[Magnetic resonance in the diagnosis, characterization, and assessment of renal carcinoma extension].

Many authors discussed the reliability of MR imaging in the diagnosis of small tumoral masses (less than 1.5 cm) affecting the kidney, and stressed its low diagnostic power. On the other hand, MR imaging is a useful technique in the study of renal masses, for its capability in the differentiation of adipose tissue (angiomyolipomas), fluid content (cystic lesions), parenchymatous tissues (neoplastic lesions). The distinction between malignant and benign masses is often impossible on the basis of MR except in the cases of invasion of the adjacent structures. MR imaging is, at present, the most accurate method in the staging of neoplastic masses of the kidney.

[Focal lesions of the spleen: contribution of magnetic resonance imaging].

MR findings in the study of focal splenic pathology are discussed in this paper. MR examination of the spleen shows some technical limits in the diagnosis of masses affecting this organ. On the contrary, the introduction of new paramagnetic contrast agents, with organospecific affinity, will no doubt improve the diagnostic power of MR imaging, in order to gain more information about these diseases.

[Characterization of hepatic neoplasms: contribution of magnetic resonance].

The reliability of MR imaging in the diagnosis and characterization of space occupying lesions affecting the liver are discussed in this study. We discuss the role of MR imaging in the diagnosis of these tumours and its advantages compared with other available techniques such as ultrasonography and CT. We also discuss the results of MR in the characterization of focal neoplastic masses.

[Role of magnetic resonance in the evaluation of space-occupying mediastinal pathology].

All MR findings in the study of mediastinal masses are discussed in this report, with the aim of evaluating the reliability of MR imaging in this pathology. The results obtained in the assessment of the extension of these masses are very useful and MR imaging appears to be the most informative of all techniques. Masses originating from the thymus, cystic lesions, substernal goitre, and esophageal tumors were considered. MRI results were compared to those obtained by CT scan which is considered of proven reliability for the evaluation of mediastinal masses.

[Images of pancreatic pathology: advantages and limitations of magnetic resonance compared with computerized tomography and ultrasound].

We discuss the usefulness of MR examination in the diagnosis of pancreatic diseases, in addition to the other available imaging techniques. MR imaging yields important information in diagnosis and staging of pancreatic tumours, and in the characterization of pancreatic masses. MRI is also valuable for the study of chronic as well as acute pancreatitis, although there are some limits such as the poor detection of calcifications in the chronic, and the excessively long time required to complete the examination in the acute form.

[Computerized tomography and magnetic resonance in the evaluation of patients with Crohn disease. Their role in the identification, assessment of extent and management of the disease].

In this study we evaluated the role of barium examinations and above all computed tomography and magnetic resonance imaging in the study of Crohn's disease. CT is most often requested for patients with suspected or known intraabdominal or pelvic abscesses. MR plays a very important role in the localization of perirectal and perianal abscesses.

Pre-operative staging of endometrial carcinoma: magnetic resonance imaging versus ultrasounds and hysteroscopy.

Ultrasound, hysteroscopy and magnetic resonance imaging has been considerated to assess the loco-regional or extrapelvic extension of the endometrial carcinoma. Sonography has demonstrated a certain inaccuracy in predicting myometrial invasion or the involvement of the canal. Hysteroscopy allows us to characterize neoplasia and to assess its extension in the cervical canal. MR imaging is more helpful in the diagnosis of channel invasion. The assessment of ovarian metastasis requires ultrasonography or coronal planes RM imaging. As regards the involvement of the pelvic and extrapelvic lymph nodes MR is more accurate than ultrasound scan.

[Correlations between magnetic resonance, ultrasonography, and computerized tomography in invasive gynecologic pathology].

In this report we evaluated the indications, the results and limits of MR examination in the diagnosis of neoplastic diseases affecting the ovary and in the staging of endometrial or cervical cancerous processes. MR findings are compared with the results of other techniques like Ultrasonography or Computed Tomography. The best advantages of MR imaging are the lack of radiation and the possibility of multiplanar studies of the pathologies. For these reasons we consider MR imaging a more accurate method for the study of neoplastic disease affecting the gynecologic sphere.

[Role of magnetic resonance in the identification, characterization and staging of prostatic cancer. Comparison with transrectal echotomography and computerized tomography].

The results of MR imaging in diagnosing, and characterizing of prostatic malignant tumour, as well as its feasibility in assessing the extension are discussed in this paper. We also compared MR findings with the results obtained from trans-rectal ultrasonography and computed tomography. In particular, we emphasize the important role of MR examination with trans-rectal coil which is, at present, the most sensitive technique for the identification and characterization of prostatic tumours as well as for assessing their extension.

[The role of bioimaging and interventional technics in the diagnostic and therapeutic management of jaundice].

