Peroneal paralysis due to exostosis of the fibula. Report of 2 cases.

Exostoses in certain situations such as the armpit or the popliteal region, can cause vascular or neurological compression syndromes. Exostoses of the neck of the fibula can in this way produce late paralysis of the peroneal nerve. Two such cases are presented in young patients, in whom the neurological damage was produced by increased growth distally of the neoplasm.

Serum tryptophan in humans with senile cataracts.

Serum free and total tryptophan levels have been measured in patients with cataracts and compared with the same levels in controls with clear lenses. No statistically significant differences have been demonstrated between the two groups of examined fasting subjects. Preliminary results seem to indicate that differences could be evident after L-tryptophan oral load.

Treatment of osteochondritis dissecans and cartilaginous fractures of the knee by osteo-cartilaginous autografts.

Osteochondritis dissecans and cartilaginous fractures of the knee joint are lesions in which, on removal of the necrotic areas of cartilage, extensive loss of substance may result, such as to limit joint function and be a predisposing cause of arthrosis. In such cases it is suggested that, in addition to curettage of the necrotic focus, an autograft of cartilage and spongy bone should be used, obtained from another part of the condyle of the same knee which is not weight bearing. The surgical technique used in fifteen such cases is described, and in particular the surgical instruments which when suitably prepared make this an easy and safe operation. The follow up was two years.

Choroidal lacunae and Aicardi's syndrome.

Three children with flexion spasms and an ophthalmologic picture characterised by choroidal lacunae are described. Aicardi's syndrome was diagnosed in two cases. A diagnosis of Aicardi's syndrome should include CT scan and/or pneumoencephalographic findings of partial or total agenesis of the corpus callosum and/or other anomalies of the interhemispheric structures. Choroidal lacunae are not entirely pathognomonic.

[Double contrast knee arthrography in meniscal lesions (author's transl)].

Double contrast arthrography, under fluoroscopic control, is a technically simple procedure with a high degree of diagnostic accuracy. The Authors performed 95 arthrographies in patients with suspect meniscal lesion, making a correct diagnosis of 84 (94%) out of 89 cases undergone surgical intervention. They conclude giving the indications to arthrographic examination, which needs to be limited to those cases in which the clinical diagnosis is difficult on uncertain.

Isolated fracture-avulsions of the tibial insertions of the cruciate ligaments of the knee.

This paper discusses the treatment of isolated fracture-avulsions of the tibial insertions of the cruciate ligaments of the knee. The indications for conservative treatment or open reduction and osteosynthesis, are based on a classification by Meyer and McKeever which takes into account the displacement of the avulsed fragment. This is the crucial factor in the choice of treatment.

Rare causes of ulnar nerve compression in the epitrochlear groove.

The commonest causes of ulnar nerve compression in the epitrochlear groove have been widely reported in the literature. The author reports four rare causes, viz.: neurinoma of the ulnar nerve (one case), compression of the ulnar nerve due to an arthrogenic ganglion (two cases), ulnar neuritis due to articular gout (two cases), and compression of the ulnar nerve due to post-traumatic fibrosis in the epitrochlear groove (one case).

[Photographic moiré method for preliminary diagnosis of spine deformity (author's transl)].

Moiré topography is a biostereometric method which produces a three-dimensional image of the shape of the trunk. In structure scoliosis an asymmetry of the two halves of the back is seen. This method has been studied and the range of the asymmetry of the moiré pattern has been compared with clinical observations and X-ray findings in case with structural scoliosis. Because of the sensitivity of the moiré method an asymmetry of at least half fringe interval could be regarded as a positive result. All the observed asymmetries less than one fringe interval had a lateral deviation of the spine of less than 10 degrees according to Cobb when W-ray examined. The moiré method seems to be very suitable for the screening of structural scoliosis, owing to: 1) its ability to detect and document even small deformities by photography; 2) the simplicity of the technique; 3) the possibility of documenting the condition of the back for comparison with subsequent examinations; 4) non-invasive for the patient.

[Activity of coagulation factors in liver coma caused by viral hepatitis].

Coagulation studies were performed on 8 patients with acute liver failure from viral hepatitis. Four died and four recovered. At the time of development of coma, levels of clotting factors resulted markedly reduced in all cases and were not of value as a prognostic index. Subsequent serial determinations (every 8 hours) during the evolution of coma showed that prothrombin activity and levels of factor I, II, V and X were not correlated with clinical conditions, most probably owing to the influence of the administration of blood and blood components. Of all coagulation tests, the assay of factor VII, which has the shortest half-life, appeared the most reliable index of hepatic synthetic function at a particular point in time.

[Autoplastic bone-grafting on repair of long bones of hands (author's transl)].

Cortico-spongious autoplastic bone grafts have in reconstructive hand surgery several precise indications. Bone infusion is certain if the technique used is appropriated permitting the cosmetic and functional recovery of the corresponding digital ray. The indications are: post-traumatic non-union, great loss of bone, bone neoplasm. The authors basing their causistic on 28 cases discuss the indications, surgical technique and results.
