Modulation of IgG and complement receptor expression of phagocytes in kidney cancer patients during treatment with interferon-alpha.

BACKGROUND: The mode of action of interferon involves both direct cytotoxic and antiproliferative effects on the tumour cell and indirect effects that facilitate immune detection by the host. Among the immunological effects of interferon-alpha is the activation of monocytes. As opsonin receptors are crucial in the function of phagocytes, e.g. monocytes and neutrophils, their modulation by interferon-alpha (INF-alpha) merit to be further clarified. We hypothesised that the role of phagocytes in defence against cancer is reflected in the expression of opsonin receptors for IgG and complement, which further could be modified by INF-alpha. PATIENTS AND METHODS: The expression of the receptors for IgG and complement was studied in neutrophils and monocytes from blood samples of 18 kidney cancer patients treated with INF-alpha and from 39 healthy individuals. Blood samples were collected prior/to and during treatment with INF-alpha, 4.5 to 13.5 MU t.d.w., subcutaneously. After lysing the red blood cells, the samples were incubated with fluorochrome conjugated monoclonal antibodies specific for IgG (Fc gammaRI, -RII and -RIII) and complement (CR1, CR3) receptors and then analysed in flow cytometry. The results were given as the mean log fluorescence intensity (a measure of receptor number) and as the proportion of receptor-positive cells. In the in vitro experiments, the direct effect of interferon-alpha on the receptors of neutrophils and monocytes was studied. RESULTS: In patients before any treatment, the expression of CR3 and Fc gammaRI receptors in neutrophils and all receptors except Fc gammaRIII in monocytes was significantly raised when compared to the controls. Treatment with INF-alpha, induced statistically significant; transient changes in CR1-receptor expression in neutrophils and Fc gammaRI expression in monocytes. Incubation of blood cells with INF-alpha in vitro confirmed the induction of CR1 receptors in neutrophils. CONCLUSION: Changes in receptor expression reflect the inflammatory activation of phagocytes in metastatic kidney cancer. The pattern of receptor expression differs from that observed in infectious diseases. Interferon-alpha both in vivo and in vitro modulates the expression of phagocytic receptors.

Early effects of 5-fluorouracil, epirubicin and cyclophosphamide therapy on common laboratory tests.

BACKGROUND: Combination chemotherapy with 5-fluorouracil, epirubicin and cyclophosphamide (FEC) is now used for adjuvant therapy of breast cancer. The effects of FEC on common laboratory tests are important when interpreting test results during the treatment. PATIENTS AND METHODS: Common hormonal tests (e.g., serum gonadotrophins, testosterone, prolactin, dehydroepiandrosterone sulphate, cortisol, parathyroid hormone (PTH), thyroid function tests), haematological blood counts and biochemical tests including specific proteins and lipids of seven women with metastatic breast cancer were assessed at baseline and before the 3rd and the 5th cycles: RESULTS: Statistically significant increases were noted in serum PTH, free triiodothyronine, sodium, prealbumin and cholesterol, but decreases in white blood cell count, alpha-1-antitrypsin, as well as immunoglobulins A and M. CONCLUSION: Except for serum PTH, cholesterol and WBC count, the changes were small. The diagnoses of diseases based on serum PTH and cholesterol test results may be influenced by FEC treatment.

Expression of C-KIT and HER-2 tyrosine kinase receptors in poor-prognosis breast cancer.

BACKGROUND: This study investigated the expression of C-KIT and HER-2 receptors and their correlation with histopathological markers of cell proliferation, differentiation and apoptosis in poor-prognosis breast cancer. PATIENTS AND METHODS: Formalin-embedded histopathological samples from forty patients with progressive metastatic breast cancer were reanalyzed to determine HER-2 expression by immunohistochemistry (ICH) and chromogenic in situ hybridization (CISH). C-KIT receptor expression, p53, Bcl-2 and Ki-67 were determined by immunohistochemistry. RESULTS: The mean p53 score was 18.03 (SD 30.69), that of Bcl-2 was 38.13 (39.25) and Ki-67 was 5.8 (SD 9.23), respectively. HER-2 expression was positive in 35% of patients by ICH and in 25% by CISH. C-KIT receptor staining was positive in 82% of the patients. A significant association was observed between HER-2-positive score in ICH and poorly-differentiated histology (p=0.03), negative Er/Pr status (p=0.04) and Bcl-2 expression (p=0.003). With CISH-determined HER-2, the corresponding p values were 0.07, 0.053, and 0.002, respectively. No correlation was found between HER-2 or C-KIT expression (p=0.456). CONCLUSION: CISH and ICH determination of HER-2 correlate similarly to hormone receptor status and Bcl-2 expression in breast cancer. C-KIT expression was commonly present and did not correlate to other prognosticators in poor-prognosis breast cancer.

Immediate effects of docetaxel alone or in combination with epirubicin on cardiac function in advanced breast cancer.

The immediate effects on cardiac function of 3-weekly docetaxel and combined docetaxel-epirubicin were evaluated during treatment of metastatic breast cancer using assessment of heart rate variability (HVR) and left ventricular ejection fraction (LVEF) measured by echocardiography. Twenty-four breast cancer patients were treated with docetaxel alone (starting dose 100 mg/m2) and 34 with a combination of docetaxel and epirubicin (starting dose for both drugs 75 mg/m2) administered 3-weekly. Single docetaxel caused no significant changes in HVR or cardiac function, whereas during combined treatment statistically significant changes were observed in mean RR intervals and in the number of supraventricular extrasystoles. Clinically the observed changes were insignificant. In conclusion, in 3-weekly administration the combined use of docetaxel and epirubicin was more likely than single docetaxel to cause changes in cardiac function.

Serum TRACP 5b and ICTP as markers of bone metastases in breast cancer.

BACKGROUND: The purpose of this cross-sectional study was to evaluate the value of serum tartrate-resistant acid phosphatase 5b (TRACP 5b) and carboxyterminal telopeptide of type I collagen (ICTP) separately and in combination as markers of bone metastases compared to total alkaline phosphatase (tALP) in breast cancer. MATERIALS AND METHODS: Two groups of patients were studied, one with verfied bone metastases (N=46) and one without bone metastases (N=141). Bone marker levels were correlated with the presence or absence of bone metastases. RESULTS: Serum TRACP 5b concentrations exhibited the largest area under the receiver-operating characteristics (ROC) curve (AUC=0.845), followed by ICTP (0.818) and tALP (0.814) when all patients were included in the analysis. With the combination of TRACP 5b and ICTP, the AUC increased to 0.881. In multivariate regression analysis, all three markers were significant predictors of bone metastases. CONCLUSION: Serum TRACP 5b, ICTP and tALP exhibited equal performances in the detection of bone metastases. The combination of TRACP with ICTP did not significantly improve the detection of bone metastases over tALP.
