[Frequency and distribution of sarcoidosis in GDR, the County of Leipzig, and the District of Wurzen--a comparison].

The epidemiology of new cases of sarcoidosis in the county of Leipzig is analyzed on the base of the average incidence in the years 1972-1974. The development of sarcoidosis in the district of Wurzen is represented on the base of the new cases of the last 30 years (1950-1979). The districts of the county of Leipzig and the places (towns and villages) of the district of Wurzen are showing very different rates. In the district of Wurzen 19 villages among 37 places in all had not registered any case of sarcoidosis within the last 30 years. The author concludes from the distribution and the pattern of this disease in the course of time, that sarcoidosis must be regarded as a cyclic infections disease.

Application of ferrokinetic investigation for differential diagnosis in bone marrow hypoplasia and their clinical relevance.

In this study, erythropoietic activity of bone marrow has been evaluated by ferrokinetics. For that purpose, the data of radioiron disappearance from plasma and its ultimate incorporation in red blood cells after the injection of about 10 muCi of 59Fe tagged autologous plasma, were analysed and fitted to the sum of three exponentials, using a non-linear least square SAAM-25 program on UNIVAC-1108 computer. A function representing time-activity relationship, was constructed. The functional physiological model of iron metabolism, proposed by Cavill and Ricketts, was used to calculate various ferrokinetic parameters in terms of coefficients and exponents of the time-activity function. After identifying the parameters that could be used as indices of erythropoietic activity of bone marrow, the study was repeated in six patients after they had undergone myelostimulative therapy. A distinct correlation was found between ferrokinetic observations and clinical and biochemical findings. It has been demonstrated that in histologically diagnosed cases with bone marrow hypoplasia, a further differentiation between pure aplasia and those with hypoplasia together with ineffective erythropoiesis was possible. This discrimination which is clinically very important, is only possible by means of kinetic investigations.

Direct anchorage of Al2O3-ceramic hip components: three years of clinical experience and results of further animal studies.

The biocompatibility of high-purity dense Al2O3-ceramics had been shown to open the possibility of direct cement-free anchorage of joint endoprostheses. Three years of clinical experience with ceramic-metal composite total hip prostheses confirm the biomechanical design criteria used for the acetabular components. They also allow for additional conclusions regarding the reaction of bone tissue towards a bioinert implant. Further research activities are directed towards a solution for biomechanically stable anchorage of the femoral component of hip prostheses.

[The effect of bemetizide-triamterene on the serum and whole body potassium in hypertensive patients (author's transl)].

The hypotensive effect of Bemetizide (B)/Triamterene (T) and its action on the potassium balance was investigated in 10 patients with renovascular hypertension. Within 14 days treatment with 50 mg B and 100 mg T a significant fall in the mean blood pressure of 174/106 to 143/95 mm Hg occurred. Whereas the serum potassium concentration fell by an average of 4.2 +/- 0.5 to 3.9 +/- 0.6 mEq/1 the whole body potassium showed only a slight insignificant regression by 50 and 80 mEq after 2--4 weeks; however, the potassium content related to the body weight remained unchanged. In contrast to monotherapy with chlorthalidone or hydrochlorothiazide, treatment with a combination of B and T caused no significant alteration of the intracellular content of potassium.

[Comparative experimental investigations with bioglass (L. L. Hench) and Al2O3-ceramic coated with mod. bioglass. II. Results of experiments with loaded implants (author's transl)].

This report is based on 4 comparative experimental series is sheep using a cementfree total hip replacement model. Basically a smooth stem design was compared to a step stem design for biomechanical interlocking anchorage. Both designs were implanted as pure high density Al2O3-ceramic components and as Al2O3-ceramic parts coated with Bioglass or Bioglass-ceramic (post op. follow up 13 months, 17 animals). None of the 4 stem modifications showed a tendency for permanent stabilisation or bonding in bone. Besides insufficient bonding of the glass coatings to the substrate and apparent biodegradability of the bioglass coatings in the body, insufficient biomechanical knowledge of endosteal direct anchorage of prosthetic devices is the main reason for failure in these experiments. An exclusive biochemical way of anchoring prosthetic parts in the marrow cavity is rejected. The results presented are discussed in the light of the recent literature.

