Immunohistochemical recognition of human nerve sheath tumors by anti-Leu 7 (HNK-1) monoclonal antibody.

Using relatively high dilutions of anti-Leu 7 monoclonal antibody and a four-step peroxidase-antiperoxidase (PAP) reaction in paraffin-embedded tissues, we tested the affinity of this antibody to the cells of 47 human nerve sheath tumors and 22 other tumors in which the differential diagnosis with nerve sheath neoplasms is known to arise. Of all the nerve sheath tumors studied 68%, including 80% of the schwannomas, contained anti-Leu 7-positive cells. All 22 non-schwannian neoplasms were entirely negative. Specimens of eight experimental malignant rat schwannomas were also negative for anti-Leu 7 antibody. Our findings suggest that anti-Leu 7 monoclonal antibody is a promising marker that may facilitate the differential diagnosis between human Schwann cell and non-Schwann cell neoplasms.

Interaction between perinatal brain damage and processes of normal brain development. Ultrasonographic and neurodevelopmental study in the first year of life.

A systematic ultrasonographic study and a prospective neurodevelopmental assessment were carried out in a population of high-risk neonates. In group A (82 preterm infants of 34 weeks gestation or less), periventricular-intraventricular hemorrhage (PVH) and periventricular leukomalacia (PVL) were the commonest lesions. The association of PVH, PVL and ventricular dilatation had a variable outcome and the prognosis was found to be poorer in the presence of diffuse or extensive PVL. In group B (115 neonates of more than 34 weeks gestation), miscellaneous ultrasound changes were observed (malformations, infections, hemorrhages and hypoxic-ischemic lesions). Malformations, hypoxic-ischemic damage and prenatal infections had a gloomy prognosis. The main targets of hypoxic-ischemic damage in the immature infant were the germinal layer and the periventricular white matter, while in the mature infant the cortex and basal ganglia were more vulnerable. A relation between the localization and the size of the lesion could be established. In conclusion, basic forms of cerebral damage should therefore be understood in terms of brain maturation, type and timing of the insult, extent and localization of the lesion.

Rapid diagnosis of herpetic infections by immunoperoxidase method.

The PAP-(peroxidase-antiperoxidase)-immunohistochemical method was tested for rapid diagnosis on corneal specimens in clinically suspected herpetic disease. The method proved accurate, easy and useful in rapid diagnosis of corneal specimens from epithelial keratitis. Corneal buttons after keratoplasties for herpetic lesions were similarly tested for the presence of herpetic antigen. In a total of five corneal buttons, antigen was found in two corneas with necrotizing keratitis, whereas it was not noted in the three corneas opacified by disciform edema.

Agenesis of the corpus callosum: real-time ultrasonographic diagnosis and autopsy findings.

Ultrasonographic appearances of agenesis of corpus callosum and its associated abnormalities are described in four cases. Clinical data, ultrasonographic diagnostic criteria, and correlation with neuropathological findings are presented. Ultrasound is the method of first choice in the neonatal period for the detection of cerebral malformations and more particularly of agenesis of corpus callosum.

Herpes simplex virus encephalitis: a case with Korsakoff's syndrome and an immunoperoxydase study 12 cases.

A case of Herpes Simplex Virus Encephalitis (HSVE) with clinical Korsakoff's syndrome was studied with conventional and electron microscopy, and with the immunoperoxydase (PAP) technique. Eleven additional cases of HSVE were studied retrospectively with the same technique. All eleven cases were positive for HSVE-antigens. In nine cases where an electron microscopic study was performed, the presence of viral particles was confirmed. The advantages of this technique for both rapid diagnosis on brain biopsy material and for retrospective studies are discussed.

[Isolation syndrome of the language area. Neuropsychologic and pathologic study].

A 60 year-old patient in whom spontaneous language was abolished still had a marked capacity for completing phrases and an irrepressible echolalia. From a formal point of view, linguistic ability remained at a high level. Auditory verbal comprehension was severely affected, but was not completely abolished. There was occipital blindness. Pathologic examination reveated extensive bilateral lesions in the regions supplied by the anterior and posterior cerebral arteries. Bilateral destruction of the supplementary motor areas was noted, but the perisylvian language areas were intact. These disorders constitute a syndrome of isolation of language areas which can be used to study anatomical correlations and particularly the role of the supplementary motor area in linguistic and paralinguistic processes.

Crossed aphasia in a right-handed patient. Postmortem findings.

A right-handed man with a global aphasia and a left hemiparesis was examined after a cerebrovascular accident. Three weeks later, oral language was only mildly impaired but writing disorders were still severe. At autopsy, there was an infarction of the territory supplied by the right middle cerebral artery. There was no lesion in the left hemisphere.

[Cerebral amyloid angiopathy. A clinicopathological study (author's transl)].

A case with repeated cerebral hemorrhages associated with severe cerebral amyloid angiopathy (CAA) is reported. CAA may be considered a rare but important cause of cerebral hemorrhages and of local cerebrovascular lesions in the aged. The morphological and ultrastructural characteristics of CAA are discussed. The etiology is still unknown; probably more than one mechanism may be involved.
