Norwegian scabies in a patient with AIDS: report of a case.

A severe form of crusted scabies is described in a patient with acquired immunodeficiency syndrome. He was treated with 1 percent lindane lotion and 5 percent sulfur ointment with 2 percent salicylic acid, which resulted in the clearing of the skin lesions.

Lichen planus with predominant plasma cell infiltrate.

We herein report the case of a patient with lichenoid plaques on the lower extremities and anonychia of all toenails. Histologically, the eruption showed typical features of lichen planus, with the exceptional finding of a lichenoid infiltrate composed mostly of plasma cells. Only two other similar cases have been reported. The explanation for the numerous plasma cells remains unknown although such cases may represent a new histologic variant.

Multiple epidermoid cysts in a renal transplant recipient taking cyclosporine A.

A forty-two-year-old male recipient of a cadaver renal transplant showed multiple nodulocystic lesions on his back and lateral aspects of his neck four months after beginning immunosuppressive therapy with cyclosporine A and prednisone. Results of histopathologic studies revealed that they were epidermoid infundibular cysts. We note the relationship between this clinical appearance and cyclosporine A, as well as this agent's cutaneous side effects.

Epidermotropic metastases from internal carcinomas.

Four cases of epidermotropically metastatic carcinoma are reported. One of them originated from a breast adenocarcinoma; in two other cases, the primary tumor was located in the large intestine; and, in the fourth case, the primary malignancy was a laryngeal squamous cell carcinoma. Clinically, the cutaneous metastases were nodular lesions and histologically, in addition to the involvement of the dermis by malignant cells, in three cases there was intraepidermal involvement by glandular structures. In the fourth case, the cutaneous metastatic tumor cells showed striking "folliculotropism." We review the literature of epidermotropically metastatic carcinoma and discuss its differential diagnosis.

Hereditary epidermolytic palmo-plantar keratoderma (Vörner type)--report of a family and review of the literature.

We describe a patient in whose family 13 of 26 members have a palmo-plantar keratoderma. The histopathological findings in the proband, his brother and father were those of epidermolytic hyperkeratosis. As in other families reported, this disorder was found to be inherited as an autosomal-dominant trait. A review of the literature on the clinical, genetic and associated features of this genodermatosis is presented. We also review the cutaneous conditions that may exhibit the histopathological pattern of epidermolytic hyperkeratosis.

Pigmented spindle cell naevus.

We report 22 cases of pigmented spindle cell naevus (PSCN). The usual appearance of these naevi is that of a heavily pigmented papule found mostly on the legs of young patients. Histologically, PSCN was characterized by symmetrical proliferation of spindle-shaped pigmented melanocytes grouped in large junctional nests. Pagetoid spread of single cells in the overlying epidermis was frequently found. In our opinion, PSCN is a distinctive benign acquired melanocytic naevus that in the past has been frequently misdiagnosed as atypical Spitz naevi or malignant melanoma.

Sclerosing-sweat-duct (microcystic adnexal) carcinoma--a tumor from a single eccrine origin.

A patient with a sclerosing-sweat-duct carcinoma of the upper lip is reported. Histologically the tumour showed solid islands and strands of squamous cells and sparse small ductal structures, some of them containing central cores of dense eosinophilic keratin. All this was embedded in a sclerotic stroma. These features, in addition to positive immunoreactivity for carcinoembryonic antigen (CEA) in the lumina of small ducts, and the presence of S-100 protein-positive cells in some cords and ducts, are consistent with the notion that this tumour exhibits differentiation toward eccrine sweat structures.

[Candida folliculitis in heroin addicts. Histological study].

Results of histopathologic study of five parenteral heroin users with candida folliculitis are exposed. Candida was found in four, pseudohyphae were observed within the hair, on the keratin of the infundibulum, within the abscess surrounding the hair follicle and horny layer. Blastospores were observed into one arteriole of the dermis.
