Seasonal and annual variation of antibody to HIV-1 among male inmates entering Maryland prisons: update.

Previous serosurveys of antibody to HIV-1 among incoming male inmates in Maryland between April and June of 1985, 1986 and 1987 have shown a prevalence of 7.0, 7.7 and 7.0%, respectively, with stability persisting after multivariate adjustment. To investigate seasonality and update annual trends, excess sera were obtained from incoming male inmates between August 1987 and August 1988. Correctional personnel also provided demographic variables of age, race, offense, category, and jurisdiction. Once rendered anonymous, specimens were assayed for antibody to HIV-1 using enzyme-linked immunosorbent assay and Western blot. For the 12-month study period, 415 (7.9%) of 5262 consecutive male entrants were seropositive. On univariate and multivariate analyses, no significant change in seroprevalence or risk by subgroup was noted by month or season. For data from April to June 1988, 113 (8.1%) of 1398 consecutive entrants demonstrated anti-HIV-1; seropositivity was associated with age greater than 25 years, Black race, and Baltimore jurisdiction. No significant change was found over time in seroprevalence or risk of HIV-1 infection by subgroup in multivariate analysis combining data for 1985-1988. These data provide additional evidence to suggest stability of HIV-1 seroprevalence in Maryland male prison entrants.

Sex differences in the hyperkinetic syndrome of childhood.

Among a clinic sample of 6525 subjects, 61 males and 18 females with an ICD-9 diagnosis of the hyperkinetic syndrome of childhood were identified. Hyperactive girls had a lower IQ and significantly higher rates of language disorders and neurological disorders, suggesting a possible neurological basis for hyperactivity in females. In contrast, there was a population of male hyperactives with less evidence of brain dysfunction and a normal IQ, evidence compatible with the model of greater male constitutional variability. There was little evidence for the polygenetic multiple threshold model of sex inheritance.

Some effects of mastectomy on reproductive success in the guinea-pig.

Virgin guinea-pigs were mastectomized in two stages between 11 and 18 weeks of age and then mated, starting 19 weeks after final surgery. In the subsequent first pregnancy, the incidence of still-births and neonatal deaths was significantly higher in the mastectomized animals (6 out of 12 mothers (50%) and 14 out of 49 young (29%) compared with intact guinea-pigs (1 out of 15 mothers (7%) and 1 out of 58 young (2%)). There was no significant effect of mastectomy on litter size and weight or on gestation period. The still-born were not significantly different in weight from those born alive. A significant relation was found between maternal weight changes in the period 20 to 5 days before parturition and the occurrence of still-births and neonatal deaths; still-births were associated with a period of reduced weight gain. No effect of mastectomy on the length of the oestrous cycle was apparent but a significant increase in the incidence of non-pregnancy was found. The results provide further evidence that mastectomy influences reproductive success in the guinea-pig and suggest that parturition is a key process affected.

Oestrogen production by the goat mammary gland: transient aromatase activity during late pregnancy.

Mammary tissue from goats had significantly higher aromatase activity at 148-149 days of pregnancy than earlier in pregnancy or during lactation. Aromatase activity was determined by production of 3H2O from [1 beta-3H]androstenedione that was inhibited by 4-hydroxyandrostenedione. The time of maximum aromatization was during peak mammary output of oestradiol-17 beta described previously in vivo in the same species. Aromatase activity in the mouse mammary gland was low and no peak was observed in late pregnancy. It is concluded that the oestrogens produced by the mammary gland in the late-pregnant goat are formed by aromatization of androgens.

Research attitudes and activities of occupational therapy clinicians.

Two hundred and seventy occupational therapy clinicians, whose primary or secondary employment function was in direct patient service, were surveyed to determine their perceived roles and attitudes regarding research. Results showed a strong belief in the importance of research in the profession, yet minimal involvement in research due to limited time, money, and skill. The role of collaborator with experienced researchers was rated as highly desirable. Few clinicians indicated no interest in research. Implications of the results and suggestions for strategies to increase clinicians' involvement and satisfaction in research through mutual experiences and continuing education are given.

Antibody to human T-lymphotropic virus type I/II (HTLV-I/II) among male inmates entering Maryland prisons.

