Cancer and occupation in women: identifying associations using routinely collected national data.

Some potentially important findings from an analysis of 119,227 cancers registered in women in England over the 7-year period 1981 to 1987 are discussed. Data are presented for four cancer sites with established occupational etiologies (bladder, pleura, lung, larynx), three occupational groups in which women predominate (teachers, nurses, barstaff), and the main female cancers of the reproductive system (breast, uterus, cervix, ovary). Excesses of bladder cancer in female rubber workers (proportional registration ratio [PRR] 350, 95% confidence interval [CI] 141-723), pleural cancer in female carpenters (PRR 1596, 95% CI 329-4665) and lung cancer in female construction workers (PRR 269, 95% CI 154-437) and barstaff (PRR 138, 95%CI 124-156) are noted. It is concluded that analyses of routinely collected data on cancer and occupation in women have the potential to provide valuable pointers for further research.

[The molecular epidemiology of the rotavirus in Spanish children. The Rotavirus Study Group (GER)].

BACKGROUND: Rotavirus are the most common etiologic agent of acute gastroenteritis in childhood. The knowledge of the circulating antigenic types is important in development of future vacunes. METHODS: Faeces from children (age < 4 years) with acute gastroenteritis admitted in the two hospitals (Hospital Severo Ochoa-Madrid and Hospital General Vic-Barcelona) have been studied prospectively during one year (October-1996 to October-1997). The detection of rotavirus was performed by ELISA (IDEIA, Dako). All samples were G-serotyping by EIA-Mabs (Silenius Laboratories) and the indeterminate or non-serotypable samples were G-genotyping by RT-PCR. P genotypes were identified by RT-PCR. RESULTS: 322 (45%) patients with acute diarrhoea causing for rotavirus were confirmed, 242 coming from the Madrid metropolitan area and other 80 from the Barcelona area. The EIA-Mabs technique made it possible to identify the G serotypes in 287 cases (89%), corresponding 207 to G1 serotype, 70 to G4 serotype and 6 to G3 serotype. In 4 patients both G1 and G4 serotypes were detected. The EIA-Mabs could not determined the serotype in 35 (11%) patients, all of whom were confirmed by RT-PCR (12 belonged to serotype G1 and 23 to serotype G4). Analysis of P genotypes was carried out in 25 patients obtained from Madrid and 17 from Barcelona; all cases were classified in the P[8] genotype. CONCLUSIONS: The most frequent serotype in both hospitals was G1. The EIA-Mabs technique were showed a high sensitivity, however, the RT-PCR technique used were even more efficient, making it possible for us to identify all the non-serotypable EIA-Mabs cases. The temporal study of circulating serotypes/genotypes of rotavirus is necessary to evaluate the efficiency of vaccines.

Childhood cancer and paternal employment in agriculture: the role of pesticides.

Previous studies have suggested that the offspring of men potentially exposed to pesticides at work may be at increased risk of kidney cancer (Wilms' tumour), brain tumours, Ewing's bone sarcoma and acute leukaemia. This paper examines the association between potential occupational exposure of fathers to pesticides and offspring's death from cancer in a large national database. Records for 167703 childhood deaths occurring during 1959-63, 1970-78 and 1979-90 in England and Wales have been analysed. Among the offspring of men with potential occupational exposure to pesticides there were 5270 deaths, of which 449 were due to cancer. Associations were assessed using proportional mortality ratios (PMRs), with adjustment for age, year of death and paternal social class. Of the childhood cancers previously linked with potential paternal occupational exposure to pesticides, the only statistically significant excess was for kidney cancer (PMR=1.59, 95% CI=1.18-2.15, based on 42 deaths). Although these results offer some support for the suggestion that paternal occupational exposure to pesticides may be related to the subsequent development of kidney cancer in offspring, other explanations cannot be excluded. In the light of the findings presented here and elsewhere, further, more detailed, research into the nature of this relationship is warranted.

White-coat hypertension: a selection bias? Harvest Study Investigators. Hypertension and Ambulatory Recording Venetia Study.

