[Cefaclor in dermatological therapy (author's transl)].

Thirty patients suffering from bacteriogenic skin diseases were given cefaclor, in most cases simultaneously with classical external dermatological therapy. The results of therapy were satisfactory in 29 of the 30 patients. Only in one relatively severe case of erysipelas did the disease continue to spread despite the daily doses of 3 x 500 mg of oral cefaclor prescribed by the dosage regimen. Side-effects observed consisted of diarrhoea in one patient, and a transient slight increase of SGOT or alkaline phosphatase in another six patients.

[Development of malignant melanomas on preexisting pigmentary lesions. Control of anamnestic data based on patients private photographic series].

We tried to confirm the anamnestical data of patients with malignant melanoma with the aid of private photos. The statement of the patients that a "birth-mark" was present before the development of the malignant melanoma could nearly always be corroborated by the photographs. The patients' statements that the tumor had developed in uninvolved epidermis in a relatively short time proved to be correct in many cases. In some cases, however, the "birth-marks" could be detected on the photographs at a time, when these marks were not yet recognized by the patients. According to the statement of the patients and the results of the pictures, lentigo maligna-melanoma, superficial spreading melanoma, and nodular melanoma can originate from a pre-existing pigmented spot or can develop in a relatively short time in uninvolved epidermis. In most cases of lentigo maligna-melanoma a long history and a slow tumor growth is stated. Generally the development of nodular melanoma is extremely short. In cases of superficial spreading melanoma the time of tumor growth was reported to be a few month or up to one or two years. In some cases, however, a much slower development of the tumor could be detected with the aid of the photographs.

[Lymphangiosis melanoblastomatosa].

After endolymphatic therapy and following surgical excision of lymphnodes in a patient with malignant melanoma the involved leg became edematous. In the edematous tissue there was a wide and diffuse spreading of the melanoma cells. Besides numerous cutaneous and subcutaneous metastatic nodules there were areas of blue-black coloring which showed a fine and reticular pattern. Histology confirmed that melanoma cells had filled up predominantly the superficial lymphatic vessels. Fluorescence histochemical investigations showed that these cells were viable and pigment-producing melanoma cells. There were also single enlarged melanocytes in the basal part of the epidermis with a high amount of fluorophores.

Melanin-producing dendritic cells and histogenesis of malignant melanoma.

In a total of 70 malignant melanomas we searched for dendritic-branched fluorescent pigment cells. Hereby we found that dendritic-branched tumor cells are especially characteristic in cases of lentigo maligna. In the flat parts of these lesions, these cells are the predominant cell type. Dendrites in the pseudonests or nodular parts of lentigo maligna can only seldom be detected. The prevailing cell type in superficial spreading melanoma and in primary nodular melanoma is the round or oval unbranched tumor cell. In some cases of nodular melanoma, cells with short dendrites could be seen. In superficial spreading melanoma, dendritic tumor cells could be observed particularly in such tumor parts, in which the malignant cells were scattered between the keratinocytes. Melanocytes can evidently produce dendrites between cells of the sebaceous gland. In the marginal parts or in parts of regression of some superficial spreading melanomas, a great area of dendritic tumor cells could also be detected in the basal parts of the epidermis. Altogether, however, in superficial spreading melanoma and in nodular melanoma they occur only rarely. Dendritic-branched cells are also visible in lymph-node metastases of SSM and NM. The fact that the dendritic tumor cells can be observed in all 3 types of tumors (according to Clark and coworkers) gives a rise to a new discussion of the dualistic theory of melanoma-histogenesis of Mishima. Although this theory could not be disproved, up to now on the basis of the present results, an unitarian development of all types of mnelanoma from melanocytes seems to be possible.

Fluorescence-microscopic investigations of pigment cells of lentigo maligna (melanosis circumscripta praeblastomatosa Dubreuilh) and lentigo maligna melanoma.

