Long-term results of coronary angioplasty for single vessel, proximal, left anterior descending disease.

The acute and long-term results of coronary angioplasty in 295 patients with isolated, proximal left anterior descending coronary stenosis are reported. The angiographic success rate was 83.4% overall, but 90.5% for non-occluded arteries treated since 1985. Clinical success at hospital discharge was achieved in 79.7%. The median duration of follow-up was 2.9 years and vital status was established in 99.7% at census. Cumulative 5-year cardiac survival was 96.2% after successful angioplasty and 95.6% for all patients. Five-year freedom from all cardiac events including cardiac death, myocardial infarction and repeat intervention was 73.8% amongst successfully treated patients, and 63.0% for all patients. After angioplasty, patients had less angina, required less anti-anginal medication and were more likely to be in gainful employment. Our data indicate that coronary angioplasty is an effective long-term treatment for selected patients with single vessel disease involving the proximal left anterior descending coronary artery.

Hemodynamic consequences of arrhythmias.

Hemodynamic effects during arrhythmias may be caused by underlying pathology (i.e., infarction) as well as disturbance of rate or conduction pattern. In all arrhythmias, compensatory mechanisms tend to restore normal hemodynamics, and with good left ventricular function this can be achieved despite wide disturbance of rhythm. Hemodynamic effects of ectopic beats can result in dramatic fall of stroke volume and reduction in cardiac output, which is greater for ventricular than for atrial ectopics. Prolonged tachycardias are also tolerated up to far higher rates (180/min) if they are atrial, not ventricular, in origin. Mean blood pressure is often maintained even when systolic pressure and cardiac output are reduced. Even in healthy young subjects it is possible for cardiac ischemia to be induced by excessive heart rates. Some of the most deleterious effects are produced by simultaneous atrial and ventricular contraction, which results in continued suppression of cardiac output, both during tachycardias and at normal heart rates. Such situations are often highly symptomatic. Few measurements are available during external chest compression, and these suggest only marginal improvement in hemodynamics, with low pressures and output.

Planar imaging techniques used with technetium-99m sestamibi to evaluate chronic myocardial ischemia.

The results of published and some unpublished studies comparing planar imaging performed with 2 radionuclides, thallium-201 (T1-201) and technetium-99m (Tc-99m) sestamibi, are reviewed. The average sensitivity for the detection of coronary artery disease (CAD) in studies involving 594 patients was 85% (range 73 to 96%). The average sensitivity for individual vessels was 65% (range 60 to 70%). The average segmental concordance between T1-201 and Tc-99m sestamibi was 89%. End-diastolic gated perfusion images improved the concordance between Tc-99m sestamibi and angiography in 22 patients from 83.4 to 87%. Semiquantitative analysis increased the concordance between T1-201 and Tc-99m sestamibi from 89 to 91%. Ventricular function derived from gated Tc-99m sestamibi perfusion images showed a significant correlation with echocardiography (n = 62, r = 0.85); with angiography (n = 70, r = 0.91); and with equilibrium radionuclide ventriculography (n = 18, r = 0.86). The ratio of lung to left ventricle uptake and the ratio of right ventricle to left ventricle uptake was assessed. Eight of 52 patients had an abnormally elevated lung index (greater than 42%) and these patients had the most severe CAD. Six of the 52 patients had an abnormally elevated right ventricular index (greater than 56%) and these patients had more severe CAD.

Exercise myocardial perfusion scintigraphy with technetium-99m methoxy isobutylisonitrile: a comparative study with thallium-201.

