A comparative study between mechanical, thermal and oxidative disintegration techniques of waste activated sludge.

The release performances of an organic and mineral activated sludge matrix were studied for a wide range of disintegration treatments like mechanical, thermal, thermal-chemical and oxidative disintegration techniques. The maximal COD release was 35% of total COD after 24 hours contact time at 95 degrees C. A limiting value of 60% COD release was obtained for 500 and 700 bars after 10 passes. Concerning theoxidative disintegration techniques (O3 and H2O2), a limiting value of around 60-65% of TOC release was observed. Therefore, it was hypothesised that thermal and mechanical treatments allow mainly for breaking apart the micro-organisms while the oxidative treatment destroys the sludge flocs and disrupts the micro-organisms. A release effect of the mineral fraction is observed only oxidative disintegration techniques.

Feasibility study of mechanically disintegrated sludge and recycle in the activated-sludge process.

The action mechanisms and performances of a combined system associating activated sludge and mechanical treatment (High Pressure Homogenizer) were evaluated for urban wastewater. Discontinuous experiments showed that the energy applied at the first pass was high enough to modify the sludge particulate fraction (high COD release) but without cell lysis. The applied shear forces led to a progressive cell break up (maximal COD release 90% total COD). Continuous experiments showed less than 20% reduction in sludge production (compared to a control run under the same loading conditions Y(TSS) = 0.35 g TSS x g(-1) COD(removed)) through the application of mechanical treatment (stress frequency = 0.2 d(-1)). Recycling of mechanical treated sludge to the aeration tank induced a slight increase in effluent TSS, but the biological performance seemed to be maintained. Significant improvements in sludge settling characteristics were observed.

[Multiple fibrofolliculomas].

Fibrofolliculoma is a rare tumor of the perifollicular mesenchyma. A 44-year old man noticed a growing number of small whitish firm papules on his neck and the upper thorax over the past two years. Histological examination demonstrated on multiple biopsies typical features of fibrofolliculoma. Further evaluation of the patient has not revealed any sign of neoplasia. The variable clinical manifestations of fibrofolliculomas are discussed.

Excess sludge production and costs due to phosphorus removal.

Based on data collected from 35 French wastewater treatment plants and on published data, excess sludge production and chemical consumption associated with phosphorus removal is estimated for the three following phosphorus removal processes: chemical precipitation, Enhanced Biological Phosphorus Removal and hybrid process. The influence of wastewater characteristics on excess sludge production are assessed. Chemical costs and costs associated with sludge disposal were calculated and results for the three phosphorus removal processes are compared. The global costs for phosphorus removal are then estimated.

Application of COD fractionation by a new combined technique: comparison of various wastewaters and sources of variability.

A new respirometric method for COD fractionation was applied to various wastewaters collected on French treatment plants. Great variations were observed especially in the readily biodegradable fraction (RBCOD) ranging from 1 to 16% of total COD. Variability of the results among the origin can be explained by the reactions occurring in the sewers. In one of the towns studied, the wastewater samples collected directly in the sewer show COD characteristics very different from the sewage which reaches the treatment plant. By analysing the same wastewater after aerobic and anaerobic storage, it was proved that RBCOD respectively decreased or increased significantly. Seasonal fluctuations were also quantified, showing that RBCOD fraction decreased from 75 mg/L to -15 mg/L during the summer whereas the variation of soluble fraction was less important.

Influence of pre-treatments and recycled flows on the COD fractions of a wastewater: a case study.

Physical-chemical and biological fractionation methods were used at different parts of the Evry WWTP in order to assess the influence of effluents recycled from the primary settling tanks and sludge treatment processes on the characteristics of the wastewater. The RBCOD doubled between the WWTP influent and the biological reactor inlet, thus doubling the denitrifying potential of the plant. The primary sludge gravity thickener effluent, subject to a great variability, appeared to contribute up to 38% of the RBCOD contained in the biological reactor influent, whereas only 30% seemed to come from the WWTP inlet. No notable increase of RBCOD was observed at the primary settling tanks' outlet for the longer residence time. The significant variations of RBCOD across the plant were not caused by fluctuations of the physical-chemical COD fractions.

Neonatal mortality of low-birth-weight infants in Bangladesh.