BACKGROUND: Results of several studies have shown that subjects with white-coat hypertension (WCH) have more target-organ damage than do normotensive controls with similar ambulatory blood pressures. OBJECTIVE: To investigate whether this is due to a selection bias. SETTING: Seventeen hypertension clinics in northeast Italy. MAIN OUTCOME MEASURES: Echocardiographic data in relation to WCH status. PATIENTS AND METHODS: Mild hypertensive subjects from the HARVEST (n = 565) who underwent two ambulatory blood pressure monitorings 3 months apart and M-mode echocardiography, and 95 normotensive control subjects. RESULTS: From first ambulatory monitoring, 90 hypertensive subjects were classified as having WCH (mean daytime blood pressure < 130/80 mmHg). Their 24 h blood pressure was similar to that of the normotensive subjects, but their left ventricular mass index was greater. From second ambulatory monitoring, only 38 of the 90 subjects still had WCH, whereas 24 h blood pressure in the other 52 had risen beyond the limit of WCH. Left ventricular mass index (89.2 +/- 2.4 g/m2), wall thickness (18.1 +/- 0.3 mm), and relative wall thickness (0.359 +/- 0.006%) of the 38 subjects with WCH at both recordings were still greater than those of the normotensive subjects (82.4 +/- 1.5 g/m2, P = 0.02; 17.2 +/- 0.2 mm, P = 0.002; and 0.337 +/- 0.004%, P = 0.025) and similar to those of the 52 subjects who no longer had WCH (88.5 +/- 2.0 g/m2, 18.7 +/- 0.2 mm, and 0.375 +/- 0.005%, all NS). CONCLUSIONS: Owing to regression toward the mean, over 50% of the subjects with WCH could no longer be classified as such from repeated ambulatory monitoring, indicating that the current diagnosis of WCH is subject to selection bias. Cardiac remodeling was present also in the subjects confirmed to have WCH by repeated blood pressure recording, suggesting that the effect of WCH has an actual impact on target organs.

Incidence and time trends in Hodgkin's disease: from parts of the United Kingdom (1984-1993).

Over 3,000 cases of Hodgkin's Disease diagnosed between 1984-93 were used to examine incidence and time trends. These data are part of the Leukaemia Research Fund's specialist Data Collection Study, which is the only large, population-based data set of its type in Europe. The age specific incidence curves showed different patterns for nodular sclerosis contrasted with all other subtypes combined (non-nodular sclerosis). For nodular sclerosis, there was a female excess for young adults, while for non-nodular sclerosis a gradual rise in incidence with age in both sexes was observed. Incidence varied over time, showing a complex pattern with a decreasing trend in males in all Rye-subtypes and no significant change among females diagnosed with nodular sclerosis. These complex patterns of change are different from those seen in other countries. It is concluded that the results provide clear evidence of the heterogeneity of Hodgkin's disease between the sexes and between subtypes, which should be taken into account in future studies.

Validation of the A&D TM-2430 device for ambulatory blood pressure monitoring and evaluation of performance according to subjects' characteristics.

OBJECTIVE: To determine the accuracy of the TM-2430 blood pressure monitor, recently developed by the A&D company. DESIGN: Evaluation was performed using the 1990 and 1993 British Hypertension Society (BHS) protocols. Monitor's performance was assessed in relation to subjects' age, sex, level of blood pressure, and degree of adiposity. METHODS: Three TM-2430 recorders were assessed according to the various phases of the protocols. Simultaneous, same-arm readings were taken for the main validation test. Outcome was classified according to the criteria from the 1990 BHS recommendations, which are based on the cumulative percentage of readings differing from the mercury sphygmomanometer standard by 5, 10, and 15 mmHg or less, and using the criteria of the Association for the Advancement of Medical Instrumentation protocol, which considers a device accurate when the mean device-observer difference is within 5 mmHg and the related SD < 8 mmHg. RESULTS: During in-use assessment 2.3% of total measurements (N = 3744) were rejected automatically by the machine and another 5.5% werre discarded after visual inspection. The main validation test was performed with 98 subjects for a total of 595 blood pressure measurements. On the basis of the percentages of measurements differing from the mercury sphygmomanometer standard by </= 5, </= 10, and </= 15 mmHg, the TM-2430 device was graded A both for systolic blood pressure and for diastolic blood pressure. Differences between mean blood pressures as measured by device and observer were 2.2 +/- 3.9 mmHg for systolic blood pressure and 0.7 +/- 4.4 mmHg for diastolic blood pressure. The device's performance did not vary according to subjects' age, sex, and body mass, and was slightly better for subjects with high blood pressures and lean arms. CONCLUSION: These data show that the A&D TM-2430 monitor satisfies the recommended BHS and Association for Advancement of Medical Instrumentation accuracy levels for both systolic and diastolic blood pressures.

Age and sex distributions of hematological malignancies in the U.K.