The evaluation of technetium-99m methoxy isobutylisonitrile for the diagnosis of coronary artery disease requires comparative validation against thallium-201, the established perfusion imaging agent. We have compared myocardial and lung uptake of both radiotracers following maximal exercise in 52 patients: 40 with angiographically proven coronary disease. Qualitative and quantitative image analysis showed the diagnostic sensitivity of technetium-99m methoxy isobutylisonitrile to compare favourably with that of thallium-201 as reflected by the mean number of ischaemic segments identified: 5.6 +/- 2.5 vs 4.8 +/- 2.1 by qualitative analysis, and 5.7 +/- 3.2 vs 5.0 +/- 2.6 segments by quantitative analysis. More reversibly ischaemic segments per patient were identified with technetium-99m methoxy isobutylisonitrile than with thallium-201: 3.6 +/- 2.3 vs 1.8 +/- 1.9. There was a higher exercise myocardial to background count ratio with technetium-99m methoxy isobutylisonitrile: 3.16:1 vs 2.58:1, and the mean exercise lung uptake normalised to left ventricular uptake ('lung index'), was lower for technetium-99m methoxy isobutylisonitrile than for thallium-201 (36 +/- 8% vs 40 +/- 10%). Five of the six patients with abnormal elevation of the thallium-201 exercise lung index also had elevation of the technetium-99m methoxy isobutylisonitrile exercise lung index, and all had extensive coronary artery disease. These results indicate that technetium-99m methoxy isobutylisonitrile is at least as effective as thallium-201 for detecting exercise induced myocardial ischaemia. However, technetium-99m methoxy isobutylisonitrile provides a better image quality and may be a more sensitive marker of defect reversibility. For both radiotracers lung uptake is increased with extensive coronary artery disease and measurement of this variable provides prognostic information.

Normal and stenotic human aortic valve opening: in vitro assessment of orifice area changes with flow.

The ability to measure aortic valve area clinically has emphasized the need to understand the changes in aortic valve orifice area during flow. To compare the performance of normal and stenotic human aortic valves we used a pulsatile flow model that simulated in vivo flow conditions. Five normal autopsy specimens and 15 stenotic valves removed at operation were mounted into the model. Valve function was assessed by analysis of video recordings of valve leaflet motion during flow. Over the flow rates tested normal valves demonstrated a linear increase in orifice area. There was no resistance to leaflet opening and valve closure was rapid. The majority of stenotic valves demonstrated an increase in orifice area at low flow rates. No valve showed any increase in maximal area beyond flow rates of 3 l min-1. Increased leaflet resistance of these abnormal valves resulted in notably slower opening and closing rates. In patients with a high cardiac output and severe stenosis, overestimation of the anatomic orifice area derived by the Gorlin equation can result. This is not related to variability in maximal orifice area.

Aortic Valve↗

In vitro measurement of stenotic human aortic valve orifice area in a pulsatile flow model. Validation of the continuity equation.

Aortic valve orifice area estimation in patients with aortic stenosis may be obtained non-invasively using several Doppler echocardiographic methods. Their validity has been established by correlation with catheterization data using the Gorlin formula, with its inherent limitations, and small discrepancies between the methods are present. To evaluate these differences further, 15 patients with severe aortic stenosis (mean transvalvular gradient 70, range 40-130 mmHg) had aortic valve area estimations by Doppler echocardiography using two variations of the continuity equation. The intact valves removed at valve replacement surgery were then mounted in a pulsatile model and the anatomical area was measured (mean 0.67 +/- 0.17 cm-2) from video recordings during flow at 5.4 l min-1. Aortic valve area calculated using the integrals of the velocity-time curves measured at the left ventricular outflow tract and aortic jet (mean 0.65 +/- 0.17 cm2) correlated best with the anatomical area (r = 0.87, P less than 0.001). The area derived by using the ratio of maximum velocities from the left ventricular outflow tract and aortic jet (mean 0.69 +/- 0.18 cm2) also correlated well with the anatomical area (r = 0.79, P less than 0.001). The index between the left ventricular outflow tract and aortic jet maximum velocities was less than or equal to 0.25 in all. In patients with severe aortic stenosis the aortic valve area can be reliably estimated using Doppler echocardiography.

Quantitative analysis of contribution of rate response in three different ventricular rate responsive pacemakers during out of hospital activity.

The contribution of rate response provided by three different types of rate responsive pacemaker (Sensolog, Meta, and Vitatron TX) was assessed in 25 patients during out of hospital activity. Pacemakers were optimally programmed and full disclosure 24-hour Holter ECG recordings were obtained during everyday activities. Close visual analysis of these tracings revealed that rate response is active for less than 15% of the 24-hour study period with high rate response (paced rate above 100 beats/min) for just 5% of the day showing peak activation between 1600 and 2000 hours. Low rate response (paced rate below 100 beats/min but 5 beats above base rate) was maximal between 0800 hours and midday and comprised 10% of all heart beats during the study period. Thirty percent of study patients were nonpacemaker dependent and rate response in these subjects was significantly (P less than 0.001) less active between 0800 hours and midday than in pacemaker dependent patients though it was equivalent at all other times. Nonpacemaker dependent patients achieved natural sinus response in the morning and tended to rely on pacemaker supplied rate response in the latter part of the day.