Differential diagnosis between medical and surgical icterus is often difficult, if based on clinical data only. Echotomographic studies represent the first choice method between diagnostic imaging techniques. If a negative result is found (due to initial phase or an intermittent icterus) a new examination can be performed after few days; a scintigraphic study with HIDA can be also performed. If a positive result is found on echotomography, a CT scan can be performed and, if necessary, an ERCP or a TPC. MR imaging is an available technique to ameliorate the evaluation of lesion's extension and its relationship with adjacent vascular structures. Talking about therapeutic implications, we find quite useful to apply an endobiliar prosthesis during the first phase, in order to better understand the real extension of the lesion. Surgical treatment is justified, if a radical resection is possible and in all cases with a prognosis of over 6 months, because of the minor percentage of long time complications in traditional surgery with respect to interventional radiologic techniques.

[Diagnostic imaging in the evaluation of non-neoplastic space occupying pathology in the sellar and suprasellar region].

Before the introduction of computed imaging techniques like CT scan and MR, the differential diagnosis of lesions affecting the sellar and suprasellar regions was very difficult, more often impossible. These techniques contributed to ameliorate the typing of pathological tissues. Non neoplastic pathological findings of this region are very different and they can variate between phlogistic processes, vascular lesions, hamartomatous formations or congenital malformations. In this work we describe the characteristic changes of morphology and radiologic aspect in the diagnostic study of this region, using CT scan and MR imaging.

[The role of magnetic resonance in the evaluation of pathological processes involving the penis].

The above paper was aimed at defining the usefulness of MRI in the study of pathologies involving the penis. Disorders considered include Peyronie's disease, fibrosis of the penis, priapism, trauma, congenital anomalies, and tumors. In Peyronie's disease and in fibrosis of the penis, MRI permits to demonstrate the abnormal thickening of tunica albuginea and erectile tissue; after administration of gadolinium, it becomes possible in some cases to demonstrate plaques of the albuginea that are not palpable. In priapism, MRI is useful both for identifying the cause and for assessing the time elapsed since blood extravasation. In traumatic and congenital pathologies, MRI is essential for the acquisition of all information on anatomy that may be useful for selecting the most suitable surgical therapy. In neoplastic pathology, MRI has proved useful for accurately assessing the extension of lesions involving the basis of the penis.

[Characterization of space occupying masses in the diencephalic-hypothalamic region. MRI vs. TC].

All morphologic aspects of neoplastic and non neoplastic masses, arising from the diencephalic-hypothalamic region are discussed in this paper. We describe the densitometric characteristics on CT scan and the MRI appearance of masses affecting this region, both of benign lesions and malignant neoplastic masses. We describe separately masses arising from within the III ventricle and those of extraventricular origin and all semiotic findings of neuroradiological techniques of computed imaging.

[Computerized tomography and magnetic resonance in the evaluation of neoplastic processes localized on the floor of the third ventricle].

In our experience, neoplastic lesions of the floor of the third ventricle are frequently responsible for neurologic and dysendocrine symptoms. Ct scan and MR imaging are the most reliable techniques of neuroimaging in the diagnostic study of neoplastic masses affecting this region. In this work, CT and MR appearance of lesions of the III ventricle floor are discussed with the aim of contributing to the differential diagnosis of neoplasms and their surgical and medical management.

[The role of tomography (X-ray and magnetic resonance) in the diagnosis and determination of the extension of tumors in the roof of the third ventricle].

We describe the CT scan and MR appearance of neoplastic lesions affecting the roof of the third ventricle. The neoplastic lesions of this region may be of neuroepithelial origin (sub-ependymoma, colloid cyst), meningeal cell origin (meningiomas), metastatic or primary localization of lymphoproliferative tumors (lymphomas) and dysembryogenetic origin (epidermoid tumor). CT scan and MR imaging are useful techniques for diagnosis and planning of surgical or medical treatment. The primary role of these imaging techniques, is discussed in the quantification of extension, biologic malignancy and anatomical relationships between pathologic lesion and normal nervous tissue.

[Images of hepatic parasitic cysts: echotomography, computed tomography and magnetic resonance compared].

Hepatic hydatic cysts are a very common finding in the endemic areas like Australia, the Middle East, New Zealand, South Africa and in some countries bordering the Mediterranean. Immigration has increased the finding of this pathology in some European and North American areas. Our purpose is to show possibilities and limits using US, TC and RM to demonstrate the presence of parasitic hepatic localizations and their evolution after therapy.

[Role of magnetic resonance in the evaluation of testicular diseases].

The above paper was intended to evaluate the usefulness of MRI in the study of pathologies involving the testis, on the basis of the authors' experience and of the relevant literature. Pathologies considered included cryptorchidism, torsion of the spermatic cord, epididymitis, orchitis, and testicular tumors. MRI proved very useful in cryptorchidism, thanks to the multiple planes and good contrast resolution between different tissues. It also yields valuable information allowing to distinguish between malignant transformation of the retained testicle and its fibrotic involution. In malignant pathologies, MRI is useful for differential diagnosis between seminomatous and non-seminomatous tissues, while for purposes of staging its accuracy is only 36% but is nevertheless superior to that of U.S. (45%). In the case of other pathologies, MRI findings are non specific, and the technique should therefore be employed only when U.S. findings are unsatisfactory.
