[Dynamic tibio-fibular fixation with the Rahmanzadeh syndesmosis plate].

In 58 patients of 585 fractures of the ankle we demonstrated the disruption of the tibiofibular syndesmosis. Near to osteosynthetic repair the tibiofibular diastrasis was restored with the syndesmosis-plate by Rhamanzadeh. Postoperative treatment happened plaster-free. Complete weight bearing was achieved pain-dependend after 2 until 4 weeks. This equipment is following from the dynamical fixation of the tibiofibular syndesmosis by preserving the physiological movements between tibia and fibula. Removement of syndesmosis-plate ensued with the remaining osteosynthetical material after one year.

Brain evoked responses, a bioassay for central actions of adrenocorticotropin (ACTH 1-39) and corticotropin releasing hormone (CRH) in humans.

Blood borne hormones of the hypothalamus-pituitary-adrenal (HPA) axis form an afferent humoral system modulating a great variety of brain functions. In humans, bioassays consistently reflecting central nervous system actions have not been developed, so far. The present experiments are part of a series of studies which have been conducted to demonstrate the afferent influences of ACTH and CRH on brain activity by recording of auditory event-related brain potentials (ERPs) in healthy human subjects. In a double-blind within-subject comparison in 12 healthy male students, influences of 4 U ACTH and 100 micrograms CRH (infused within 2 h; the infusion starting 1 h prior to ERP recordings) were compared with the effects of a placebo infusion. Influences of the hormones were tested for the early, brain stem generated ERP components (BAEPs) and for the late components of the ERP associated with cortical stimulus processing. ACTH and CRH showed no consistent effect on BAEPs. ACTH, but not CRH, significantly reduced amplitudes of late ERP components, in particular, the Nd and P3 components which are considered signs of selective attention. These results are consistent with findings from previous studies demonstrating impaired ERP signs of selective attention in humans after administration of the 4-10 fragment of ACTH which lacks the peripheral adrenocorticotropic activity. Together with these foregoing studies, results from the present experiments further establish ERP methodology as a sensitive bioassay for CNS actions of hormones in humans.

[The effect of physical training on the pharmacokinetics of steroidal contraceptives].

In 8 female sports students (runners) aged 20 and 21 years and 10 women between 20 and 40 years of age, who regularly underwent body building training, serum levels of ethinylestradiol (EE2) and levonorgestrel (LNG) have been determined under conditions of exercise as well as in rest, prior to and following the intake of 1 tablet of Gravistat (0.05 mg EE2, 0.125 mg LNG). Blood samples were obtained at points 0, 2, 6 and 24 hours. Exercise (5000 meter running and intensive body building training of one hour duration, respectively) was started immediately after intake of the steroid combination. There were no significant differences in the serum concentration levels obtained during exercise and rest. But exercise consistently resulted in a slight decrease of EE2 and LNG serum concentrations and the area under curve values.

[Clinical and immunologic effects following oral immunization against influenza in children in day care and infection susceptible young children].

57 infants and small children (9 months - 3.1 years) were orally immunized by an inactivated influenza vaccine (Mississippi 1/85; dosage: 110 micrograms HA); the control group (n = 15) received placebo. After three months the influenza specific Ig-concentrations of serum and secretions demonstrated a controverse course: Specific IgG (serum) decreased and specific IgA (nasale secretions) increased statistically significant (p less than 0.025), especially in children suffering from frequently relapsing respiratory infections (n = 31). Moreover the immunized children had also a better clinical outcome in the following 3 months: the number of days with cough, febrile symptoms and the antibiotics therapy were significantly decreased. Recommendations are given for an improvement of the orale influenza vaccine and its indicated administration in small children.

Isolation of Inoue-Melnick virus and detection of antibodies in coded cerebrospinal fluid specimens from patients with disorders of the central nervous system.

Inoue-Melnick virus was isolated from coded cerebrospinal fluid samples from 10 of 25 patients with chronic diseases of the central nervous system. Of 13 multiple sclerosis patients and 12 patients with a variety of other symptoms and signs 4 and 6, respectively, were positive. Replicate samples from the same patient yielded the same results, either positive or negative. Neutralizing antibody was present in cerebrospinal fluid at a dilution of 1:25 in the virus-positive patients. Only a single virus-negative patient was positive for antibody at this dilution.
