[Recurrent purulent bacterial meningoencephalitis].

During the period of 25 years there were 55 patients treated in our Institute because of recurrent purulent bacterial meningoencephalitis(rpbme). This group consisted of 42 males (76%) and 13 (24%) females, the prevalent number (53%) of patients being under 21 years of age. The diagnosis of rpbme was based on the commonly accepted criteria and confirmed by the laboratory results of CSF examination. The cause of the recurrences was established considering the skull X-ray examination, CT and MRI. The evaluation of the clinical status was based on the Glasgow Coma Score (GCS). During the first hospitalisation, severe or critic clinical status was noted in 42 patients (76%) and moderate in 13 (24%). The subsequent recurrences were mostly moderate, rarely severe or mild. The number of recurrences varied from 1 to 9. During the first hospitalisation, the etiologic factor was detected in 39 patients (71%), i.e. Streptococcus pneumoniae in 28 (51%), Neisseria meningitidis in 8 (14%), Pseudomonas aeruginosa and Staphylococcus aureus in 2 and 1 patients respectively. In 37 patients (67%) rpbme developed following cranial trauma, in 18 cases (33%) with single or comminuted fractures of the anterior cranial fossa (in 4 cases accompanied by CSF nasal exsudate). In 4 it followed neurosurgical intervention, in 3 it accompanied recurrent purulent highmorities, in 1 case--after removal of the nasal polyps and subsequent CSF nasal exsudate, and in 1 patient with recurrent mastoiditis. In 6 cases (11%) the cause of the recurrences remained unelucidated. The clinical signs and symptoms, diagnostic difficulties and the causative treatment of rpbme are discussed. In the authors' opinion, surgical treatment of the communication between the CSF and the external environment prevents the recurrences and is the only successful way of treatment. Special attention is drawn to the great diagnostic value of CT and MRI. The use of other modern techniques, e.g. positron emission tomography (PET) is recommended, because it is useful not only in the functional evaluation of the cerebral tissue after the injury, but also in assessing the dynamics of pathologic changes.

[Sepsis--generalized infection. The treatment of adults based on personal observations].

Basing on the own experience, the authors discuss causative treatment of sepsis, mainly of unknown etiology. Emphasis is on the depression of immunological system in the acute phase of the disease. Therefore, a combined treatment with 2, often 3 or even 4 bacterial antibiotics is recommended, together with passive immunotherapy, and in certain cases surgical removal of the infection foci.

[Bacterial meningitis in patients with sepsis syndrome].

The authors discuss problems connected with diagnosis, management and treatment of bacterial meningitis among patients with the sepsis syndrome. Considering secondary organ changes bacterial meningitis belongs to the severest one and as a life-threathing sequel of sepsis demands immediate use of proper casual treatment. The authors show the therapeutic difficulties in this group of patients particularly when the etiological organism is unknown. They discuss this problems and present their own schemes of tretment. They indicate the value of passive immunotherapy and surgical removal of the primary source of infection. They emphasize final result depends on secondary organ changes, age, immunity of patient and the kind of etiological agent.

[Tests of Helicobacter pylori survival in milk].

The survival of microaerophilic bacteria Helicobacter pylori (Hp) isolated from gastric mucosa was studied in 2 ml and 10 ml samples of milk contaminated with a suspension of these bacteria in 20% glucose solution. Milk was kept at 5-7 degrees C temperature in refrigerator and at room temperature. Hp was found in samples kept at 5-7 degrees C during 3 days. On the other hand, Hp was not found after 24-hours keeping at room temperature.

[Rappaport--Vassiliadis medium--a new liquid selectively-multiplying medium for detection of Salmonella in food].

On the basis of the available literature, the history of development of the Rappaport--Vassiliadis (RV) medium, its preparation and applicability in food control methods were described. Results of comparative studies on the effectiveness of Salmonella isolation upon use of different specifically-multiplying media: MK, medium containing sodium acid selenite, R25/37 degrees C, RV, were presented. The number of Salmonella isolations in samples multiplied in RV medium was found to exceed that obtained upon use of the remaining media. Moreover, RV medium was easy to prepare; it proved to be economical and was stable for up to 7 months when refrigerated.

[Modification of the test for determining bacterial capacity for nitrate reduction].

For facilitation of the detection of bacteria able to reduce nitrates the presently used method of 24-hour broth culture with KNO3 and the method of growing on slant agar cultures with KNO3 (standard method) were compared, with certain modifications introduced. It was found that shortening of the incubation period from 24 to 6 hours and reduction of the volume of broth with KNO3 from 10 to 0.5 ml made possible obtaining of a greater number of correct results as compared to the standard method. In the 24-hour broth cultures with KNO3 done as yet in case of negative results it is indispensable to confirm them in the test with zinc dust. The placement of Griess reagents on points onto colonies growing on KNO3 agar may lead to false positive results.

[Effect of preincubation time and concentration of selective media on the effectiveness of salmonella organism isolation from frozen meat homogenates].

Double concentrated RV and SC media were used for selective multiplication of frozen meat homogenates preincubated in buffered peptone water during 6 and 20 hours. It was found that cultures of 2 ml of homogenate preincubated during 6 hours in 10 ml of twofold concentrated RV medium, and selective multiplication during 20 hours made possible obtaining of a greater number of Salmonella isolates as compared to samples preincubated during 20 hours and transferred in volumes of 0.1 ml into 10 ml of normal concentration medium. On the other hand, the growth of 1 ml of a culture preincubated during 6 hours transferred into 10 ml of twofold concentrated SC medium yielded a similar number of salmonella isolates as in the case of samples preincubated during 24 h and transferred in 1 ml volumes into 10 ml of the SC medium without concentration change.

Postnatal treatment of rats with beta-adrenergic agonists or antagonists influences differentiation of sexual brain functions.

Sexual differentiation of the brain seems to be influenced by postnatal interaction of gonadal steroids with neurotransmitter systems, in particular the adrenergic system. Stimulation or inhibition of alpha-adrenergic receptors during early postnatal development had previously been shown to influence steroid-induced sexual differentiation of brain functions. In the present study newborn male and female rats were treated either with salbutamol, a selective beta 2-adrenergic receptor agonist, isoprenaline, a general beta-adrenergic receptor agonist, or with alprenolol, a general beta-adrenergic receptor antagonist. In adulthood the female animals were ovariectomized and were tested for the capacity to show an LH-surge response and female sexual behaviour after priming with estradiol benzoate (EB) and progesterone (P). Male animals were tested for expression of male sexual behavior and, after gonadectomy and priming with EB + P, for the capacity to show female lordosis behavior. In summary, our results suggest that activation or inhibition of beta-adrenergic receptors during postnatal development permanently impairs the responsiveness of the center for cyclic gonadotropin release to gonadal steroids in female rats and impairs the expression of ejaculatory behavior in male rats. A slight stimulatory effect on the expression of female lordosis behavior was observed in male and female rats after postnatal activation of beta-adrenergic receptors.
