[Shock lung (ARDS): glucocorticoids?].

Numerous experimental investigations have demonstrated the benefit of glucocorticoids in the initial phase of the shocklung syndrome (ARDS), showing an increasing pulmonary perfusion by blocking the formation of vasoactive substances and a stabilization of endothelial and lysosomal membranes; another effect is to be seen in the inhibition of neutrophilic chemotaxis and pulmonary leukostasis, leading to reduced inflammatory reaction and less severe pulmonary damage. Clinical studies of glucocorticoid therapy in ARDS indicate a higher rate of surviving patients, when they were treated early and with glucocorticoids in a high dosage. The ill-defined condition of ARDS, however, and the use of different dosages in most of the studies limit the comparison of success dependent on those regimes; e.g. the given dosage varies from a single dose of 250 mg. methylprednisolone to several infusions of 30 mg./kg. body weight. According to the pharmacokinetics of glucocorticoids, the desired effects cannot be expected by application of a single and small dosage. To verify the benefit of high-dosage glucocorticoids in ARDS a stronger definition is needed to mark the onset of the syndrome and the start of glucocorticoid treatment. Additionally the dosage regimen should be standardised.

[Endometritis--a diagnostic problem. Differentialdiagnostical considerations including histological and microbiological methods (author's transl)].

In 100 patients with the assumed diagnosis "endometritis" an examination program including histological and microbiological methods was conducted. In only 73% of these, the histological examination confirmed the assumed diagnosis, and only in 3% concerning the microbiological tests; therefore other diagnoses such as peritoneal irritation at ovulation, adenomyosis uteri and spastic pelviopathias were dominant. The diagnosis of an endometritis therefore is often an exclusive one and should induce subtle examination methods before an unspecific antibiotic therapy is begun. This procedure is absolutely possible regarding out-patients.

[The cardiac situation of newborn children after long-term tocolysis with fenoterol (author's transl)].

516 newborn children were subject to a thorough cardiac and neurological test program after a long-term tocolysis with Fenoterol with and without a supplementary medication of Verapamil. No symptoms of a myocardial damage caused by the drug appeared neither in the electrocardiogram nor by means of lab-chemical parameters such as CK-MB, Serum-Myoglobin and CK-B. Except for premature children, no significant neurological characteristics were found either. Due to the low quantity of Fenoterol passing through the placenta and based on the minimum cumulation of Fenoterol in the fetal tissue, especially in the fetal myocardium, a tocolysis does not seem to have any significantly dangerous fetal effects, if control parameters are strictly adhered to. The endeavoured therapeutical objective, namely the prevention of premature labour, thus justifies the application of the betasympathomimetic Fenoterol.

[Sensitivity of creatine kinase and creatine kinase MB in myocardial infarction. Evaluation of a new optimized standard method (author's transl)].

In 70 patients with confirmed transmural myocardial infarction the sensitivity of creatine kinase and creatine kinase MB at different times after admission was investigated by 364 measurements. Beside the number of correctly positive results the optimized standard method hitherto used was compared with the new optimized standard method with N-acetylcystein as activator. During the first 12 hours after admission the percentage of correctly positive results was 64,4% for creatine kinase (GSH) and 86,7% for creatine kinase (NAC). The sensitivity of cretine kinase MB, however, was found to be 71,1% (GSH) and 86,7% (NAC). With respect to the poor specifity of creatine kinase the sensitivity of creatine kinase MB, especially when using the new optimized standard method is superior. Similar results were established 24 and 48 hours after admission. Creatine kinase MB only fails in detection of myocardial infarction if the stroke is older than 3 days or if the enzyme activity determination intervals are too long.

[Creatine kinase and creatine isoenzyme activities in newborn. Development of the organ-typical isoenzyme pattern during the fetal period (author's transl)].