With fluorescence-histochemical methods (formalin-induced fluorescence), the different stadia of development of lentigo maligna (Morbus Dubreuih) and lentigo maligna melanoma were investigated. In this way, the special stadia can be clearly characterized using the fluorescence-microscope. In the earliest stadium of malignancy, only the pigment-cells of the basal part of the epidermis seem to be numerous. At this time, these cells are mainly arranged as palisades in the basal layer, and their strongest dendrites are usually directed towards the corneal layer. As the malignancy progresses, the pigment-cells are arranged in several layers in the basal epidermis. In this case, polymorphism of the cells is obvious and their dendrites spread in all directions. A further stadium shows pathological alterations resembling a lentigo with long rete ridges. The atypical cells cluster together into the so-called pseudonests, predominantly at the tips of these rete ridges. With the fluorescence-microscope, dendrites are seldom visible here. In the stadium of tumorous growth, all above-described alterations of the epidermis in lentigo maligna are no longer detectable. Malignant cells in the dermis, which form as a tumor mode, seldom show dendrites. These cells are mainly round or oval-shaped; at best there are some spindle cells in certain areas of the tumor. The epidermis covering the tumor node is infiltrated by some tumor cells but the characteristic alterations as described above for the various stadia of lentigo maligna are no longer visible. Even fluorescence-microscopically, a tumor node of lentigo maligna does not seem to be different from a primary nodular melanoma or a tumor node of a superficial spreading melanoma, if the flat parts of the tumors are not considered in the diagnosis.

[The nonspecific epifocal Malek-Mansour immunotherapy of malignant cutaneous melanoma using DNCB].

This is a report on first experiences with epifocal DNCB therapy of Malek-Mansour in metastases and primary tumors of melanoma. Clinical, histological, and fluorescencemicroscopical documentation of six cases with melanoma metastases and twelve cases with primary melanomas, treated epifocally with DNCB (solution or ointment) are given. Metastases of melanoma react variably to DNCB, whereas primary tumors especially flat forms the SSM with or without invasion, usually disappeare totally after four to twelve DNCB applications. The results of Malek-Mansour et al. could be confirmed. During the DNCB treatment, which does not require a preceding sensibilization, initial caustic effects, chemosurgical effects, and a longterm "immuno-chirurgical" effect must be distinguished. An apparent immunological effect of treatment is shown by the therapeutical reaction of non-treated metastases. A sensitization to DNCB occurred also in all cases of older patients. Recurrences of melanoma, which could be due to the new method of therapie, did not occur within 18 months.

[Malignant lentigo on glans penis. Differentiation using a new method].

A lentigo maligna melanoma on the glans penis and the orificium urethrae is described. From the clinical point of view a superficial spreading melanoma as well as a lentigo maligna could be considered as differential diagnoses. The flat part of this lesion, however, was a lentigo maligna when investigated using a lightmicroscope and especially fluorescence-microscope. In the lightmicroscope only single heavily pigmented and dendritically branched cells could be seen in the basal parts of the epidermis. The definite shape of the other tumor cells was not identifiable. With the fluorescence microscope, however, in nearly all fluorescing and atypical pigment cells, dendritic branching clearly was present.

[Clinical and experimental observations on idiopathic urticaria due to the contact with heat].

This study describes the probably eleventh case, mentioned in literature, of acquired heat contact uticaria in an otherwise healthy young woman. With regard to true contact induction heat contact urticaria clearly differs from the more common cholinergic uticaria. On the other hand, heat contact urticaria is completely analogous to cold uticaria because of the exposure area, reversible blocking by unphysiological prolonged heating of the skin, sensitivity to antihistamines and resistance to corticosteroids. In this case, whealing of the skin occurred on exposure to heating of 39 degrees C for 5 min. With a temperature of 44-46 degrees C, The shortest time for wheal induction was 3-5 sec. At 70 degrees C, the shortest time for maximal reaction was only a split second. An "optimal temperature" for wheal induction could not be determined. Local anaesthesia with 2% Xylocain caused a considerable blocking of wheals. Histamine and cholinergic drugs showed normal skin reactions after intradermal injection. Antihistamines administered parenterally or perorally were highly effective. Corticosteroids, however, given systemically in high doses proved to be ineffective. During our observations, a spontaneous remission appeared with a clinical symptom-free state; on unphysiological high temperature stimulus, however, contact uticaria could still be demonstrated. The pathogenetic uniformity of sporadic heat contact urticaria and problems of therapeutical controls are discussed.

Histochemical findings in different types of malignant melanoma: biological and clinical significance.

