Computed tomography in the diagnosis and differential diagnosis of glioblastomas. A qualitative study of 295 cases.

The CT patterns of 295 glioblastomas examined with pre- and postcontrast scans using an EMI scanner Mark I (Matrix 160/160) have been reviewed and compared with the CT appearances of other brain tumors, metastases and abscesses. There is a great variety of CT patterns with glioblastomas. However, a garland-shaped CT appearance, representing a subgroup of ring-shaped lesions, seems to be most typical for glioblastomas since it was observed in 19% of ring-shaped glioblastomas but in only one out of 172 metastases and in no case of an astrocytoma grade II or an abscess in our series. The initial CT diagnosis, based on the CT finding, the patient's history and the clinical data, was correct in 69.8% of the glioblastomas, 41 recurrent glioblastomas included. In 12% of the cases the presence of a glioblastoma was within differential diagnosis. These results lead to the conclusion that in many cases additional diagnostic methods, such as serial scintigraphy and/or cerebral angiography, are required for a reliable differential diagnosis.

Serial CT studies of a metastatic pinealoma with reference to the radiotherapeutic problems.

Serial CT studies performed for 1 year on a female patient suffering from a pinealoma are reported. Under cobalt therapy (5000 rads) a remission of the pinealoma was achieved. A short time later, metastases developed and cobalt therapy was repeated (6000 rads). Rapid tumor remission was seen again, but rapid occurrence of probable metastases too. The question whether radionecrosis or metastases were present cannot be answered.

Renal phlebography: an aid in the diagnosis of the absent or non-functioning kidney.

There are clinical situations that may preclude the use of retrograde pyelography and renal arteriography in the differential diagnosis of the absent, dysgenetic and non-functioning kidney. At times these procedures are non-diagnostic. We present 8 cases to demonstrate the efficacy of selective renal phlebography as an aid in the diagnosis of these entities.

Serial magnification angiography under hypocapnia and hypertension.

Serial magnification angiography under hypocapnia and hypertension was performed in 200 cases of cerebral tumours and cerebrovascular lesions. The advantages over conventional angiography are evident; the extent of a tumour is easier to determine as well as in most cases the nature of the mass lesion.

[Pantopaque cisternography in diagnosis of acustic neuromas (author's transl)].

In 6 out of 37 patients, examined by pantopaque cisternography the suspect of a acustic neuroma could be strengthened. All of these tumors showed growth beyond the internal auditory meatus. Smaller intrameatal tumors were not found. The indication for cisternography were made almost according to the demands from Fisch and Wegmüller, therefore it is asked, wether it should not be better to follow the methods of Valvassori who sometimes performes cisternography based on audiometric findings alone. As results of our own experience and similar cases reported elsewhere, this seems justified. Because of the small amount of contrast medium used the side-effects of Pantopaque-cisternography are practically nil.

[Encephalographic findings in tumors of the pineal region (author's transl)].

6 encephalograms were selected from a series of 31 patients with clinical and radiological signs of a tumor in the pineal region, in order to obtain differential diagnostic criteria concerning space occupying lesions of this area. Neuroradiologically, it is possible only in a very few cases to decide whether a tumor had arisen from the pineal gland or from adjacent structures of the midbrain. At the time of the radiological examination usually the tumor has reached a size that an encephalographic distinction between pineal gland and quadrigeminal plate cannot be made. Computerized axial tomography can contribute essentially to solve this problem as shown by one example.

[Pantopaque-myelographic findings in brachial plexus root avulsion (author's transl)].

The myelograms of 7 patients with traction injury of the brachial plexus are compared with the operation findings. It appears that not only the roentgenological sign of the "traumatic meningocele" but also other myelographic abnormalities are to be considered as indications of a nerve root lesion. These changes are demonstrated. The myelogram of a 6-months-old child showing a nerve root avulsion due to birth trauma is represented. Myelographic examination of each patient with posttraumatic brachial palsy should be performed as soon as possible. A sufficient myelographic diagnosis of the nerve root lesion can be achieved only in considering also the less impressive radiological signs.

Dependence of resting urethral pressure on age and sex of healthy subjects and on technical details.

The authors used the Harrison and Constable variation of the original method of Brown and Wickham. The maximal pressure level in the urethral pressure profile depends on technical details. Younger healthy individuals show a higher resistance than older ones. Prostate enlargement leads to an elongation of the posterior urethra. The physiological elasticity loss disposes older persons to stress incontinence.

[Technique, complications, possible misinterpretations and indications of retrograde renal phlebography (author's transl)].

A chronological survey is given of the gradual development of retrograde renal phlebography whereby the different techniques are contraste with each other in detail. Today's method of choice is the retrograde selective visualization of renal veins using the Seldinger procedure via the femoral vein, with or without pharmacological influence and applying larger doses of contrast medium (up to 30 ml) and higher injection pressures (up to 6 kg/cm2). Indications for simple and for pharmacophlebography are specified and exemplified by means of a scheme. Possible complications and misinterpretations are discussed.