Is accurate rate response programming necessary?

Exercise capacity and general well-being are improved by appropriately programmed rate responsive pacemakers when compared to fixed rate units. Ten patients had activity sensing DDDR units implanted for combined AV block and sinus node incompetence. Ten patients had Sensolog activity sensing VVIR units implanted for complete heart block. The effects of over and under programming of rate response in both dual and single chamber activity sensor rate adaptive pacemakers has been assessed subjectively by visual analog scales and specific activity questionnaires and objectively by graded treadmill testing and the performance of standardized daily activities. Patients were randomly programmed to absent rate response (VVI in the Sensolog group), hyporesponsive (DDD in the dual chamber group), appropriate response (VVIR, DDDR according to Manufacturer's instructions) and over responsive (VVIR+, DDDR+) in a double-blind crossover design. Thirty percent of patients demanded early crossover from VVI, 30% from DDDR+ and 50% from VVIR+. Perception of Exercise Capability was similar to objective exercise treadmill times which were shorter in VVI than in VVIR or VVIR+ (P less than 0.05) or control subjects (P less than 0.001). There was no difference between any dual chamber mode or control subjects. General well-being was poorest in DDDR+ and VVIR+ modes despite objective improvement in exercise capacity. Symptoms were least in VVIR and DDDR and all but one patient chose appropriate programming as their overall preferred mode. Thus, even inaccurate rate response programming results in similar and improved exercise capacity compared to absent rate response but overprogramming is unacceptable to most patients, confirming that appropriate programming and sensor specificity is critical in rate responsive pacing.

Inappropriate discharges by the implantable cardioverter defibrillator during postoperative testing: implications for intraoperative assessment.

Inappropriate shocks were delivered to a patient while in sinus rhythm by an implantable cardioverter defibrillator (ICD) during routine prehospital discharge testing. This was induced by the standard programmer when the "read" telemetry sequence was initiated. The ICD was removed and found to suffer from electrical artifact that was sensed as ventricular tachycardia during telemetry. To avoid inadvertent telemetry-induced shocks during routine testing, all ICDs should be interrogated, using a standard programmer, intraoperatively, with the unit in "defibrillation on" mode.

Supraventricular tachycardia control with Tachylog II: long-term follow-up.

Ten patients aged 16-63 years (mean 36.3) had the Siemens P56T "Tachylog II" pacemaker implanted for treatment of drug refractory supraventricular tachycardia. The pacemaker incorporates a noninvasive electrophysiological study (EPS) facility and a sophisticated Holter function combined with a unique "learning" self-search antitachycardia algorithm. The Holter reveals that new tachycardias arise that are not previously detected at conventional EPS. The number of stimuli in the initiation sequences during noninvasive EPS proved highly variable, however, termination sequences remained constant in the long term. There was variability of timing of stimuli in the long term that was significantly greater for "new" tachycardias than for "original" arrhythmias. Long-term follow-up (at 1 month, 6 months, and 1 year) have shown that 90% of patients have good tachycardia control without the need for drugs. All patients confirm the acceptability of this pacemaker for control of their arrhythmias in the long term.

Balloon dilatation of the aortic valve in a pulsatile flow model: assessment of the mechanisms and the magnitude and duration of changes in valve area and gradient.