By means of histochemical methods (gel-film incubation-media) superficial spreading melanoma, nodular melanoma and lentigo maligna melanoma are investigated. The result of this examination is that with regard to their enzyme spectra, the nodular melanoma and the nodular part of the superficial spreading melanoma are very similar. Glucose-6-phosphate dehydrogenase shows the strongest enzyme reaction, followed by succinate dehydrogenase and lactate dehydrogenase. The beta-hydroxybutyrate dehydrogenase reaction is always weak. The reaction of acid phosphatase is between negative and weakly positive. Significant differences, however, are observed in lentigo maligna and in lentigo maligna melanoma. In both, the strongest formazan deposits are seen with succinate dehydrogenase, sometimes also with lactate dehydrogenase. The glucose-6-phosphate dehydrogenase reaction, however, is sometimes considerably weaker. In the case of lentigo maligna melanoma, the activity of beta-hydroxybutyrate dehydrogenase often is increased, and acid phosphatase also shows higher reactions than in the other melanomas. These differences in the enzyme pattern correspond to the different biological behavior of the tumours. The enzymatical and biological characteristics of lentigo maligna melanoma possibly derive more from the characteristics of the tumour itself which are not dependent on the area.

[New method for the more precise differentiation of melanomas. Significance of formalin-induced fluorescence for the prognostic evaluation of malignant melanomas].

The value of formalin-induced fluorescence method in diagnosis of malignant melanoma. With this method in the case of malignant melanoma the melanin producing cells can be clearly demonstrated. Shape and arrangement of the tumor cells in the epidermis are to be judged accurately. It is possible to distinguish special cases of lentigo maligna melanoma from superficial spreading melanoma. In the first kind of tumor the cells mostly have long dendrites like melanocytes, whereas in superficial spreading melanoma we predominantly find round cells which only seldom show dendrites. Furthermore the fluorescence method is of great value in exact determination of the level of invasion. Only in this way it is possible to detect single tumor cells in connective tissue, in vessels and in fat tissue. Also in examination of lymph nodes even single cells or small clusters of melanoma cells can be detected.

[Fluorescence histochemical and microfluorometrical investigations of pigmentary tumors of the skin (author's transl)].

Junction nevus, dermal nevus, melanosis circumscripta praecancerosa Dubreuilh, superficial spreading melanoma, and nodular melanoma were investigated and characterized by use of the formalin induced fluorescence method (FIF). In the vicinity of junctional nevus cell clusters and near tumor cells of the superficial spreading melanoma increased numbers of melanocytes are found. These show different types of dendritic branching. Spherical nevus cells however are completely devoid of dendritic processes. On the other hand, the atypical pigment cells in melanosis circumscripta praecancerosa Dubreuilh exhibit a shape similar to that of melanocytes, whereas the globular cells of superficial spreading melanoma have the appearance of nevus cells. The arrangement of nodular melanoma cells resembles that observed in dermal nevus. However the characteristic decrease in fluorescence intensity from epidermal junction to deeper dermis as observed in the dermal nevus was missed in nodular melanomas. Dendritic pigment cells displaying formalin induced fluorescence (FIF) could be demonstrated in all types of malignant melanomas investigated in the present study. The fluorophores of the pigment lesions are characterized microspectrofluorimetrically by (1) ill-defined emission maxima between 470 and 490 nm and (2) a clear-cut excitation maximum at 430 nm accompanied by a lower one at 320 nm. Hydrochloric acid vapor induces a hyposochromic shift of the 430 nm excitation maximum to 370-380 nm and a marked elevation of the 320 nm maximum. These results indicate fluorophores of DOPA and its derivatives; in this respect there are no marked differences between melanocytes, nevus cells and the cells of malignant melanoma.

Increase of melanocytes around malignant melanoma.

In split epidermal sheets with clinically normal appearance a quantitative study was carried out on dopa-positive cells in the vicinity of malignant melanomas. These data were then compared with the number of melanocytes found in the skin of the contralateral body side of the same patient. In the epidermis around superficial spreading melanoma and lentigo maligna melanoma, the number of dopa-positive cells was usually significantly higher than in the contralateral body side. On the other hand, no difference was generally found around nodular melanoma.

[Vitality evaluation of malignant melanoma metastases following endolymphatic radionuclide therapy using the Falck-Hillarp method].

A case of malignant melanoma involving the regional lymphnodes was treated by endolymphatic isotope therapy. Because of the great diameter the radiation of some metastases was not completely sufficient. Fluorescent and living cells of malignant melanoma between damaged cells could be seen in the center of these metastases. Melanophages surrounded concentrically the central part of the metastases enclosing the melanoma cells. It is shown that in the search and identification for pigment producing melanoma cells the fluorescent histochemical method of Falck-Hillarp is superior to usual light microscopical methods. Beyond of this, the phenomenon of fluorescence in malignant melanoma cells probably is an indication for the vitality of these cells.
