[The malignant neoplasm in pediatrics].

A summary of diagnostic and therapeutic progress in pediatric oncology is presented. The results are impressive, in that at least 1 child in 3 may be successfully treated. A truly effective treatment for the individual child requires the close cooperation of specialists on a national, regional and local scale. Pediatricians and general practitioners who suspect neoplasm are required, on the one hand, to ensure speedy and adequate clarification of the case, and, on the other, to cooperate in the treatment and care of children with cancer. Briefing and instruction of parents is of great practical importance.

Relative underweight in cystic fibrosis and its prognostic value.

On the basis of observations in 117 children with cystic fibrosis, seen from January 1956 to June 1976, it is demonstrated that the relative underweight (weight loss corrected for height) is most pronounced in children with predominantly pulmonary sypmtoms. The degree of underweight closely correlates inversely with survival. Because of its prognostic value, it is recommended that this clinical parameter be included in the checkups which are periodically carried out on children suffering from cystic fibrosis.

The detection of porphyria in photosensitive patients.

The biochemical diversity of the various porphyris often leads to incomplete investigation of photosensitive patients and porphyria may be excluded wrongly on the basis of normal urinary porphyrins alone. Establishing a biochemical referral centre for photosensitive patients suspected of having porphyria led to the diagnosis of 5 cases of porphyria cutanea tarda (PCT) and 2 cases of erythropoietic protoporphyria (EPP) among 34 patients referred by dermatologists over a period of 12 months. Iron overload was conformed in 3 the PCT patients by plasma ferritin assay. Studies on the available families of the two EPP patients revealed elevated red cell protoporphyrin levels in several clinically asymptomatic relatives.

Critical appraisal of haemofiltration and ultra-filtration. The development of ultra-short dialysis: preliminary results.

The clinical experience obtained with 2 hours every other day recirculation dialysis, using 20-40 liters of dialysate, without sorbents, and standard cuprophane dialyzers of 1.0-1.5 sq.mt. is reported. So far, over 350 treatments in 8 patients have been performed. After 2 hours of treatment the removal of urea, creatinine, phosphate and uric acid, is similar to that obtained by 4-6 hours of haemofiltration. The alkalinazation of the patient through direct venous infusion of bicarbonate, makes predialysis acid-base significantly better than in standard haemodialysis and haemofiltration. Asymptomatic correction of severe fluid overload is easily obtained like in isolated ultrafiltration. The role of osmolality and vasopressors are discussed. A dry weight below the value obtained by previous dialysis treatment is achieved, and volume dependent hypertensions as in haemofiltration are corrected after 2-8 weeks. As an additional advantage, this method offers a highly semplified technical approach and a further reduction of the dialysis time.

Liver involvement in cystic fibrosis. A report of 9 cases.

9 out of 204 unselected cystic fibrosis (CF) patients seen at the Department of Pediatrics, University of Berne, Switzerland, over the last 20 years had clinically overt liver disease. In 7 patients liver cirrhosis was demonstrated (3.4%). Focal biliary cirrhosis was the pertinent finding in 3 cases, whereas another 3 showed unspecific nodular cirrhosis. One infant presented with prolonged obstructive jaundice due to partial extrahepatic atresia and delayed passing of meconium. Two other patients had steatosis. A review of the literature dealing with clinical and laboratory findings in CF patients with liver disease is given.

[Australia antigen: biological and technical considerations (author's transl)].

The A. examines the constitution of Australia antigen by the point of view of the relevant groups, and exposes his own classification of these structures. After having dealed with some epidemiological data and with some diagnostic and physiopathologic aspects of the serum hepatitis, the A. describes the cautions that have been taken in IGEA Laboratory against the infection danger. The final part of this work is dedicated to a circumstantial report of Austria II radioimmunologic method with discussion and criticism about some technical data risen from a recent experiment carried out on about 3,000 serums. In particular, RIA method has been submitted to a series of quality controls to evaluate its sensitivity and specificity.

Epoxide-diol metabolic pathway of cytenamide in the rat.

Cytenamide administered intraperitoneally to rats is biotransformed to cytenamide-10,11-epoxide and 10,11-dihydro-10,11-dihydroxycytenamide. These metabolites were separated by chromatographic methods and their structures elucidated by mass spectrometry. The structure of the epoxide was confirmed by direct comparison with an authentic sample. The formation of a chemical artifact, cytenamide-syn-11-hydroxylactone, in the urine was observed. The formation of these metabolites in vitro was demonstrated by incubation of cytenamide with rat liver microsomes.

[Morbidity and mortality in cystic fibrosis. Results of 20 years of experience].

In this study the clinical data and treatment concept in 197 patients with cystic fibrosis, seen during the past 20 years at the University Children's Hospital of Berne (Switzerland), are presented. The results showed that during recent years the median age of survival, the mean age at death and the cumulative survival rates have improved. Furthermore, this study demonstrates that by forming groups of patients with similar age at diagnosis the cumulative survival rate declined in the group with early diagnosis much more markedly than in the group of patients with later diagnosis. Further evaluation of these data revealed that most patients of the former group had a more severe form of cystic fibrosis than the latter group. Since there are different patterns of severity in this disease, it will be necessary to develop staging for the purpose of statistical comparisons.

A to O bone marrow transplantation in severe aplastic anaemia: dynamics of blood group conversion and demonstration of early dyserythropoiesis in the engrafted marrow.

A to O bone marrow transplantation was performed in a 25-year-old male affected with severe aplastic anaemia, the donor being an HLA compatible brother. Three plasma exchanges had to be performed with an Aminco separator to remove the original and recurring anti-A isohaemagglutinins. The dynamics of O to A blood group conversion were followed by means of differential agglutination. An early wave of marked dyserythropoiesis was observed in the engrafted marrow. Mild to moderate GvHD was treated successfully with MTX, bolus high dosage 6-methylprednisolone and, at relapse, with intravenous ALG.

Peptichemio: A new oncolytic drug in combination with vincristine and prednisolone in the treatment of non-Hodgkin lymphomas.

Peptichemio, a new oncolytic drug with alkylating and antimetabolic properties was employed in combination with vincristine and 6-methylprednisolone (PVP) for the treatment of diffuse non-Hodgkin's lymphomas (NHL), stages III and IV. 52 of 66 patients who entered the PVP protocol were considered evaluable: the overall remission rate in diffuse lymphocytic poorly differentiated lymphomas (DLPD) was 56.6 and 55% in diffuse histiocytic lymphomas (DHL). Median duration of complete remission was 16 months in DLPD and 13 months in DHL. Dramatic results were obtained in "oncologic emergencies," such as mediastinal involvement and spinal cord compression. Bone marrow depression from PVP was moderate and never caused discontinuation of therapy. According to these results the PVP protocol would appear susceptible of offering a major contribution to the chemotherapy of diffuse NHL.