Serum specimens that had been obtained for routine operational procedures from consecutive entering male inmates during Spring 1987 and the same calendar period in 1988 were tested to identify prevalence and risk groups for antibody to HTLV-I/II. Specimens were assayed for antibody to HTLV-I/II using ELISA, RIPA, and Western blot techniques. Demographics were compared by serostatus using chi 2 and Fisher's exact tests. Of the 1,932 inmates entering prison, 49.3% were 25 years of age or older, 70.1% were black, 62.4% were committed from the Baltimore metropolitan area, 34.1% were intravenous drug users, and 7.0% demonstrated antibody to HIV-1. Among 1,932 inmates, 18 (0.9%) were HTLV-I/II seropositive. All seropositives were black; age greater than 25 years old was significantly (p less than 0.01) associated with seropositivity. Reactivity to HTLV-I/II did not vary significantly by year of entry, HIV-1 serostatus, jurisdiction, offense category, or sentence. Prevalence of HTLV-I/II among incoming male inmates was elevated compared to available local population comparisons. Additional blinded epidemiological serosurveys of antibody to HTLV-I/II are indicated for prison populations in order to monitor the extent and scope of infection in this population.

A non-contacting intraoperative electron cone apparatus.

Aluminum tubes, 20 cm long, of various diameter and face angle have been used to collimate electron beams for intraoperative radiotherapy. The tubes placed in a body cavity are clamped to the treatment couch. A thin rod is fixed coaxially to a disc which is placed on the entrance face of the tube. A stepwise procedure involving couch translation, rotation, and gantry rotation is used to align the central axis of the beam with the axis of the cone. Further collimation is provided by an aperture mounted in an assembly which slides into the standard accessory holder of the machine and is separated by a 10 cm air gap from the tube in the patient. Careful adjustment of this aperture diameter results in a relatively uniform dose across the treated area. The use of 3 mm wall thickness aluminum tube provides for minimal leakage outside of the cone. A camera has been mounted to a plate that also mounts in the standard accessory holder and is used to document the area treated.

Postnatal depression: what can a health visitor do?

Postnatal depression has been a subject that has provoked interest and research for many years. Marce published a paper in 1858 on puerperal and lactational psychoses, and since then many other writers have pursued different avenues of interest, primary cause, affect and treatment. A great deal of this work over the years has been done by psychiatrists and physicians, principally being interested in the more severe postpartum psychosis, when many of those suffering are admitted to an acute psychiatric hospital. However, as research has continued the less severe forms of postnatal depression and the upsetting effect they have on the patient's family continue to be a source of concern, not only to the professionals themselves, but more importantly, to the mothers who are suffering from this very upsetting problem. Professor Cox stated that he found it ironical that primary care workers regularly visited mothers in the puerperium and only rarely recognized postnatal depression. This led me to believe that we, as health visitors, should set about trying to put this right. As Professor Cox rightly said, it's a sad paradox that considering the medical scrutiny mothers receive over the antenatal and postnatal period, many depressed mothers still are not identified. The author therefore attempts to devise some kind of strategies for health visitors to identify mothers suffering from postnatal depression, and considers whether any links can be found between those suffering from postnatal depression.

Performance of serological assays for early detection of human immunodeficiency virus type 1 seroconversion.

Early detection of sexually transmitted human immunodeficiency virus type 1 (HIV-1) infection was investigated in newly infected persons to determine the sensitivities of currently available serologic techniques. Serial serum samples were obtained from 51 newly infected persons in a cohort of 1,153 homosexual or bisexual men participating in the Baltimore Center of the Multi-Center AIDS Cohort Study during the first 2.5 years of follow-up. Of 51 participants, 45 seroconverted between any two seminannual visits and 6 were found to have been infected just prior to study entry. Five enzyme-linked immunosorbent assays (ELISAs), two immunoblots, and an HIV-1 P24 antigen capture assay were performed on a panel of all serial serum samples from these individuals. The sensitivity of ELISAs varied between 50 and 84% in seroconverters with less-developed antibody response. In this group of seroconverters, the most sensitive antibody assay was an immunoblot from Biotech (95%) and HIV-1 P24 was found infrequently (9.5%). The sensitivities of ELISAs and immunoblot were 100% in individuals with more-developed antibody patterns, and no HIV-1 P24 was detected among them.

Relaxin detected by immunocytochemistry and northern analysis in the mammary gland of the guinea pig.