OBJECTIVE: To ascertain the role of low birth weight (LBW) in neonatal mortality in a periurban setting in Bangladesh. METHODS: LBW neonates were recruited prospectively and followed up at one month of age. The cohort of neonates were recruited after delivery in a hospital in Dhaka, Bangladesh, and 776 were successfully followed up either at home or, in the event of early death, in hospital. FINDINGS: The neonatal mortality rate (NMR) for these infants was 133 per 1000 live births (95% confidence interval: 110-159). The corresponding NMRs (and confidence intervals) for early and late neonates were 112 (91-136) and 21 (12-33) per thousand live births, respectively. The NMR for infants born after fewer than 32 weeks of gestation was 769 (563-910); and was 780 (640-885) for infants whose birth weights were under 1500 g. Eighty-four per cent of neonatal deaths occurred in the first seven days; half within 48 hours. Preterm delivery was implicated in three-quarters of neonatal deaths, but was associated with only one-third of LBW neonates. CONCLUSION: Policy-relevant findings were: that LBW approximately doubles the NMR in a periurban setting in Bangladesh; that neonatal mortality tends to occur early; and that preterm delivery is the most important contributor to the NMR. The group of infants most likely to benefit from improvements in low-cost essential care for the newborn accounted for almost 61% of neonatal mortalities in the cohort.

Process using DO and ORP signals for biological nitrification and denitrification: validation of a food-processing industry wastewater treatment plant on boosting with pure oxygen.

The simultaneous removal of carbonaceous and nitrogenous pollution by the activated sludge process is becoming common in industrial and municipal wastewater treatment plants. An oxygenation monitoring process has been developed, which is based on the dynamic analysis of ORP and DO signals and allows the detection of specific characteristic points at the end of the biological nitrification and denitrification. The aim of this study is to validate this process in a food-processing industry WWTP (slaughterhouse) having large variations of carbonaceous and nitrogenous loads. In order to treat during the peak period, pure oxygen is used. The first part of the study provides a precise diagnosis of the WWTP operation by the analysis of the ORP and DO signals. It is particularly easy to estimate the level of nitrogen treatment actually achieved and the oxygen requirements, and to detect the over- or under-oxygenated phases. Thanks to the monitoring process, the aerobic period of each cycle is reduced to the optimal duration, providing a reduction of 30% on the energy consumption compared to a traditional schedule. We have demonstrated that the use of pure oxygen associated with the existing air system is particularly relevant for the peak period. The revamping of an existing plant to simultaneously treat the carbon and the ammonia in the same basin is now technically feasible.

Is provisional stenting the effective option? The WIDEST study (Wiktor stent in de novo stenosis). Widest Trial Investigators' Group.

AIM: To compare the immediate and late outcomes of patients treated by a policy of routine stent implantation with routine balloon angioplasty and the use of stents only when an ideal result has not been obtained. METHODS: A nine centre, multinational, randomised study of 300 patients with coronary artery disease thought suitable for treatment of a single lesion by balloon angioplasty or stent implantation. Only new lesions in patients who had not undergone previous bypass surgery were included, and totally occluded vessels were excluded. RESULTS: The initial procedure was considered successful in 96% of patients. There was more complete angiographic restoration of luminal diameter in patients treated by elective stent (minimum lumen diameter (MLD) 2.68 mm for stent v 2.27 mm for balloon; p < 0.007), but analysis of the subgroup of balloon angioplasty patients who crossed over to stenting showed that they achieved similar results to the elective stent group. Late luminal loss was greater in stented patients than in those undergoing balloon angioplasty only, and by six months the angiographic benefit of stenting had disappeared (MLD 1.90 mm for stent group v 2.00 mm for balloon angioplasty). Angiographic and clinical results in the balloon angioplasty group were assisted by the high crossover rate (30.1%). Both groups had similar symptom relief, with 58.9% of patients improving by two or more angina grades. The need for further revascularisation was also similar in the two groups at one year (18.2% in the stented group v 17.1% in the balloon angioplasty group). Haemorrhagic complications at the local arterial entry site were more common than expected and were distributed equally between the patients receiving full anticoagulation and those receiving antiplatelet treatment only. The results of both Wiktor stent placement and balloon angioplasty were similar to the findings in the stent group in previous randomised studies (Benestent II, STRESS). CONCLUSIONS: Provisional stenting appears to offer the same longer term outcome as elective stenting in this selected group of patients. Improvement in the results of conventional balloon angioplasty in the past 10 years means that a policy of obtaining an ideal result without the use of stents appears to be practicable in many of these patients, with consequent cost savings.

The shifting patterns of metastatic melanoma.

