Acute myelomonocytic leukemia (M-4 subtype) with abnormal marrow eosinophilia and a normal chromosome 16.

A variety of chromosome 16 abnormalities, including inversion, deletion, and translocation in patients with acute myelomonocytic leukemia and abnormal marrow eosinophilia have been reported recently. The authors have identified an AMML patient who had a normal karyotype in 50 metaphases. In particular, chromosome 16 was closely examined for abnormality and was found to be entirely normal. In addition, the authors describe new cytochemical and ultrastructural features of the associated abnormal eosinophils.

[Insurance and preventive medicine].

Not only do insurance companies have to pay in case of death, injuries or disease, they are also concerned with their prevention. This is particularly true for the "Swiss National Accident Insurance Fund" (Caisse nationale suisse d'assurance en cas d'accidents--(CNA): for them the prevention of work related accidents and occupational diseases is required by law. Preventive activities in this area are very promising. The progress in the sickness insurance programmes for preventive medicine in the general population has, however, not been as successful. To date, the legislation denies payment for preventive medical care. Why is there this difference? In the case of accidents and occupational diseases, the cause of the pathologies are for the most part exogenous and develop in well known and controlled environments. In the case of disease or invalidity in the general population, the factors are in a large part endogenous and therefore very difficult to supervise, as they develop in much more complex and uncontrolled environments. Nevertheless progress has been done in this field as well. At present, some selected scientifically proven preventive examinations could be included in insurance programmes as part of a general plan and with strict quality control of laboratory findings.

The influence of nitrous oxide anaesthetic on middle-ear fluid.

Nitrous oxide anaesthesia invades the middle-ear cavity, resulting in a positive pressure within this cavity. It has been suggested that the pressure may be sufficient to force open the Eustachian tube and evacuate fluid from the middle ear. This study examined the possible influence of nitrous oxide on middle-ear fluid. Pre- and intra-operative tympanograms were obtained on 39 children scheduled for myringotomy surgery. Fluid was found in 83.1% of the operated ears while the absence of fluid was noted in 16.9%. It is possible that the nitrous oxide anaesthetic did cause an evacuation of fluid from this latter group of ears prior to actual surgery.

Noise-excluding enclosure for audiometry.

Excessive ambient noise adversely effects the efficiency of hearing conservation programs. Noise-excluding enclosures generally reduce the masking effects of such noise more effectively than standard earphones only. Research has shown, however, that pure-tone thresholds obtained with some enclosures may not be valid. The present study examines one particular type of enclosure, the Audiocup. The results revealed (1) that pure-tone thresholds obtained with Audiocups are almost identical to those measured with standard earphones only and (2) Audiocups more effectively reduce the masking influence of white noise. It is concluded that Audiocups may be used for screening and for threshold audiometry in less than ideal testing environments without fear of invalidating the results.