The activity of the creatine kinase isoenzyme was measured in the serum of 133 healthy newborn. In contrast to the conditions in the adult, a normal range of 0-45 U/1 was found. No creatine kinase BMB activities were established. The increased creatine kinase-MB activities in newborn could be explained by means of an examination of the creatine kinase isoenzyme pattern in the skeleton muscle of foetuses and newborn. Depending upon the gestation age, creatine kinase-MB activity levels were found amounting to as much as a multiple of ten of the adult levels. Due to the deviation of the creatine kinase isoenzyme distribution in the organ tissue of newborn, identified in this study for the first time, creatine kinase-MB activity seems to be unsuitable as an indicator of myocardial damage during the neonatal period.

[Creatine kinase and creatine kinase isoenzyme MB: determination of normal values and values for myocardial infarct, using the new, optimized method with N-acetyl cysteine as the activator (author's transl)].

Preliminary results are reported for the determination of creatine kinase and its isoenzyme MB in healthy individuals and in patients with acute, transmural myocardial infarct, using N-acetyl cysteine as the activator of the enzyme. In healthy individuals, the upper normal limit for creatine kinase activity was 54 U/l; the decision level for the presence of creatine kinase MB activity 9 U/l. The limiting values were only slightly different from those for the GSH-activated creatine kinase, and the results with the old and new methods showed a linear relationship with a slope of 1.0. The differences in the activation of creatine kinase MM by GSH and by N-acetyl cysteine are discussed; the activities obtained for creatine kinase and creatine kinase-MB by the GSH-activation method cannot simply be recalculated to give the appropriate values of the N-acetyl cysteine activation method.

Study to confirm the lack of severe cardiac side-effects following treatment with the beta-adrenergic drug fenoterol.

100 patients with premature contractions were treated with the beta-adrenergic agent fenoterol for up to 9 weeks. To rule out a detrimental effect on the myocardium, the creatine kinase isoenzyme MB, which is specific to the myocardium, was determined regularly in addition to electrocardiographic monitoring. No pathological enzyme activity was detected even in long-term tocolytic therapy with the maximum dose.

[Clinical and laboratory aspects of malignant hyperthermia in children with special reference to creatine kinase isoenzymes (author's transl)].

The clinical and laboratory findings in 4 children with signs of malignant hyperthermia are reported. In all cases an extraordinary elevation of creatine kinase activity in serum was observed. By investigation of the creatine kinase isoenzyme activities we tried to determine the origin of creatine kinase. In contrast to other reports, creatine kinase BB derived from brain was found to be absent in all cases, although creatine kinase MM and MB showed remarkable alterations. A certificate for all patients who have survived malignant hyperthermia is suggested.

[Creatine kinase isoenzyme MB and CK/GOT-ratio in the differential diagnosis of acute myocardial infarction (author's transl)].

In 56 patients with certain transmural myocardial infarction the reliability of myocard specific creatine kinase isoenzyme MB is compared with the usual estimation of creatine kinase activity in serum and the use of CK/GOT activity quotient. In measurements at different times CK-MB proves to be more reliable than other laboratory parameters: during the first 24 hours after hospital admission CK-MB indicates the infarct and correlates with ECG alterations in practically all cases. In 11 patients with CK-activity increase over 100 U/l of extracardial origin, myocardial involvement can be excluded by measuring CK-MB-activity. Formation of CK/GOT quotient shows erroneously an infarction in about half the cases.

[The possibility of heart-muscle damage through fenoterol (elektrocardiographic and blood-chemistry studies with particular reference to the heart-muscle-specific isoenzym creatinkinase MB.) (author's transl)].

Fifty patients with premature uterine contractions were treated with the uterine relaxant Fenoterol in combination with the calcium antagonist Verapamil. In order to exclude the possibility of myocardial damage, the heart-muscle-specific isoenzym Creatinkinase MB was routinely evaluated, in addition to ECG-checks. In none of the cases could a significant enzyme activity be shown, even in long-term therapy with high doses of Fenoterol.
