[The genetics of Gilbert syndrome].

Investigation of 19 unrelated patients with Gilbert's syndrome and family studies in 21 first degree relatives of 7 of these patients are in agreement with an autosomal-dominant mode of inheritance with incomplete penetrance. The calculated penetrance in our study is 57%. HLA typing (locus A, B, C) showed a slight, insignificant increase in antigens A11 and BW35. Furthermore, this syndrome did not segregate in parallel with certain haplotypes within families.

[A new antihypertensive imidazoline derivative (St 600, Fluoronidine): I. Haemodynamic investigations at rest and during exercise (author's transl)].

Treatment of 12 hypertensive patients with a new imidazoline derivative, St 600 (1.5 to 12.0 mg daily over at least 7 days) resulted in a significant lowering of the blood pressure, mostly to normal values, parallelled by a reduced heart heart rate. A significant decrease in arterial resistance was observed at rest and during exercise in 6 of the patients, whereas a slight increase or no change was recorded in the remaining 6 patients. The changes in resistance in the pulmonary circulation were analogous to those in the systematic circulation for the 2 groups, respectively. The latter half of the patients displayed a decrease in stroke volume, cardiac index and oxygen consumption. No negative inotropic effect was observed.

[Eosinophilic granuloma (histiocytosis-X) with lung and bone manifestations].

A 26 year-old male presented with a spontaneous pathological fracture of the right femur caused by an osteolytic lesion. A chest X-ray demonstrated the coexistence of interstitial pulmonary fibrosis. Fibrocaseous tuberculosis and widespread malignancy were ruled out by appropriate investigations. The differential diagnosis, as based on the histological examination of the bone tumour, rested between hyperparathyroidism complicated by pulmonary disease and eosinophilic granuloma with lung manifestations. Hyperparathyroidism was ruled out by the absence of clinical and biochemical evidence of a disturbance of calcium metabolism. The diagnosis of histiocytosis-X was established by the histological appearance of the lung biopsy.

[Serum concentration of triiodothyronine in autonomous thyroid gland adenoma].

Serum Concentration of T3 was measured in 14 patients with autonomous thyroid adenoma using a chromatographic method. 9 patients had increased levels of T4, 5 were normal. In both groups serum-T3-concentration was significantly elevated. In patients with normal T4-values hypertriijodothyroninemia seems to be the biochemical base for clinical signs of hyperthyroidism. The pathogenetical importance of elevated T3-levels is discussed for T3-secretion and T4/T3 conversion.

Gilbert's syndrome and HL-A. Preliminary report.

HL-A antigens were determined in 18 unrelated patients with Gilbert's syndrome and 3 families where this condition occurred in 2 generations. The data obtained do not point out an association between HL-A antigens and Gilbert's syndrome.

A dot-immunobinding assay for antimitochondrial antibodies.

A dot-immunobinding assay was established for the detection of antimitochondrial antibodies. Nitrocellulose strips were coated with sonicated rat liver mitochondria and incubated in the presence of human sera. The resulting immune complexes were visualized with an enzyme-linked second antibody. Antimitochondrial antibodies were found in the sera of 96% of patients with primary biliary cirrhosis, 17% of patients with autoimmune hepatitis and 4% of patients with progressive systemic sclerosis. Sera of patients with other liver diseases or with systemic lupus erythematosus, specimens positive for M1 and M3 mitochondrial antibodies and samples from normal controls were all negative. Antinuclear and other cytoplasmic antibodies were not detected in this assay. The dot-immunobinding assay for antimitochondrial antibodies is rapid and sensitive, and obviates the need for expensive equipment.

Antibodies to nuclear lamin proteins in liver disease.

Sera of patients with autoimmune liver disease contained antibodies reactive with nuclear lamins. These antigens were identified in immunoblotting experiments, using isolated nuclei, nuclear matrices, nuclear lamina-pore complexes and purified lamins as antigen source. The lamins were, furthermore, characterized by 2-dimensional gel electrophoresis. Antibodies to nuclear lamins were found in 75 per cent of the active lupoid hepatitis cases, but not in patients with inactive disease. Anti-lamin antibodies were detected in 8 per cent of primary biliary cirrhosis sera. The autoimmune liver disease sera recognized predominantly the nuclear lamins A/C, and less frequently the lamins A/B/C or lamin B.
