Natural hazards: causes and effects. Lesson 5--Tropical cyclones (hurricanes, typhoons, baguios, cordonazos, tainos).

The many conclusions drawn and lessons learned from past cyclones can be used to mitigate and better respond to future occurrences. Some of the most helpful are listed below: 1) Outbreaks of cholera do not follow cyclones. Cholera must be endemic previously to a community; 2) Waterborne diseases do not increase as a result of cyclones; 3) Massive food aid rarely is required after a cyclone; 4) Used clothing almost never is needed. It usually is culturally inappropriate. Though accepted by disaster victims, it almost never is worn; 5) Blankets can be useful, but if they are needed, they usually can be found locally and do not need to be imported; 6) Assistance by outsiders is most effective in the reconstruction period, not the emergency phase; 7) Most needs are met by the victims themselves or their local governments; 8) In general, victims do not respond to disasters with abnormal behavior. Cyclones do not incite panic, hysteria, or rioting; 9) Cyclone relief and reconstruction programs should be integrated with long-term development programs; 10) When properly executed, reconstruction assistance can provide a strong stimulus to recovery and a base for future development work; 11) Reconstruction programs should seek to reduce vulnerability to future disaster; 12) Re-establishment of the local economy, income security, and agriculture usually are more important to cyclone victims than is material assistance; and 13) Churches, schools, and other large buildings that often are designated as cyclone shelters usually are not safe. The number of deaths attributed to destroyed or flooded shelters is alarming. Most experts agree that the best alternative is adequate warning and evacuation of the threatened areas.

Natural hazards: causes and effects. Lesson 7-Drought.

Drought has long been recognized as one of the most insidious causes of human misery. Today, it is the natural disaster that annually claims the most victims. Its ability to cause widespread misery is estimated to be increasing. While generally associated with semiarid climates, drought may occur in areas that normally enjoy adequate rainfall and moisture. In the broadest sense, any lack of water for the normal needs of agriculture, livestock, industry, or human population may be termed as a drought. The cause may be lack of supply, contamination of supply, inadequate storage or conveyance facilities, or abnormal demand. Drought is a condition of climatic dryness severe enough to reduce soil moisture and water below the minimums necessary for sustaining plant, animal, and human life. Drought usually is accompanied by hot, dry winds and may be followed by damaging floods. More socially relevant than technically correct is the definition used by Ari Toubo Eibrahim, the minister of agriculture in Niger, who has said that a drought is "Not as much water as the people need."

Anti M. leprae IgM antibody determination by ultramicroimmunoenzymatic (UMELISA HANSEN) for the diagnosis and monitoring leprosy.

The relationship between the IgM antibody response, antigenic load as well as the clinical improvement after chemotherapy was studied in order to obtain useful data for the early diagnosis and monitoring leprosy. A level of 82% (94/115) agreement was obtained between IgM UMELISA HANSEN and slit-skin smear examination. Discrepant results were observed in 16 patients who showed positive IgM response despite negative by the skin smear examination. In these patients, the IgM response was seen to be associated to the early signal for bacilli recurrence in the skin. In one of these patients the presence of bacilli was demonstrated in the skin, two months after IgM antibodies being detected by UMELISA HANSEN. Also in one of the treated patients positive by both diagnostic techniques, a remarkable decrease in the IgM antibody levels was seen, correlating with a significant clinical improvement. Moreover it was found a direct relationship between the IgM antibody response and bacterial antigenic load, regardless the time elapsed in the disease's evolution.

[Intestinal parasite infections and schistosomiasis in a poor urban area, in townships of the sugar cane belt and in villages of the semi-arid area of North-East Brazil].

We determined the prevalence of intestinal parasite infections between 1993 and 1998 in the populations of a poor quarter of Recife (Pernambuco), in two townships in the sugar cane belt and in three villages of the semi-arid area far from the coast. Intestinal schistosomiasis was present in the sugar-growing area but was not observed in the populations inland that use dams to provide irrigation and fishing. Ascaris was very common in the city and the sugar cane belt and large numbers of hookworms were observed, especially in the sugar-growing area. These nematodes were very rare in the semi-arid area. This distribution probably results from both climatic conditions and human behavior. The high frequency of Amoeba cysts demonstrates that the peasants, farmers and fisherman living in these areas have poor hygiene practices. There are probably many, complex relationships between education, income, lifestyle and intestinal parasite infections.

Specific interactions of synthetic peptides derived from P. falciparum merozoite proteins with human red blood cells.

In the search for strategies which might help in the elucidation of molecular mechanisms involved in the red blood cell (RBC) invasion by P. falciparum merozoites, and with the specific aim of establishing whether synthetic peptides derived from selected parasite proteins bind to human RBCs, 26 different peptides were chemically synthesized and radiolabeled. It was found that the peptides could be grouped, according to their RBC-binding kinetics, into high, medium and low binding activity. A correlation was detected between the high binding activity of a peptide and the presence of either a KEK motif (or its variants LEK or KEL) or a NVXAA (where X is V or Y). Peptides with medium or low binding activities did not possess either of these two consensus sequences. Selective modification of amino acids within the KEK motif diminished their uptake or binding capacity. Competitive inhibition assays of labeled or unlabeled peptide demonstrated a correlation between the presence of KEK or NVXAA motifs and a high binding activity of a peptide. Invasion-inhibition studies showed a direct correlation between a peptide's binding activity and inhibitions of human RBC reinvasion. Other experiments showed that high binding activity peptides show a decreased uptake with related and nonrelated human erythrocytes.

A neurochemical approach to the study of sleep.

Sleep deprivation and recovery induce, in the hypothalamic preoptic region of the rat, opposite changes in cyclic AMP concentration which, when plotted in the standard form of the normal distribution, define two significantly divergent distributions. Thus, these opposite functional conditions may be related to corresponding changes in a biochemical parameter within an area involved in the control of the sleeping-waking cycle.

[Perisinusoidal fibrosis in idiopathic thrombopenic purpura].

Perisinusoidal fibrosis of the liver was discovered in a 22 year-old woman with idiopathic thrombocytopenic purpura requiring splenectomy. No etiology of fibrosis could be found. As shown by immunocytochemistry, collagen types I, III, IV, laminin, and fibronectin were increased. Under electron microscopy, numerous collagen bundles and fragments of basement membrane-like material were demonstrated. Perisinusoidal cells loaded with lipids showed several of the characteristics of fibro/myofibroblasts, while the activity and number of Kupffer cells were apparently increased. The exact mechanism of fibrosis in this case remains unknown but it is likely that platelet destruction and/or Kupffer cell hyperactivity played a role.