Eighteen stenotic aortic valves (17 removed at operation) mounted in a pulsatile flow duplicator were dilated with a balloon catheter. Sequential measurements showed that the valve area initially increased from a mean (SD) of 0.52 (0.16) to 0.78 (0.17) cm2. It was 0.73 (0.16) cm2 five minutes after dilatation and this was little changed at four weeks (0.70 (0.15) cm2). Initially the mean transvalvar gradient fell significantly from 54 (27) to 32 (8) mm Hg but increased to 35 (10) mm Hg at five minutes and to 40 (11) mm Hg at four weeks. In six valves stretching of the orifice was the only mechanism responsible for the changes while in the remainder there was tearing through commissures with a greater initial increase in area (0.31 v 0.18 cm2) and a smaller decrease in area at five minutes (0.03 v 0.08 cm2). Fractures of calcific deposits in non-commissural positions were seen in one valve only. This laboratory study of isolated aortic valves showed a significant but small increase in valve area after balloon dilatation, which was greater when commissural tearing had occurred. Recoil of the stretched orifice was complete at five minutes and there was little further change over the next four weeks.

Comparison of the normal sinus node with seven types of rate responsive pacemaker during everyday activity.

The heart rate response of 59 patients aged 17-79 years implanted with seven different types of rate responsive pacemakers was evaluated during graded exercise treadmill testing and during standardised daily activities. The heart rate response in patients with pacemakers was compared with the chronotropic response in 20 healthy controls of similar age and sex distribution who performed identical protocols. All pacemaker types adequately simulated the control heart rate response during the graded exercise treadmill test except during the early stages of exercise. However, during everyday activities, the response of ventricular rate responsive (VVIR) pacemakers was varied. Activity sensing systems rapidly overresponded to staircase descent, to changes in walking speed, and to suitcase lifting with the pacemaker arm, and these systems did not respond to mental stress. "Physiological" sensors (QT and minute ventilation units) responded slowly to rapid changes in physiological demand. The QT pacemaker patients did respond to mental stress but showed a paradoxical increase in rate during the recovery phases of burst exercise protocols such as staircase ascent/descent and walking deceleration. Dual chamber pacemakers in VDD, DDD, and DDDR modes most closely simulated the normal chronotropic response during everyday activities. Graded exercise treadmill testing, in isolation, may not be the best way to asses or program the heart rate response in patients with the heart rate adaptive pacemakers because changes in heart rate during everyday activities may deviate considerably from the normal sinus response despite satisfactory simulation of the normal chronotropic response during treadmill testing.

Clinical outcome of coronary angioplasty for single-vessel disease.

The acute and long-term results of coronary angioplasty in 412 patients with single-vessel coronary artery disease are reported. Angiographic success was observed in 85.5% of all lesions and 84.7% of all arteries in which dilatation was attempted. Success rates improved during the study; since 1985, the angiographic success rate for non-occluded arteries has been 91.0%. Early complications occurred in 6.8%; angioplasty achieved clinical success in 80.3% of all patients at discharge from hospital. The median duration of follow-up was 772 days, with clinical status established for 98.8% of patients. 5 years after clinically successful angioplasty, cumulative cardiac survival was 96.6% and freedom from cardiac death and myocardial infarction was 90.4% (95.8% and 85.4%, respectively, on intention-to-treat analysis). Within 5 years of successful angioplasty, 12.4% of survivors underwent coronary bypass surgery and 16.9% had repeat angioplasty. Freedom from all cardiac events at 5 years was 70.2% after clinically successful angioplasty and 60.2% on intention-to-treat analysis. Angina symptoms were improved in over 80% of patients and abolished in 75%.

Multicenter long-term results of antitachycardia pacing for supraventricular tachycardias.

This report describes the multicenter experience of the Tachylog antitachycardia pacemaker (Siemens-Elema) in the treatment of drug-refractory, recurrent supraventricular arrhythmias. The device has been implanted in 63 patients (mean age 47 years). The arrhythmias were atrial tachycardia in 4 patients (6%), atrioventricular nodal reentry tachycardia in 23 patients (37%), circus movement tachycardia via an overt bypass tract in 17 patients (27%) and via a concealed bypass tract in 19 patients (30%). The mean follow-up period was 30 months. In 28 patients (44%) arrhythmia control was achieved with the pacemaker therapy alone. In 31 patients (49%) drug therapy had to be reintroduced to obtain control of recurrent arrhythmias. Four patients (6%) were definite nonresponders. In 4 patients (6%) the pulse generator was explanted either because it was not tolerated or because of tachycardia-sensing failure. No syncope or death was observed during follow-up. Thus, antitachycardia pacing gives satisfactory results in selected patients with drug-refractory supraventricular tachycardias.

