[Comparative evaluation of surgical and combined treatment of cancer of the proximal region of the stomach].

The paper deals with an analysis of the results of treatment of 351 cases of radical surgery for cancer of the proximal part of the stomach (surgery-303, surgery + preoperative large-fractionated radiation-48). It was found that radiation was not followed by increased blood loss during operation nor did it interfere with application of any surgical procedure. Concentrated irradiation did not involve an increment in postoperative lethality and complication rates. Five-year survival rates were 37.4 +/- 8.0 in cases of combined treatment and 20.4 +/- 2.7% in those of surgery. The improvement in survival rates recorded in cases of combined treatment was largely due to a decreased incidence of local recurrences.

Primary complications after surgery for cancer in the cardio-oesophageal region.

Results are presented from a study of 230 patients who during the past 20 years underwent radical surgery for cancer in the cardio-oesophageal region. Major postoperative complications arose in 94 patients and were lethal in more than half of them. Most of the complications, and the causes of death, were of pulmonary-pleural nature or peritonitis of varying genesis. Peritonitis occurred in half of the lethal cases and was mostly due to insufficiency of the anastomosis. The death rate and the incidence of primary complications were unrelated to the extent of the operation (total gastrectomy or upper partial gastrectomy). Oesophago-intestinal and oesophago-gastric anastomoses showed approximately equal frequency of insufficiency, but peritonitis was more common after total gastrectomy than after proximal resection of the stomach.

[Value of combined operations in surgery of cancer of the cardial portion of the stomach].

The authors are analyzing 109 combined operations performed for cancer of the cardial portion of the stomach with and without involvement of the oesophagus. Both immediate and remote results of the treatment are analyzed. In order to increase the radical effect of operation in metastazing of cancer into the neighbouring organs and presence of metastases in the regional lymphatic nodes the combined operations are recommended.

[Complications and lethality after radical operations for cancer of the cardial portion of the stomach].

The author presents an analysis of the data on 94 patients with cancer of the gastric cardiac portion with and without the involvement of the esophagus, in whom radical operations (proximal gastric resection and gastrectomy) were followed by various complications. The main complications and causes of patients' death were pleuropulmonary complications and peritonitis. Peritonitis was more frequently due to the anastomosis suture incompetency. Peritonitis resulted in the postoperative mortality totally in 50% of patients. Suture incompetency of entero-esophageal anastomoses was as frequent as that of gastro-esophageal ones.

[Late results of surgical treatment in carcinoma of the cardia of the stomach].

The author studied the findings obtained in 27 patients operated upon for carcinoma of the cardia of the stomach with/without involvement of the esophagus, who survived more than 5 years after radical operations. 16 patients survived more than 10 years, 4 of them--20 years. The common prognostic factors, i. e. metastases, anatomic type of the tumor and its size are analysed.

[Myopathy (terminology, clinical picture, and diagnosis)].

The paper deals with a description of the clinical picture and diagnosis of the main types of myodystrophies taking into account formes of muscular damages and a trend of generalization of a pathological process at the early phase of the disease. The clinical picture of endocrine myopathies was also studied. The authors revealed almost the same formula of muscular damages. Muscles of the pelvic girdle and femurs mainly suffered and to a lesser degree--muscles of the shoulder girdle and shoulders. According to the data of the clinical picture, histopathology and electromyography endocrinic myopathies are phenocopies of hereditary myodystrophies.
