillae of the Bactrian camel (Camelus bactrianus) was studied by using light and scanning electron microscopy. Filiform and lenticular papillae were considered as mechanical papillae but fungiform and vallate papillae were considered as gustatory papillae. Filiform papillae were distributed mostly in the anterior two-thirds region of the tongue. Each filiform papilla consisted of one primary papilla and a few smaller secondary papillae. Lenticular papillae were distributed on the torus linguae. The larger papillae were arranged in two parallel lines medially whereas the smaller papillae were laterally located. Most of the fungiform papillae were found on the lateral margins of the anterior two-thirds of the tongue. These papillae were small and round. Intra-epithelial taste buds were located on the dorsal surface of each papilla. The vallate papillae were arranged in two rows on each rim of the torus linguae. Each round- and flat-shaped vallate papilla was surrounded by a prominent gustatory groove and an annular pad. A few taste buds were observed in the lateral epithelium of the papillae. The keratinization of the covering stratified squamous epithelium of the mechanical lingual papillae was relatively thicker than those of the gustatory papillae. The lingual papillae of the Bactrian camel exhibited some different characteristics from other domestic ruminants. These morphological characteristics of the tongue of the Bactrian camel might have evolved to assist the camel in prehension and manipulating of the inorganic stiff plants that grow in its environment and therefore might relate to the feed and feeding habits of the animal.

Abnormalities of gonadotrophs in the adenohypophysis of infertile male PD rats.

Male PD strain rats are sterile in the homozygous condition (pd/pd) due to abnormal spermatogenesis detectable at around nine weeks of age. Previous studies have indicated electron microscopic abnormalities in the Sertoli cells of pd/pd males at three and 12 weeks of age. Since spermatogenesis and Sertoli cell function depend on gonadotropins (luteinizing and follicle-stimulating hormones [LH and FSH]) and testosterone, production and/or secretion of these hormones might be altered in pd/pd males. The aim of the study reported here was to investigate the hormonal status of pd/pd males at three, six, and nine weeks of age. Although alteration was not evident in the LH-immunoreactive cells, FSH-immunoreactive cells in pd/pd males were small in size with scant cytoplasm and were reduced in number and area (73 and 51% of phenotypically normal pd/+ males, respectively) at three weeks of age, although serum FSH concentration was similar to that in pd/+ males. At six and nine weeks of age, percentages of the areas occupied by LH- and FSH-immunoreactive cells in pd/pd males were higher than those in pd/+ males. Serum FSH concentration in pd/pd males was significantly high at nine weeks of age, although a difference in serum LH and testosterone concentration was not evident. These results suggest that FSH production in pd/pd males is decreased at three weeks of age. This might be associated with the Sertoli cell abnormalities and subsequent abnormal spermatogenesis seen in adult life.