The evaluation of ventricular function using gated myocardial imaging with Tc-99m MIBI.

Left ventricular function is an important prognostic indicator in patients with coronary artery disease. We have assessed a method of providing this information as an adjunct to myocardial perfusion imaging using Tc-99m MIBI (2-methoxy-2-methyl-isopropyl-1-isonitrile). Two separate studies, at rest and during exercise, were performed following an injection of 400-600 M Bq of Tc-99m MIBI in 62 patients. Cardiac gating permitted excellent myocardial edge definition during the cardiac cycle. Radionuclide fractional shortening (RFS) was calculated from the anteroposterior (AP) and the septum to lateral wall (SL) axes in diastole and systole. Results were compared with echocardiographic fractional shortening (EFS) and the ejection fraction (EF) obtained from the gated equilibrium blood pool using Tc-99m-labelled red blood cells. The RFS in the AP axis correlated closely with echocardiographic FS (r = 0.89, P less than 0.001). The RFS in both axes was averaged to provide a global RFS. Global RFS correlated closely with LV radionuclide EF (r = 0.83, P less than 0.001). Inter- and intra-observer reproducibility studies have shown a variability for the procedure of less than 10%. In conclusion, gated perfusion imaging with Tc-99m MIBI, provides useful functional information as an adjunct to perfusion imaging.

Coronary angioplasty and left ventricular function in single vessel coronary artery disease.

Left ventricular function was investigated in 86 patients with single vessel coronary artery disease before and three to six months after successful angioplasty. Before angioplasty thallium-201 perfusion scintigraphy and technetium-99m gated equilibrium ventriculography in most patients showed that stress testing (exercise and ice water stimulation and isometric handgrip respectively) induced myocardial perfusion defects that were associated with a mean (SD) drop in left ventricular ejection fraction from 64 (6)% to 56 (7)%. After angioplasty there was residual coronary stenosis of less than or equal to 20% of the diameter of the vessel in 78 patients (group 1) and of between 20 and 50% in eight patients (group 2). After the procedure the perfusion defects seen during stress resolved in 86% of group 1 and in 87% of group 2. Despite the apparent improvement in myocardial perfusion left ventricular dysfunction persisted in group 2--that is during stress the left ventricular ejection fraction fell from 65% (6) to 56% (5). In group 1, on the other hand, the improvement in myocardial perfusion was associated with significant improvement in left ventricular function with a normal increase in ejection fraction from 63 (5) at rest to 67 (6) during stress. Radionuclide studies, one to six weeks after angioplasty in 30 group 1 patients showed continuing left ventricular decompensation during stress in nine (30%) of them despite correction of perfusion defects. But reinvestigation three to six months after the procedure showed recovery of left ventricular function with an increase in ejection fraction from 66 (5) at rest to 69 (7) during stress. These data indicate that coronary angioplasty procedures that give a residual stenosis of </= 20% improve myocardial perfusion and the response of the left ventricle to stress. The functional improvement may be delayed for up to three months, however, possibly because arterial healing at the angioplasty site is delayed. On the other hand, when the residual stenosis is between 21 and 50% of the diameter of the vessel subclinical left ventricular dysfunction during stress may persist indefinitely.

Staffing in cardiology in the United Kingdom 1988. Fifth biennial survey.

This is the fifth survey of staffing (consultants and senior registrars) in cardiology in the United Kingdom. Data from previous years--including the fourth survey (1986) that was not published separately--are used to show the trends over the past decade. In 1988 there were less than six cardiologists per million population. The United Kingdom, with Ireland, has fewer cardiologists than all other European countries with reliable figures. The ratio for Europe as a whole is approximately 45 per million population; the recommended figure for the United States of America is 60 per million. The distribution of cardiologists in England and Wales is still very uneven. Seven million people--nearly 15% of the population--have no immediate access to special expertise in cardiology. Women are particularly poorly represented in the specialty. This survey indicates that the crisis in staffing for cardiology continues. It will worsen as the possibilities grow for effective management of heart disease and as needs increase with the greater average age of the population. Resolution of this crisis should be a major priority in policies aimed at countering the ravages of heart disease.