Management of cutaneous melanoma becomes more difficult as tumor cells metastasize to sites that are remote from the primary tumor. There is a hierarchy of risk associated with cancer spread by different routes and to different organs. Differences in clinical outcome indicate critical variations in pathobiology at different stages in disease evolution. Prevention of the evolution of melanoma is critical, especially beyond the stage of nodal involvement.

Heterologously expressed polypeptide from the yeast meiotic gene HOP1 binds preferentially to yeast DNA.

HOP1 protein, present in sporulating cells of Saccharomyces cerevisiae and believed to be a component of the synaptonemal complex, has been expressed in Escherichia coli fused to a biotinylated tag protein. Once solubilized from bacterial inclusion bodies, the HOP1 fusion protein was purified by using a combination of avidin-affinity chromatography and gel filtration FPLC and refolded. Sequence comparisons indicate that the HOP1 gene product contains a zinc finger motif, which may confer DNA binding properties, and the recombinant polypeptide was used to assess the putative DNA binding properties of the product of native HOP1 protein using a gel-shift assay. Protein and protein-DNA complexes were detected by exploiting the affinity of streptavidin-alkaline phosphatase for the biotinylated tag protein after Western blotting. The HOP1 fusion protein bound unambiguously to digested genomic yeast DNA. This binding possessed some degree of specificity, was maintained under a wide range of salt concentrations, and was unaffected by the presence of high concentrations of competitor DNA (synthetic poly[dI-dC].poly[dI-dC]). In contrast, no shift was detected when the fusion protein was incubated with digested genomic DNA from Arabidopsis, or with lambda/HindIII DNA. Incubation with digested genomic DNA from Lilium produced a small change in the mobility of the protein. The biotinylated tag protein failed to show any DNA binding activity. Scatchard analysis indicated an apparent yeast genomic DNA:HOP1 fusion protein dissociation constant of K(d) = 5 x 10(-7) M.

Evaluation of pulsed dose oxygen delivery during exercise in patients with severe chronic obstructive pulmonary disease.

BACKGROUND: Oxygen conserving devices may lead to substantial increases in the duration of oxygen provided. A study was undertaken to compare the performance of a pulsed dose oxygen delivery (PDOD) system with continuous flow oxygen or air during a maximal walking test. METHODS: Fourteen patients with chronic obstructive pulmonary disease (COPD) and arterial oxygen desaturation on exercise (mean (SD) forced expiratory volume in one second (FEV1) 0.83 (0.28) 1, arterial oxygen pressure (PaO2) 8.38 (1.24) kPa, arterial carbon dioxide pressure (PaCO2) 5.95 (0.86) kPa) were randomised to perform a walking test using air administered via a cylinder or continuous flow oxygen at 2 l/min or by a PDOD system. RESULTS: There was no significant difference in the mean arterial oxygen saturation (SaO2) using the PDOD system or with continuous flow oxygen (p = 0.33). Patients showed greatest desaturation whilst walking with the air cylinder (SaO2 79.2 (8.59)%) which was significantly different from the desaturation with both continuous flow oxygen (87.6 (5.85)%, p = 0.001) and PDOD (85.6 (7.36)%, p = 0.004). There was no significant difference between the distance walked using oxygen delivered at 2 l/min by continuous flow or via the PDOD (p = 0.72; CI 0.34 to 1.08). The mean (SD) distance walked on continuous flow oxygen (203.6 (106.1) m) and PDOD (207.9 (109.8) m) was significantly greater than the distance walked with the air cylinder (188.6 (110.02) m); (1.12 fold increase in distance, CI 1.01 to 1.23, p = 0.02, and 1.14 fold increase in distance, CI 1.01 to 1.28, p = 0.03, respectively). CONCLUSIONS: These findings suggest that the pulsed dose oxygen conserving device was as effective as continuous flow oxygen in maintaining arterial oxygen saturation and that the use of this device was associated with similar improvements in exercise tolerance to patients taking continuous flow oxygen therapy.

Hyperthyroidism: a novel feature of the tubulointerstitial nephritis and uveitis syndrome.

Tubulointerstitial nephritis and uveitis syndrome presents with either renal or ocular manifestations, and associated weight loss. We report two adolescents with transient hyperthyroidism early in the course of the syndrome. Hyperthyroidism may represent an unrecognized feature of the disorder and potentially contributes to the accompanying weight loss. Thyroid function should be evaluated in patients with tubulointerstitial nephritis and uveitis syndrome and symptomatic therapy provided when necessary.
