Application of dermal substitute (Integra) to donor site defect of forehead flap.

Although forehead flap has limited use, it may have a role as a 'third' line option. Its primary complication is the poor cosmetic defect on the forehead. A case is presented in which, after harvesting the flap for reconstruction of the oral cavity, a dermal substitute (Integra) together with split thickness skin graft was used for the management of this wound. The result was significantly improved over using split skin graft alone.

Carcinosarcoma of the parotid gland: ultrasound and computed tomography findings.

Carcinosarcoma is a rare tumour of the salivary gland. The ultrasound and CT appearances of a rapidly enlarging carcinosarcoma in the parotid are described. Both modalities revealed a 4-cm tumour with relatively benign features and no evidence of lymphadenopathy. Ultrasound showed a well-defined mass of homogeneous low echogenicity and posterior acoustic enhancement. Computed tomography showed a parotid mass of homogeneous attenuation similar to muscle, and homogeneous enhancement following contrast. The clue to the malignant nature of the lesion was a short indistinct superficial margin with tumour extension into the adjacent subcutaneous tissues.

Development and evaluation of a new composite Laserskin graft.

BACKGROUND: Tremendous effort has been made to improve the graft take rate of cultured epidermal autograph. The purpose of this study is to develop and evaluate a new composite Laserskin graft (CLSG) as a human skin substitute for wound resurfacing. METHODS: The seeding efficacy of cultured keratinocytes on plain Laserskin was compared with the 3T3 cell-seeded Laserskin and allogenic fibroblast-populated Laserskin. Three different types of CLSG, 2 cm in diameter each, were prepared and tested in rats. Type A CLSG consisted of proliferative allogenic rat fibroblasts on both sides of the Laserskin with autologous keratinocytes also on the upper side. Fibroblasts and keratinocytes were seeded only on the upper side of the Laserskin in type B CLSG. Keratinocytes alone were seeded on plain Laserskin in type C CLSG. Type B CLSG consisting of autologous keratinocytes and autologous dermal fibroblasts was tested on five selected wounds (5x5 cm each) of a patient with full-thickness burn. In another burn patient, type B CLSG consisting of autologous keratinocytes and allogenic dermal fibroblasts was grafted onto three wounds (5x5 cm each). RESULTS: The seeding efficacy of human keratinocytes on plain Laserskin increased from 75% to 95% when proliferative allogenic fibroblasts were grown as a feeder layer on the Laserskin. The seeding efficacy of rat keratinocytes increased from 36% to 88% in the presence of a proliferative allogenic fibroblast feeder layer, whereas human/rat keratinocytes had respective seeding efficacy of 98%/91% on Laserskin preseeded with mitomycin C-treated 3T3 cells. Skin biopsies of grafted type A CLSG on day 14 after grafting showed complete epithelialization without severe inflammation in 16 of 20 (80%) grafted surgical wounds in rats. There were eight (40%) and seven (35%) "takes" of the CLSG in types B and C, respectively. The infection rate in type B CLSG was two (10%). There was one (5%) infection in types A and C. The respective take rates on the two patients grafted with type B CLSG were 60% and 100%. CONCLUSION: The animal experiment and the preliminary clinical data showed that the CSLGs consisting of autologous keratinocytes and of autologous/allogenic fibroblasts are good human skin substitutes in terms of durability, biocompatibility, high seeding efficacy for keratinocytes, high graft take rate, and low infection rate.

Radial forearm free flap: hybrid version.

The hybrid version of the radial forearm free flap provides an alternative method for reconstruction in the head and neck. It is versatile and has certain theoretical and practical advantages, including high-flow venous drainage, long venous pedicle, lymphatic drainage, and the avoidance of venous anastomosis or venous transpositional graft. In those situations when the availability of veins is reduced after a radical neck dissection or the quality of vein is poor because of previous irradiation, the hybrid free flap is the method of choice for microvascular reconstruction in the head and neck region.

Burn injuries during paint thinner sniffing.

Thinner sniffing is popular among school children in Asian countries because it is readily available at low cost. Besides its toxicity to major organs, thinner inhalation is associated with various burn accidents. Four teenagers were admitted to the Burns Unit of the Prince of Wales Hospital over the period of 1996-1997. They sustained 3-25% TBSA flame burn and two of them had inhalation injuries as a result of the ignition of a cigarette during thinner sniffing. None of them had evidence of thinner intoxication as shown by blood tests. In the management of their acute burn injuries, their hidden social and family problems were explored. With the cooperation of different disciplines, early psychosocial intervention was given and their behavioral and psychological disturbances were successfully managed.

Central calcifying odontogenic cyst. Case report.

A case of central (intraosseous) calcifying odontogenic cyst affecting the mandible is reported. The clinical, radiographic, and histological features are reviewed and discussed.

Lingual nerve retractor.

An anatomically designed lingual nerve protector is presented. Its use on two thousand patients and a low incidence of temporary lingual nerve damage is reported.

Malignant melanoma of the oral cavity--an increasing clinical diagnosis?

Oral malignant melanoma is fortunately a rare lesion. However the actual incidence in the population at present is unknown but may be increasing. The overall prognosis remains poor despite more radical surgery with adjuvant radiotherapy, chemotherapy and immunotherapy. The clinical presentation of oral malignant melanoma can vary widely from a typically pigmented macular or proliferative lesion to a non-pigmented soft vascular tumour; single or multiple, primary or metastatic. Our experience of five patients with malignant melanoma of the oral cavity is presented and the management of such lesions discussed.

A complication of the Dautrey procedure.

A case of longstanding recurrent dislocation of both temporomandibular joints treated surgically by the Dautrey procedure is presented. A complication occurred at the time of surgery. The management of this problem and the outcome after a 3-year follow-up period is described.

A study of natal teeth in Hong Kong Chinese.

The purpose of this study was to review cases of natal teeth in babies born between January 1984 and December 1988 at the Queen Elizabeth Hospital Hong Kong. There were 53678 births at the hospital during this period and 48 babies had natal teeth, a prevalence of 1:1118. The 48 babies had 72 natal teeth, all mandibular central incisors. The natal teeth in two babies were supernumerary teeth of a predeciduous dentition. Twenty nine natal teeth were extracted because they were hypermobile, three were extracted because they traumatized the ventral surface of the babies's tongue, and 16 were not extracted. The subjects were reviewed one day after the initial examination, and then after 1 week, 1 month and at 6-month intervals. No appreciable space loss occurred following the extractions. Root development occurred in teeth that were not extracted, but the crowns were hypoplastic in three cases.

Intra-mandibular salivary monomorphic adenoma.

Monomorphic adenoma can affect the major or minor salivary glands. However, intra-mandibular monomorphic adenoma has only been reported once in the literature. A second case with recurrent behaviour is reported.

Supero-lateral dislocation of sagittally split bifid mandibular condyle.

A case of supero-lateral dislocation of the mandibular condyle into the temporal fossa is reported. The condyle was concurrently fractured antero-posteriorly forming a bifid mandibular condyle. The previously reported cases of supero-lateral dislocations into the temporal fossa are discussed and the literature of bifid condyle is reviewed.

Mandibular ankylosis associated with a bifid condyle.

A case of bilateral temporo-mandibular joint (TMJ) ankylosis with a bifid mandibular condyle is reported. The patient had a history of trauma 39 years previously. The right condyle was mushroom-shaped and the left condyle was bifid. The treatment of ankylosis is briefly discussed and the literature on bifid condyles is reviewed.
