[Stewart-Treves syndrome and lymphangiosarcoma].

Lymphangiosarcomas represent the most dangerous complication of chronic lymphedema usually following mastectomy and irradiation therapy on account of breast carcinoma, well-known as Stewart-Treves syndrome. Still more rarely documented are lymphangiosarcomas arising in chronic lymphedema due to other causes. Two cases of Stewart-Treves syndrome on the forearm and one case of lymphangiosarcoma of the hand following a fifty-year existing chronic lymphedema after burn injury in childhood are reported. Clinical description and illustrations of all three cases are given, including list of references. Therapeutic procedures are described.

[Multiple symmetrical lipomatosis. A retrospective study of 14 cases and review of the literature].

"Multiple symmetric lipomatosis" is an accumulation of fatty tissue in upper areas of the body mainly effecting middle aged men. It is a rare benign disease connected with differing disorders. We report 14 male patients suffering from "multiple symmetric lipomatosis" and focus on the etiology and on associated disorders. Differential diagnosis, pathohistological considerations, clinical behaviour, and operative treatment are discussed. A review of the literature is presented.

[Complications and failures in free-flap transplantations].

Complications and failures in nine cases of free flap transfer are reported. With increasing experience the number of failures declines. In six of seven cases with total flap necrosis technique and tactics in microsurgical procedure were responsible for the disaster. The failure in the other case was due to thrombosis of an axillary vein and thoracodorsal vein leading to necrosis of a latissimus dorsi flap. A dorsalis pedis flap was saved by arterial inflow through a flap vein. The fact that not one flap could be saved by revision is accounted for by the circumstance that the monitoring looks at capillary filling alone. It is possible that the Laser-Doppler-flowmetry is a useful and practical method for objective monitoring.

[Stonefish stab injury of the hand].

The authors report a sting injury by a stonefish in the hand. First aid and surgical treatment are discussed. The biology of the stonefish is reviewed.

[Skin-soft tissue replacement on the lower leg--therapy and prophylaxis of osteomyelitis].

The adequate and early replacement of soft tissue in the lower leg is equivalent to the management of bone and prevention of osteomyelitis. The acute or delayed primary management of soft tissue loss with early flap coverage prevents infections. In the treatment of osteomyelitis, flap coverage provides the necessary blood supply for healing. In the first stage we remove bone until bleeding indicates viable tissue, gentamycin-PMMA-chains are introduced before flap coverage. In the second stage, if necessary, we substitute the gentamycin-PMMA-chains with bone graft.

[Replantation of the extremities with special reference to flanking measures].

Replantation of extremities is presented and defined. A historical survey is offered. In addition to first aid, transport, preliminary procedures, and surgical treatment, special attention is given to conservation measures with consideration of experimental and pathophysiological principles. Experimental work performed by the author on rabbits is presented. The controversial value of perfusion is viewed in the light of clinical and experimental information; it is seen as the only conceivable means of long-term conservation. The risk of postischemic syndrome is discussed in relation to conservation. Hyperbaric oxygenation, which has as yet only been applied by a few authors in replantation of extremities, is also discussed. Results obtained are documented with the author's own clinical material.

[Experiences with dorsalis pedis flaps].

Experiences with 8 dorsalis pedis flaps are reported. The advantages are: Thinness and pliability of the flap, long pedicle and wide calibre of vessels, low rate of complications. The flap is very suitable for hand and face reconstruction.

[Hand injury from a spindle conveyor].

A case report is presented of a left hand injury in a four-year-old girl with a bone defect of the mid hand caused by a spindle conveyor. The technical problems both of age and bone reconstruction where the epiphysis is missing, are discussed.

[Importance of the proliferating trichilemmal cyst].

The biological behavior of proliferating trichilemmal cysts is commented on, based on two cases. Two female patients, 71 and 70 years of age, developed solitary monstrous tumours of the scalp in the course of repeatedly recurring trichilemmal cysts. The biological behaviour of both was marked by infiltration of the galea aponeurotica, and in the latter case, invasion of the calvaria and the venous sinuses of the brain with a lethal outcome. Histologically, the differentiation of the cyst walls ranged from pseudoepitheliomatous hyperplasia to areas largely lacking differentiation, indicating a trichilemmal carcinoma. Therefore, a more cautious evaluation of proliferating trichilemmal cysts, commonly underestimated as pseudocarcinomatous, is indicated. The second case in particular shows that on occasion an invasive carcinoma can arise in a trichilemmal cyst. Radical surgery can not only prevent recurrence with loss of differentiation, but also enable a better histological evaluation of the deep-lying tumour tissue.