[The relevance of histological results for the evaluation of materials for bone replacement (author's transl)].

Different groups have observed and reported deviating tissue reactions for the same material for bone replacement: for the "bioinert" Al2O3-ceramic close bone contact is mentioned, others report about a soft tissue layer adjacent to the implant; for the "bioactive" glass ceramic interfaces capable to transmit considerable shear forces are described, others found no interface bonding.--The analysis of the results of animal experiments and the evaluation of clinical follow-up studies show that these discrepancies had been caused by the differences of the biomechanical situations in the examined interfaces. It is pointed out that the negligence or not mentioning of the load pattern at the site of examination can lead to misinterpretations about the possibilities and limits of applicability of biomaterials.

[Evidence and estimation of immunoglobulins in human nasal mucosa and nasal fluid--a study towards age dependence (author's transl)].

The localization of IgA, IgG and IgM in healthy human nasal mucosa in relation to age--a fluorescence-microscopic study as well as simultaneous quantitative estimation of these immunoglobulins in nasal fluid. These investigations confirm the dominant role of IgA. The IgA concentrations decreases with age. IgG concentration is nearly independent of age. IgM scarlely existing in normal human mucosa and nasal fluid.

Long-term administration of vitamin D steroles in incipient and advanced renal failure: effect on bone histology.

In 36 patients with incipient and advanced renal failure (CCr 80--30 ml/min X 1.73 m2), serum chemistry including ionized Ca, serum PTH and fractional intestinal absorption of Ca (whole body counter; two-dose-technique; 47Ca p.o. and i.v. to correct for urinary and endogenous fecal loss were measured. Quantitative bone histology after in vivo tetracycline double labeling was evaluated from undecalcified sections before and 18 months after therapy with vitamin D3 or 5,6-trans-25-OH-CC in a dose sufficient to raise intestinal absorption and/or urinary excretion of Ca. Intestinal absorption of Ca was impaired in some patients at a GFR of 60 ml/min/1.73 m2. After up to 10000 U/d 5,6-trans-25-OH-CC and 8000 IU/d vitamin D3, respectively, fractional intestinal absorption of Ca rose and was normalized in all patients. There was a concomitant rise in urinary Ca. Serum PTH fell, but did not always return into the normal range. Ionized Ca rose in all patients. Bone histology was evaluated in 17 of these 36 patients after informed consent was obtained. The mass of mineralized bone (Vv) rose in 7/17 patients, pointing to a positive calcium balance. Volumetric density of osteoid (Vvos) and surface density of osteoid (Svos) fell in 10/17 patients concomitant with an increase in the fraction of mineralizing seams and a decrease in the number of lamellae in osteoid seams. Osteoclastic resorption (OCl) fell as did the fraction of woven osteoid seams. However, woven osteoid failed to disappear completely and osteoclastic resorption stayed elevated in some patients. 5,6-trans-25-OH-CC and vitamin D3, in doses that normalized intestinal absorption of Ca, failed to restore completely bone histology to normal although mineralization and collagen texture of osteoid were consistently improved. The dose response characteristics to vitamin D of different abnormalities of Ca metabolism appear to be non-uniform.

[Nitrogen balance and plasma amino acids in premature and newborn infants with partial parenteral feeding during the first 3 days of life].

Eight pre-terms, five with postnatal asphyxia, two hypotrophic newborns and one term infant with severe postnatal asphyxia (mean birth weight 2317 g) were fed in addition to oral nutrition partialy parenterally with glucose solution and a mother-milk adapted amino-acid solution during their first three days of life. At increasing protein supplies with a parenteral share of more than 50% during the three days, the N balances resulted positive on all days although the calories supplied were far below the postulated optimum. On days two and three N retention was about 60% of the supply. Every four hours after discontinuation of the total 16 hours' amino-acid infusion the amino acid levels in plasma were controlled by means of column chromatography. The resulting mean values did not show any imbalances of the amino-acid homeostasis. The maximum mean loss in weight of the present group was 4% of their birth weight during the first ten days of life and thus equalled the loss in weight of normal, healthy pre-terms. Clinical and chemical paramters did not reveal any disadvantageous side effects of parenteral nutrition.