Relaxin is a product of the endometrial gland cells in the guinea pig. Mammary tissue was collected from intact cyclic animals, on days 35 and 63 of pregnancy, days 5 and 21 of lactation, and day 28 postpartum. Tissue was collected from six cyclic hysterectomized animals. Sections of mammary gland were immunostained with the avidin-biotin immunoperoxidase method, and antisera to porcine relaxin which were raised to different preparations in different laboratories. Light uniform staining was evident in the cytoplasm of all of the cuboidal epithelial cells forming the mammary duct system in cyclic animals; mammary gland from hysterectomized animals showed similar staining. At midpregnancy light staining was seen in some epithelial cells, and by late pregnancy there was only faint staining. On day 5 of lactation there was intense and uniform staining throughout the epithelial cells of the alveoli. By day 21 of lactation the cells still immunostained for relaxin, but on day 28 postpartum there was a return to the cyclic staining pattern. Endometrium from animals at 55-60 days of pregnancy and mammary gland from day 6 of lactation were used for poly(A)+ RNA isolation and Northern analysis. Three 48-mer oligonucleotide probes were used. Poly(A)+ RNA from endometrium of late pregnant guinea pigs and from the mammary gland in lactation hybridized with the same two probes and failed to hybridize with a third under moderate stringency conditions. The results suggest that the major source of relaxin in the guinea pig is sequential, the pregnant uterus and the lactating mammary gland. The local and/or systemic significance is not known.

Inspired oxygen concentration during general anaesthesia for caesarean section.

The effects on maternal oxygen saturation, foetal wellbeing and umbilical blood gases were compared when parturients received either 30 or 50% oxygen prior to delivery by Caesarean section under general anaesthesia. Maternal arterial oxygen saturation was significantly increased in the group receiving 50% oxygen. There was no difference between the two groups in terms of Apgar score minus colour, time to sustained respiration or umbilical cord blood gas estimations. The use of 30% inspired oxygen during uncomplicated Caesarean section is advocated.

Temporal trends of human immunodeficiency virus type 1 (HIV-1) infection among inmates entering a statewide prison system, 1985-1987.

Acquired immune deficiency syndrome (AIDS) became the leading cause of death among Maryland State prisoners in 1985. To identify the prevalence, risk factors, and temporal trends for infection with the human immunodeficiency virus type 1 (HIV-1) in the statewide prison system, excess sera were obtained from incoming male inmates during specified periods between April and June 1985, 1986, and 1987. Correctional medical personnel also provided demographic variables of age, race, offense category, sentence, jurisidiction, and an indicator of intravenous drug use. Once rendered anonymous, specimens were assayed for antibody to HIV-1, using ELISA and Western blot techniques. For data from April to June 1985, 1986, and 1987, the crude prevalence of anti-HIV-1 was 7.1, 7.7, and 7.0%, respectively. Although one-third of incoming inmates were identified as intravenous drug users (IVDUs), the drug use variable was missing for 70% of the 1985 sample, and 40% of the 1986 sample. Several strategies were used to examined temporal trends in the context of missing data. Univariate analyses suggested no substantial change over time for either HIV-1 seroprevalence or risk of infection among IVDUs.(ABSTRACT TRUNCATED AT 250 WORDS)

Insulin, dexamethasone and their interactions in the control of glucose metabolism in adipose tissue from lactating and nonlactating sheep.

1. Lactation results in decreased glucose and acetate utilization and increased lactate output by sheep adipose tissue. 2. The ability of insulin to stimulate acetate uptake was lost in adipose tissue from lactating sheep, whereas both the response and the sensitivity (ED50) for insulin for stimulation of glucose conversion into products other than lactate were decreased. These impairments were partly restored by prolonged incubation of adipose tissue for 48 h. 3. The ability of insulin to stimulate lactate output was not altered by lactation. 4. Dexamethasone inhibited glucose uptake, lactate output and glycerol output in adipose tissue from both non-lactating and lactating sheep, with an ED50 of about 1 nM. Dexamethasone inhibited acetate uptake by adipose tissue from non-lactating sheep, but this effect was not observed with adipose tissue from lactating sheep. 5. Dexamethasone inhibited the stimulation of glucose uptake at all concentrations of insulin used; the effect varied with insulin concentration and resulted in an accentuation of the insulin dose-response curve. The insulin dose-response curve in the presence of dexamethasone was muted during lactation. 6. The overall effect of these adaptations is to ensure that glucose and acetate utilization by adipose tissue after an insulin surge is diminished during lactation.

The iminodipropionitrile (IDPN)-induced dyskinetic syndrome in mice: antagonism by the calcium channel antagonist nifedipine.

Chronic administration of IDPN leads to the development of a persistent syndrome which is characterized by lateral and vertical neck dyskinesias, random circling behaviors, and locomotor hyperactivity. Although the dihydropyridine (DHP) calcium channel antagonist nifedipine inhibited all aspects of the syndrome, lateral head dyskinesias (laterocollis) and circling abnormalities were the most significantly affected signs. Dysregulation of calcium-dependent processes might be involved in the pathogenesis of the IDPN-induced dyskinetic abnormalities and clinical disorders of movement in humans.