This paper reports on an analysis of nearly 27,000 hematological malignancies diagnosed in the U.K. in the 10-year period 1984 to 1993. The unique observations provided in this analysis are the similarity of the sex-specific curves by age for acute myeloid leukemia, myelodysplasia types, polycythemia rubra vera, and myelofibrosis (or myelosclerosis). The unusual age-sex-specific distribution of essential thrombocythemia, suggesting a unique epidemiology, has never been reported before. The unusual female excess of the nodular sclerosing adolescent peak in Hodgkin's disease and its rapid fall with time is potentially of great importance. An even younger childhood peak of acute lymphoblastic leukemia is presented. Overall these data represent the most reliable available in the U.K., being population based, specially collected and will form the basis of considerable further investigations.

Leukaemia and non-Hodgkin's lymphoma in children and young adults: are prenatal and neonatal factors important determinants of disease?

A medical record-based study of leukaemia and non-Hodgkin's lymphoma diagnosed before the age of 30 years was carried out at three hospitals in the south of England. Findings for 177 cases and 354 age- and sex-matched controls are presented here. For documented viral infection in pregnancy, the odds ratio (OR) was 6.0 [95% confidence interval (CI) 1.2-29.7] for leukaemia and infinity (95% CI 1.24-infinity) for non-Hodgkin's lymphoma. Mothers of leukaemic cases were more likely to be anaemic, the OR for a pregnancy haemoglobin below 10 g being 3.8 (95% CI 1.3-11.1). An association with birthweight was found for acute myeloid leukaemia, the OR for birthweights > 3500 g being 6.2 (95% CI 1.3-29.8). Further, the preceding siblings of those diagnosed with any form of leukaemia were also more likely to weigh > 3500 g at birth (OR 2.2; 95% CI 1.1-4.4). Overall, leukaemic cases appeared to be comparatively robust at birth with respect to other indicators of well-being, the ORs for jaundice, phototherapy, admission to special care nursery and neonatal intensive care all being less than 1.0. Further, no relation between childhood leukaemia and neonatal administration of intramuscular vitamin K was noted (OR 0.6, 95% CI 0.3-1.4; for acute lymphoblastic leukaemia diagnosed between the ages of 1 and 6 years).

Delayed-type hypersensitivity elicited by synthetic peptides complexed with Mycobacterium tuberculosis hsp 70.

Four synthetic peptides bearing dominant CD4+ T-cell epitopes of the 19,000 and 38,000 MW proteins of Mycobacterium tuberculosis were used to provoke a delayed-type hypersensitivity (DTH) reaction in mice previously immunized with recombinant 19,000 and 38,000 MW proteins. It was found that an effective enhancement of the DTH reaction was obtained if the peptides were administered as a complex with M. tuberculosis hsp 70 protein. The increase in reactivity was not obtained when hsp 70 and peptide were co-injected at the same site, but not in complex, or when the specific peptide was displaced from the complex by an irrelevant peptide with high capacity to bind hsp 70. One of the antigenic peptides whose capacity to complex with hsp 70 is low, failed to show the enhancement of DTH when injected together with hsp 70.

Sex ratios: are there natural variations within the human population?

OBJECTIVE: To investigate on a population basis the suggestion that certain factors naturally alter the odds of having a boy or a girl, and that some women are predisposed towards having children of one particular gender. DESIGN: Routine data analysis. POPULATION: Routinely collected data on singleton infants born in Scotland from 1975 to 1988, linked so that births (live and still) to the same mother could be identified. The analyses relate to 549,048 first to fifth order births occurring to 330,088 women whose records were complete from the first delivery onwards. MAIN OUTCOME MEASURES: Gender of infant. RESULTS: Of 549,048 births, 51.4% were male. Apart from random variation, the sex ratio of 1.06 remained constant at all birth orders (P = 0.18). The probability of a male infant appeared unrelated to the genders of the preceding siblings (P > 0.20 in second to fifth deliveries), and there was no evidence of variation with maternal age (P = 0.31), maternal height (P = 0.69), paternal social class (P = 0.12), maternal social class (P = 0.57), year of delivery (P = 0.84) or season of birth (P = 0.41). Whilst mothers whose children were all the same gender were more likely to continue childbearing than those with children of different genders, there was no evidence that those with daughters were more likely to continue than those with sons. CONCLUSIONS: The suggestion that some women have a natural predisposition towards having children of a particular gender is not supported by these data. On a population basis there is no evidence to suggest that gender determination is anything other than a chance process.

Spontaneous abortion in dry cleaning workers potentially exposed to perchloroethylene.

