[Arthrolysis and arthroplasty of the elbow joint. A comparison of surgical results between children and adults].

Because of stiffness of the elbow joint until 1988 146 operations have been done in 132 patients on the Department of Orthopaedic Surgery in Heidelberg. 55 procedures have been done in children and adolescents younger than 16 years, 91 times in adults. In both groups posttraumatic stiffness and stiffness of other origin have been treated. Procedures have been mobilisation in anesthesia, simple or enlarged arthrolysis as well as arthroplasties. Results have been evaluated by the method of Cauchoix and Deburge (relative gain of range of movement). In nontraumatic stiffness of the elbow younger patients under the age of 16 years showed better results than the adults, in posttraumatics the results seemed to be better in the adults. Mere mobilisation in anesthesia showed disappointing results in both groups generally.

[Results of surgical therapy of knee flexion contractures in patients with myelomeningocele].

This study reports the results of 98 operations for correcting knee-flexion-contractures, which were performed between 1972 and 1989 in 60 patients with myelomeningocele. 13 knees had hamstring lengthening and 85 knees had radical flexor release. In 4 patients, who had flexion-contractures of more than 50 degrees, the soft tissue release was combined with supracondylar extension osteotomy. 58 patients had additional hip-flexion-contractures and 39 patients had feet deformities. The average age at time of surgery was 8 years, 3 months. The average follow-up-period was 65 months with a minimum of 1 year and a maximum of 13 years. In 92 knees a permanent extension ability could be achieved. In 6 knees a recurrence of flexion-contracture occurred making a second surgery necessary. The main problem in the postoperative period were 13 skin necroses, which were seen mainly after an s-shaped incision. As a conclusion a perpendicular midline incision is recommended. After surgery 11 patients could be provided for the first time with an orthoses. In the remaining the upright body position or the erection of the orthoses could be improved.

[Spinal surgery on patients with myelomeningocele. Results 1971-1990].

Between 1971 and 1990 33 kyphectomies were performed at the Orthopaedic Hospital, University of Heidelberg. The average age at time of surgery was 5 years/5 months; the follow-up period was 5 years. The best results were obtained in the group with congenital rigid kyphosis. A marked postoperative deterioration was considered in patients with a flexible paralytic kyphosis, probably due to a too short area of fusion. In 19 patients the correction of the kyphotic deformity was the prerequisite for being provided with orthotic devices. The most serious complication was the intraoperative death of 3 patients. It can be presumed, that after transection of the spinal cord a non-functioning CSF-shunt lead to an acute elevation of the intracranial pressure. Therefore an adequately functioning shunt must be secured preoperatively.

Intestinal milk-bolus obstruction in formula-fed premature infants given high doses of calcium.

Supplementation of milk formulae with calcium salts is recommended for low birth weight infants. We observed a serious complication of calcium administration in too high a dosage. Three premature infants (birth weight 1,080-1,900 g and gestational age 28-35 weeks) developed an obstruction of the distal ileum during the second week of life, leading to multiple bowel perforations and development of septic peritonitis in two cases. All three patients required surgical removal of a bolus blocking the intestinal lumen. Analysis of the bolus material from one infant indicated that the formation of soaps from calcium and long-chain fatty acids had caused the obstruction. All three infants had been fed a formula for premature infants with the addition of a recently introduced commercial mineral supplement, resulting in a very high calcium concentration of the feed (40.6 mmol/L). Simulation of syringe feeding demonstrated that even higher calcium concentrations could occur in milk portions containing sedimented calcium compounds.

[Heterogeneity of antimicrosomal autoantibodies in chronic hepatitis C virus infection and delta hepatitis].

Microsomal antigen autoantibodies are typical of type 2 autoimmune hepatitis, and a strong association with chronic hepatitis C virus (HCV) infection has been reported in certain geographical areas. These autoantibodies have been denominated LKM-1 to differentiate them from those associated with thienylic acid-induced hepatitis (LKM-2) and from those seen in patients with chronic delta hepatitis (LKM-3). To investigate the antigenic specificity of autoantibodies associated with chronic hepatitis C and delta, we analyzed 52 LKM-1 positive serum samples from patients with chronic hepatitis C and 17 LKM-3 positive serum samples from patients with chronic delta hepatitis by indirect immunofluorescence and Western blotting (immunoblotting). Reactivity of subjects with chronic hepatitis C was heterogeneous: only 5 out of 52 LKM-1 positive patients, tested by Western blot, recognized a single protein of 50 kD, previously identified by Manns et al. with an immunogenic epitope of cytochrome P450IID6. Thirteen of the 52 patients also reacted with a 70 kD microsomal protein, and 12 out of 52 reacted only with a 59 kD protein. Twenty-two sera, notwithstanding the high titer in immunofluorescence, did not evidence any reactivity when tested by Western blot. The same sera tested positive in LKM-1 ELISA when solubilized human microsomal proteins were used. Fourteen out of 17 LKM-3 positive sera from patients with chronic hepatitis delta recognized a 55 kD microsomal protein in Western blot; three sera, HCV and HIV positive, did not react with any protein by Western blot. None of these sera was positive in ELISA LKM-1.(ABSTRACT TRUNCATED AT 250 WORDS)
