Renal function at hospital admission as a prognostic factor in adult hemolytic uremic syndrome. The Italian Registry of Haemolytic Uremic Syndrome.

The clinical records of adult patients with a diagnosis of hemolytic uremic syndrome were retrospectively reviewed with the aim of evaluating the long-term outcome of renal function. The setting is the Italian Registry of Haemolytic Uraemic Syndrome, with which 13 Nephrology Centers have participated. Clinical and laboratory data of 43 patients with hemolytic uremic syndrome were evaluated. The mean age at onset was 34.3 +/- 18.3 yr. Men and women were equally affected. No seasonal trend in presentation was observed. In 20 patients, hemolytic uremic syndrome was primitive, whereas in 23, it was associated with another disease (cancer, preeclampsia, malignant hypertension, vasculitides). Gastrointestinal symptoms were the most frequently observed prodromes. Thirty (70%) patients required dialysis during the acute phase of the disease. Six patients died during the acute phase of the disease, and one died later after discharge (overall mortality, 16%). After 1 yr of follow-up, 11 (26%) patients had recovered a normal renal function, 14 (33%) had hypertension and/or renal insufficiency, and 11 (26%) were on regular dialysis. When prognostic factors of survival and recovery of renal function were considered, it was found that older age was associated with higher mortality in the acute phase, whereas severe renal involvement at the onset of the disease (as expressed by elevated serum creatinine) was associated with a long-term unfavorable prognosis.

Effects of sodium meclofenamate on postoperative pain following periodontal surgery.

The analgesic activity of single doses of meclofenamate (100 mg) was compared to aspirin (500 mg) and to placebo on 99 outpatients with moderate to severe pain following periodontal surgery under double-blind conditions. Pain intensity differences scores (PID) at the first, second, and third hour after the ingestion of the first capsule of each medication were used to determine the analgesic efficacy of the studied drugs. Kruskal Wallis test followed by the Mann-Whitney rank sum test were used in the statistical analysis of results. Meclofenamate was statistically superior to placebo and superior to aspirin in the second hour of evaluation, while aspirin was not superior to placebo during the 3-hour period of pain evaluation. It is concluded that meclofenamate is a non-steroidal antiinflammatory drug with interesting analgesic properties which can be used as directed as an alternative to aspirin or acetaminophen for the control of postoperative pain following periodontal surgery.

Combined mustard and Rastelli operations. An alternative approach for repair of associated anomalies in congenitally corrected transposition in situs inversus [I,D,D].

We report on two patients with congenitally corrected transposition of the great arteries in situs inversus who underwent successful anatomic repair of associated cardiac anomalies--ventricular septal defect and pulmonary outflow tract obstruction. Surgical intervention was influenced by the recently introduced technique of Ilbawi and colleagues in cases of congenitally corrected transposition in situs solitus. Principles of the correction are as follows: (1) patch redirection of venous flows at the atrial level through an incision in the left-sided right atrium; (2) patch closure of the ventricular septal defect through a right ventriculotomy, baffling the left ventricle to the aorta; and (3) valved conduit interposition between the right ventricle and the pulmonary artery. If the ventricular septal defect is restrictive, it can be safely enlarged by extensive resection of the anterosuperior border, because of the posteroinferior location of the bundle of His in this anomaly. With this technique, the left ventricle and the native mitral and aortic valves are recruited for systemic work, with intrinsic long-term advantages. It is, however, a complex procedure involving substantial use of prosthetic material. More experience is necessary to establish whether this approach is superior to currently available physiologic repairs.

[Why a gynecological consultation service in a hematology day hospital. Our experience].

The paper described the role of a gynaecological advisory service in an hematological day hospital. The main gynaecological problems which arose during anti-cancer and immunosuppressive therapy were classified according to patients' symptoms. The paper stresses the need for close collaboration between hematologists and gynaecologists and underlines the psychological importance of this aspect from the patient's point of view.

[Effectiveness of and tolerability to oral desmopressin in the treatment of central diabetes insipidus].

Intranasal desmopressin represents the treatment of choice in Central Diabetes Insipidus. Nevertheless, this route of administration bears some practical disadvantage, linked to either difficult delivering technique, or the status of nasal mucose. The antidiuretic effectiveness of oral desmopressin has been recently demonstrated, both in experimental animals and in man. In our study we compared oral vs. intranasal desmopressin efficacy in 13 patients affected by Central Diabetes Insipidus. The results show that the peroral administration of Desmopressin at a mean dose of 500-600 micrograms/die determines an antidiuretic effect comparable to that of intranasal route, without affecting body weight, arterial pressure and chemical analysis. Side effects, generally limited to the first week of treatment, were described (nausea, vomiting, headache, dizziness [corrected], bitter taste, epygastralgia, asthenia, epystassis), inducing 4/13 patients to withdrawal the trial.

DiGeorge anomaly associated with 10p deletion.

DiGeorge anomaly (DGA) represents a heterogeneous entity, which is often sporadic, although familial cases and the association with monosomy 22q11 have been reported. Recently, a few patients with 10p deletion syndrome and immunological and other laboratory findings similar to DGA have been described. We report on an additional case of partial DGA associated with 10p deletion.

Effect of a new peptidyl-hypoxanthine derivative on natural killer cells and antitumor activity.