OBJECTIVES: To investigate the association between spontaneous abortion and work within dry cleaning units in the United Kingdom where the solvent perchloroethylene is used. METHODS: A retrospective occupational study of reproductive outcome in 7305 women aged 16 and 45 years, who were currently or previously employed in dry cleaning or laundry units in the United Kingdom. Data on workplace exposures and reproductive outcome were obtained by postal survey. A sample of reported spontaneous abortions was validated from medical records. Machine operator versus non-operator was used as a surrogate for exposure to perchloroethylene in dry cleaning units as no data on individual doses were available for women in this study. RESULTS: The response rate was higher for current workers of dry cleaning units (78%), than for past workers of dry cleaning units (46%). Similarly, the response for current laundry workers (65%) was higher than that for past laundry workers (40%). Overall, the reproductive characteristics of the respondents were similar to expectation. Examination of exposure at the time of pregnancy, however, showed that the rate of spontaneous abortion varied according to the type of work the women did during the pregnancy or in the three months before conception: being lowest for pregnancies not exposed to either dry cleaning or laundry work (10.9%), higher for those exposed to laundry work (13.4%), and higher still for those exposed to dry cleaning work (14.8%). Within the group of pregnancies exposed to dry cleaning, the proportion was higher if the woman reported that she worked as an operator at the time of the pregnancy (17.1%) rather than as a non-operator (11.6%). Adjusted odds ratios for the period 1980-95 showed that the risk was over 50% higher in operators than non-operators (p = 0.04). The physical demands of the two jobs are likely to be similar. A higher risk was found when work as a dry cleaning operator was compared with no work in either dry cleaning or laundry units during pregnancy. Exposure to dry cleaning as a non-operator was not associated with any excess risk. CONCLUSIONS: Women who worked in dry cleaning shops at the time of their pregnancy or in the three months before who described themselves as operators were about half as likely again to report that their pregnancy ended in a spontaneous abortion than women who described themselves as non-operators.

Target organ damage and ambulatory blood pressure in stage I hypertension. The Hypertension and Ambulatory Recording Venetia Study.

According to recent international guidelines the decision on whether to treat young subjects during the early phase of hypertension should be based not only on their office blood pressure but also on their ambulatory blood pressure and whether target organ damage has occurred. Few data on the prevalence of hypertensive complications in young subjects with mild hypertension are available. In the Hypertension and Ambulatory Recording Venetia Study (HARVEST), a multicenter trial conducted in northeast Italy, the percentage of young borderline-to-mild hypertensive subjects with echocardiographic left ventricular hypertrophy was 4.5% and the percentage with concentric remodeling was 4%. Clear differences in cardiac size and geometric adjustment to ambulatory systolic pressure between the two sexes were found. The impact of blood pressure on the walls of the left ventricle and on the left ventricular mass was remarkable in women but weak in men. The assessment of left ventricular systolic function confirmed that many young mild hypertensive subjects have an increased ejective performance. The left ventricular contractility evaluated by midwall measurement was, however, found to be depressed in 9.2% of the HARVEST participants. Their left ventricular diastolic function was similar to that of 50 normotensive controls. The prevalence of microalbuminuria [albumin excretion rate (AER) > 30 mg/24 h) was 6.1%, only slightly higher than that found by other authors among normotensive subjects and much lower than that observed among patients with more severe hypertension. For our stage I hypertensives, however, the AER was correlated to the 24 h blood pressure with high statistical significance, whereas we found no relationship between the AER and left ventricular mass index either for all of the subjects taken together or for the men and women considered separately. The results suggest that renal and cardiac involvement do not occur in parallel during the initial phase of hypertension.

Cancer in electrical workers: an analysis of cancer registrations in England, 1981-87.

Associations between work in the electrical and electronic industry and cancer incidence were assessed using data for 371 890 cancers registered in England between 1981 and 1987, of which 7981 were in electrical workers. Proportional registration ratios (PRRs) were calculated, both with and without the commonest cancers, with adjustment for age, social class, cancer registry of origin and sex. Of four cancers previously linked with work in the electrical and electronic industry (leukaemia, brain, breast and melanoma), only two were significantly raised: leukaemia (PRR=124, 95% CI=109-142, based on 217 cases) and malignant brain cancer (PRR=118, 95% CI=103-136, based on 204 cases). A significantly increased risk was also observed for pleural cancer (PRR=201, 95% CI=167-241, based on 115 cases). The histology of almost 90% of pleural cancers was coded as mesothelioma, confirming the previously observed association between pleural cancer and exposure to asbestos in electrical workers. The extent to which workplace exposures to extremely low frequency electromagnetic fields explains the excesses seen here for leukaemia and brain cancer requires further study.
