[Problems of diagnosis and treatment of cervicitis].

Infectious cervicitis is a common disease in women of reproductive age. The symptoms are not characteristic and they are often neglected by the patient and the physician. Cervicitis is a very important clinical entity, because the infection can ascend to the genital tract. The most common causes for the infectious cervicitis are Chlamydia trachomatic and Neisseria gonorrhoeae. A variety of methods for the precise diagnosis must be employed--gynecologic examination, colposcopy, pathomorphology and cytology and bacteriological examination as well. Therapeutic modalities must include coverage for both Chlamydia trachomatis and Neisseria gonorrhoeae. Bacteriological test of cure is mandatory.

[The appearance of ciprofloxacin resistance in the microbial strains isolated from gynecologic patients].

The sensitivity of 415 microbial strains isolated from clinically taken samples from gynecologically diseased women was determined. The urine strains showed sensitivity up to 93.3%, but 7 Gram/-/ strains isolated in the last 3 months of the study were resistant to Ciprofloxacin. The cervical secretion strains, those of CD aspirates, of IUP and wound secretions were found sensitive up to 81.2%. The 50 Enterococcus strains isolated in gynecological infections were found in 28% resistant to Ciprofloxacin, with 7 of the 19 resistant strains were isolated in the last 3 months of the study. The appearance of resistant strains in some microbial species, pointed out in the literature as markedly sensitivity to Ciprofloxacin, as well as the data of a fast developed resistance Ciprofloxacin in the course of treatment, allowed us to recommend their application in the infections caused by microorganisms resistant to the habitual antibiotics.

[Clinico-microbiological findings in women with a genital discharge].

The authors compare the prevalence of the most common vaginal infections at present and before 10 years and find that the prevalence of Trichomonas vaginalis infection is diminished and there is a tendency for the increase in the incidence of bacterial vaginosis. The determination of the vaginal pH and the application of 10% KOH for the "sniff test" may prove very useful in the diagnosis of vaginal infection.

[The identification of species of the genus Candida].

A new CHROMagar selective medium (Becton Dickinson) was tested. Strains of Candida species isolated after 48 hours were identified by specific dying of the colonies on the selective medium. The inoculation data were verified by filamentation and fermentation tests. No differences were found. Together with the most frequently isolated species C. albicans, in 20% of the cases we found C. glabrata considered recently as a cause of the recurrent vaginal candidosis with ketoconazole and 5-flucytosine resistant strains.

[The bacterial vaginosis problem].

The changing concepts through the years concerning this condition are briefly reviewed. The pathogenesis of the cardinal symptoms are explained, concerning the changes in the vaginal ecosystem. The importance of the complex clinical criteria is pointed out including the determination of the vaginal pH, the test with 10% KOH and the observation of "clue cells".

[A direct microscopic preparation of the gastric aspirate is an aid in the diagnosis of congenital neonatal infection].

The date of direct microscopic preparation (DMP) from gastric aspirate were compared with microbiological investigations of the same gastric aspirate, the ear secretion, blood culture in 112 newborn premature babies, on the day of their birth. A negative (-) DMP (without leucocytes and microorganisms) completely coincided with negative (-) cultures in 94 babies. Correspondence between the data of DMP (more than 4 leucocytes on field and present of microorganisms) and the isolated microbial species from the gastric aspirate, ear secretions and blood-culture was found in 18 babies. The most frequently isolated microbial species in our investigation were--Streptococcus group B, srt. gr. D, S. aureus, E. coli, C. albicans. The DMP of gastric aspirate from the newborn proved to be a valuable diagnostic method, which gives the possibility of the clinician for a fast information concerning the condition of the baby in the first hours after birth.

[The bacterial vaginosis problem. II. The microbiology of bacterial vaginosis].

Microbial species in the vaginal content of healthy woman in her reproductive age were described, as well as the importance of hydrogen peroxide (H2O2) producing Lactobacillus spp. The characteristics of the basic microbial species associated with the BV was presented. Discussed were the various possible methods for BV diagnosing. The coloured microscopic preparation was accepted and applied as the main one in the routine practice.

[The therapeutic potentials of the vaginal antimicrobial preparation Chlorchinaldin].

The aim of this study is the clinical testing of the vaginal wide-range, antimicrobial, quinolone--Chlorchinaldin/0,2/by POLFA, which has a strong antibacterial, antifungal, trichomonal and keratoplastic effect. The medicine was used on 43 patients with complaints of aggravated fluor. The clinical, colposcopic and microbiologic study showed: candidiasis--in 16 trichomoniasis--in 8, bacterial vaginalis--in 3 and anaerobic vaginitis--in 2 patients. The rest of the women (14) had various aerobic microbial findings. The treatment was daily using one vaginal tablet, every night for 10 days. A control examination was carried out one week after the any of therapy. The results showed a positive clinical effect, since in 67.4% there were negative microbiological findings. The lack of effect in cases with bacterial vaginosis and anaerobic vaginitis was painted out. These results give reason to believe that the wide antimicrobial range of Chlorchinaldin B will be complementary to the currently used drugs in the treatment of vaginal infections.

[The sensitivity to the preparation cefotaxime of bacteria isolated from newborn infants].

Four hundred forty-four of S. aureus and 54 strains of E. coli were isolated from the neonates, born during the first six months of 1994 in the Maternity Hospital "Maĭchin dom". They were tested with the antibiotics--ampicilin, cephamandol and cefotaxime. Our results showed that 14.5% of the tested S. aureus were sensitive to ampicilin (e.g. broad-spectrum penicillins) and 85.5% resistant. Twenty percent of the strains E. coli were sensitive to ampicilin and 80% showed resistance. Fourty nine percent of the tested S. aureus were sensitive to cephamandol (a second generation cephalosporine) and 51% were resistant; 46.6% of the isolated strains E. coli were sensitive to cephamandol and 53.4% were resistant. Over 87% of the tested S. aureus were sensitive to cefotaxime, a third generation cephalosporine, and only 13% were resistant. From the tested strains E. coli 92.3% were sensitive to cefotaxime and 7.7% were resistant. The present study proved a high sensitivity of the isolated strains to cefotaxime. In addition this antibacterial drug showed good tolerance by the neonates when given in two doses per day and a lack of suppressive effect on the normal bacterial flora of the intestinal tract of the neonates.

[The sensitivity to antibacterial preparations of enterococcal strains isolated from gynecological patients, pregnant women, puerperae and newborn infants].

The sensitivity to a given number of antibiotics of 205 strains of enterococci, isolated for a period of one year was studied. The enterococci were isolated from materials, sent for microbiological examination in different clinical settings 55 strains of enterococci from the lower genital tract were tested (vaginal and cervical secretions), 36 strains from the upper genital tract (lochia, peritoneal aspirates, IUD), 8 strains from wounds (perineal and abdominal), 37 strains from urine and 69 strains from the newborns. There was a high sensitivity towards carbenicillin--92.7% towards azlocillin 91.2% and towards ampicillin--83.9% The strains, isolated from gynecologic patients showed increased resistance towards aminoglucosides--49.5% towards gentamycin and 46.5% towards amicacin. The resistance towards gentamicin in the newborns was 68.1%. The strains from urine were resistant to cyprofloxacine in 29.7%. All the enterococci tested were practically resistant to cefamandole and oxacilline. The author suggests that for initial therapy in proven or suspected enterococcal infection carbenicilline, azlocillin and ampicillin. They are suitable both for gynecologic patients and newborns. The aminoglucosides have to be administrated after testing because of the significant resistance found for the different strains. The same is true for erythromycin and cyprofloxacine