A new immunomodifier, [omega-(hypoxanthin-9-yl) pentoxy-carbonyl-leucyl-methionine] (RM06), was synthesized and its effect was evaluated on the activity of Natural Killer (NK) cells. Results indicate that RM06 is able to boost the NK activity of normal mice as well as to augment the regeneration of NK activity of lethally irradiated mice transplanted with syngeneic bone marrow (BM). This later effect also correlated with a significant increase in anti-tumor activity as evaluated by the resistance to metastasis in mice injected with syngeneic melanoma cells. These data indicate that RM06 is able to modulate the NK cell activity as well as the antitumor resistance.

Retention of amphotericin-B therapeutic efficacy at half doses by synergistic activation of phagocytes.

Amphotericin B (AMB) is a mainstay in the treatment of serious systemic fungal infections, such as those occurring prevalently in immuno-compromised patients treated with immunosuppressive agents or affected by Acquired Immunodeficiency Syndrome (AIDS). However AMB is an extremely toxic agent whose therapeutical utilization is often accompanied by acute side effects and chronic impairment of renal function. It is here reported that the preactivation of polymorphonucleated cells (PMN) in vivo, by a new immunomodulatory agent (PCF 39:N alpha-5[1,6,dihydro-(6-oxo-9 purinyl) pentoxycarbonyl]-L-Arginine) allows marked reduction of the AMB doses with full retention of therapeutic efficacy. This was observed in an experimental fungal infection induced in mice by intravenous inoculation of Candida albicans.

Noncoordinated expression of S6, S11, and S14 ribosomal protein genes in leukemic blast cells.

The steady state levels of mRNAs codying for the ribosomal proteins S6, S11, and S14 have been evaluated in quiescent and proliferating human fibroblasts and in resting and proliferating human peripheral blood mononuclear cells. It was found that the amounts of ribosomal protein mRNA are very similar and are not increased by serum or mitogen stimulation. The constitutive expression of these genes appears to be coordinately regulated and it is not modified after protein synthesis inhibition by cycloheximide. The ribosomal protein mRNA was also assayed in 15 different populations of human leukemic blast cells. In these populations the abundance of each ribosomal protein mRNA is remarkably different from the other. The results of our present experiments indicate that the expression of the three ribosomal protein genes undergoes independent noncoordinated changes in the large majority of the leukemic populations studied.

Inhibition of human lymphocyte ferrochelatase activity by hemin.

Ferrochelatase activity in human lymphocytes was found to be 50% inhibited by 10.5 microM hemin under maximal velocity conditions. The inhibition was not prevented by dithiothreitol or glutathione, suggesting that the hemin was not interacting with the sulphydryl groups of ferrochelatase. Human serum albumin, but not bovine serum albumin was able to prevent the inhibition consistent with the known formation of the tightly bound methemalbumin complex with human albumin. Kinetic studies performed under initial velocity conditions with hemin concentrations ranging from 2 to 8 microM revealed the inhibition to be non-competitive with respect to the metal substrate (zinc) and competitive with respect to the porphyrin substrate (mesoporphyrin). The kinetic analysis indicated that hemin binds to both the enzyme and enzyme-metal complex at a site normally occupied by the porphyrin substrate, and a second molecule of hemin could bind to the enzyme-metal complex but with a much lower affinity than the first molecule. We conclude that the product inhibition of ferrochelatase by hemin should be considered as a possible site of regulation of heme biosynthesis.

Metal inhibition of ferrochelatase activity in human lymphocytes.

The activity of the terminal enzyme of haem biosynthesis, ferrochelatase (EC 4.99.1.1) was measured in sonicates of human lymphocytes. We used a sensitive method with zinc and mesoporphyrin as substrates and quantification of the product zinc-mesoporphyrin by HPLC. A variety of metal ions and organometal compounds were examined as possible inhibitors of ferrochelatase activity. Inhibition was observed with copper and mercury (but not with lead) and with tributyltin and methylmercury. The kinetics of ferrochelatase inhibition were examined for each of the four inhibitors identified.

Analgesic efficacy of flurbiprofen as compared to acetaminophen and placebo after periodontal surgery.

The purpose of the present investigation was to compare the analgesic efficacy of flurbiprofen, a nonsteroidal anti-inflammatory drug, which is a phenyl propionic acid derivative, with acetaminophen and placebo in 63 adult outpatients with moderate to severe dental pain following periodontal surgery. After surgery was completed under local anesthesia, the patients received under double-blind conditions an envelope containing four tablets of either flurbiprofen 100 mg, acetaminophen 500 mg, or placebo and they were instructed to take one tablet every 6 hours when postoperative pain reached moderate to severe intensity. To determine analgesic efficacy and patients recorded pain intensity on a scale of 0 to 3. Patients were allowed to remedicate after 1 hour if pain was not reduced. Flurbiprofen was shown to possess an adequate analgesic effect superior to either placebo (P less than 0.005) or acetaminophen (P less than 0.01) in the parameters studied. Our results seem to further support earlier data obtained with the drug in dental patients with postoperative pain after the surgical removal of impacted third molars; therefore, it is concluded that flurbiprofen used as directed is a new alternative for the proper treatment of pain following periodontal surgery.

[Cutaneous angiogenesis associated with vulvar epidermoid carcinoma].

A case of cutaneous angiogenesis, probably induced by tumour angiogenic factors (TAF), is described. The patient, a 39-year-old female, affected by a vulvar squamous cell carcinoma, developed some erythematoangiomatous lesions on the lower abdominal quadrants, contiguously to the tumour. Histopathologically, the lesions were characterized by vascular neoformation in the superficial dermis and a slight lymphohistiocytic lesions resolved after the polychemotherapy which preceded enlarged vulvectomy. This supports the hypothesis of a close correlation between neoangiogenesis and TAF.
