One-year prognosis of primary ventricular fibrillation complicating acute myocardial infarction. The GISSI (Gruppo Italiano per lo Studio della Streptochinasi nell'Infarto miocardico) investigators.

The 1-year prognosis of 293 patients discharged alive from the hospital after acute myocardial infarction (AMI), who experienced primary ventricular fibrillation (VF) in the acute phase, was compared with that of a reference group of 6,337 patients identified from the same population included in the Gruppo Italiano per lo Studio della Streptochinasi nell'Infarto miocardico (GISSI) trial. There was no difference in the 6- and 12-month mortality between the patients with primary VF and the reference group (3.7 vs 2.7% and 4.1 vs 4.2%, respectively). Survival of the 2 groups was also similar when patients were stratified according to infarct site (anterior and posterior), and whether or not they received treatment with streptokinase during AMI. Thus, long-term mortality of patients discharged alive after AMI complicated by primary VF is low and is not influenced by previous fibrinolytic therapy or by infarct site. The excess mortality of patients with primary VF is confined to the hospital phase, after which survivors represent a low-risk subgroup.

[Evaluation of beta cell function in non-insulin-dependent diabetes mellitus (NIDDM) and its correlations with clinico-metabolic parameters].

Authors studied plasma C-peptide in basal period and after stimulation with one mg of glucagon intravenous in 73 patients with NIDDM. The results have confirmed initial clinical diagnosis of NIDDM. Significative correlations were obtained between basal C-peptide and C-peptide response to glucagon (r = 0.861; p less than 0.01); delta C-peptide (difference between C-peptide basal and after response to glucagon) (r = 0.361; p less than 0.01); body mass index (r = 0.423; p less than 0.01). The significant correlations between basal C-peptide, C-peptide response to glucagon and delta C-peptide show that basal C-peptide measurement is a sufficient test for beta-cell secretory capacity, even though do not give indicative signs for therapy choice of NIDDM for its multifactorial pathogenesis.

[Duodenal ulcer, study of gastric secretion and rational treatment with famotidine in different doses].

The authors report their experience with 50 patients affected by duodenal ulcer, 80% of them with a chronic relapsing ulcer. They all underwent endoscopy and study of gastric secretion (BAO and PAO) before beginning therapy with famotidine (40 mg nocturnal) and during this treatment. In 43 patients the secretory values of BAO and PAO resulted above the ulcerogenic limit during the treatment. In 2 cases the secretory values did not decrease under the ulcerogenic limit even during the therapy with 40 mg of famotidine. These 2 cases had been affected by relapsing duodenal ulcer, non responding to H2 antagonists and nocturnal doses of 60 and 80 mg famotidine till the secretory values decreased below the ulcerogenic limit. In such a way we obtained ulcer healing in 4 weeks. The therapy was prolonged for two years with the same dosage at the beginning and later with half doses without any relapse and secondary effects. We emphasize the utility of secretory studies in peptic ulcer and suggest that non-responding duodenal ulcer can depend on very high acid gastric secretion treated by too low doses of H2 antagonists.

Double circuit esophagojejunoduodenal plasty in reconstruction of the alimentary tract after total gastrectomy.

The results of a new reconstructive technique of the digestive tract after total gastrectomy with a double circuit EDDP were reviewed. Since 1976, 141 patients with carcinoma of the stomach have been surgically treated at our hospital. Of the 68 patients who had undergone total gastrectomy, 17 had a double circuit EDDP performed. In this short series, no postoperative complications occurred. The results of clinical, laboratory and instrumental assessments in the eight patients available for follow-up study were quite satisfactory. Weight loss was moderate, if not absent. All patients assessed were content with their quality of life and were able to resume their previous work activities again.

Magnesium in tumoral tissues, in the muscle and serum of subjects suffering from neoplasia.

Mg2+ content is significantly increased in malignant neoplastic mammary tissue compared with normal mammary tissue and benign neoplastic tissue of the breast. Significant variations of the ion were not found in the skeletal muscle tissue (rectus abdominis and pectoral muscle) of subjects suffering from malignant neoplasia with diffused metastasis. It seems that the variation of Mg2+ content in malignant neoplastic mammary tissue is a local factor linked with the biological anomalies of the neoplastic cell.

Reconstructive breast surgery following mastectomy and adjunctive radiation therapy.

With modern radiation, surgical therapy, and surgical reconstructive techniques, it is now possible to give adequate treatment for breast cancer and to reconstruct the breast as well, using a silicone implant prosthesis. Twelve mastectomies and reconstructions were performed on 8 patients. Seven of these had pre- or postoperative irradiation to one breast region, and 1 patient received irradiation to both breast areas. The clinical staging of patients with combined irradiation and surgery was Stage I--3 cases; Stage II--4 cases; and Stage III--1 case. Primary surgical procedures consisted of modified radical mastectomy for ten breast lesions and simple mastectomy for two others. Megavoltage radiotherapy was administered with a CO-60 Unit. Pre-operative irradiation of 4000 rads to the breast, chest wall, and regional lymphatics was given to 3 patients. Postoperative radiation therapy of 5000 rads to the chest wall and 4400-5000 rads to the regional lymphatics was given to 5 other patients, one of whom received bilateral irradiation. Adjunctive chemotherapy was employed in 5 patients with interruption during the period of implant surgery. Reconstructive surgery was performed 5 months to 14 months after irradiation in 7 patients and 51/2 years later in one patient. The procedure was a single stage operation with placement of a silicone prosthesis under the pectoralis muscle. No significant morbidity was encountered from radiotherapy, chemotherapy, or from the surgical procedures. Healing of surgical wounds after implantation was uncomplicated. Cosmetic results have been good. All patients are alive and well, 8 months to 6 1/2 years postirradiation and three months to three years post-breast reconstruction. Results to date have been most gratifying.

The reporting habits of staff in a psychiatric hospital.

This study examined the frequency, length, and content of staff's discussions and written reports about 90 hospitalized psychiatric patients. The reporting habits of staff on three different units of one hospital were compared. On the units where nursing staff were primary case managers, discussion was less likely to include mention of treatment goals or plans than where psychiatrists were case managers, or where nursing staff were case managers and patients were encouraged to attend case reviews. There was no difference between the units on recording habits, but nursing staff were consistently more dependable than psychiatrist at record-keeping.
