Recombinant-growth hormone (rec-hGH) therapy in infertile men with idiopathic oligozoospermia.

The study was designed to be an open test for short-term efficacy of rec hGH-therapy on spermatogenesis of ten infertile men with idiopathic severe oligozoospermia (1-5 x 10(6)) and normogonadotropinemia or moderate ipergonadotropinemia (FSH: range 1.90-9.60 IU/L, mean +/- SD 5.78 +/- 2.84; LH: 2.70-12.70 IU/L, mean +/- SD 6.60 +/- 3.61). FSH, LH, Testosterone, GH, and IGF-I were evaluated. Seminal parameters: sperm concentration, sperm motility and morphology were studied before and after therapy. Five responder patients showed an increase of seminal parameters both in sperm concentration and total number of motile spermatozoa. The short-term rec-hGH therapy significantly increased sperm concentration and total motile spermatozoa in five out of ten infertile patients who didn't show any modification in seminal parameters to other preavious treatment.

[Prognostic factors in operated breast carcinoma].

The traditional prognostic factors (tumour size, lymph node involvement, receptor status) have now been shown to have limits in terms of prognostic definition. These limits may be partially overcome when parameters exist to determine the extent of tumour biological aggression and the patient's degree of immune response. It is important to clarify these links because they may lead to more precise indications regarding the prognosis and choice of therapy, above all in cases of breast cancer operated with no lymph node involvement. Breast carcinoma is often surrounded by inflammatory cells showing host and tumour interaction. The aim of this study was to evaluate the prognostic value of lymphocyte infiltration in operated breast cancer. The degree of lymphocyte infiltration observed in 56 breast cancer patients was compared with other prognostic factors (tumour size, lymph node status, histological variants, necrotic areas and desmoplastic reaction). This preliminary study allowed the authors to examine the degree of peritumoral lymphocyte density as an important predictive index of overall survival in patients with breast cancer and N-.

Evaluation of the morphological and functional damage to human sperm subjected to freezing at -196 degrees C and to refrigeration at +4 degrees C.

A study was carried out on five healthy, fertile donors to evaluate refrigeration at +4 C compared to cryopreservation at -196 degrees C. These donors had produced more than two pregnancies in different women with their cryopreserved semen in an AID program. The following parameters for evaluation and comparison were used: (i) the percentage of forward sperm motility, (ii) the percentage of swollen sperm after hypoosmotic stress (swelling test) and (iii) the sperm morphology observed both with a light microscope after staining and with an electron microscope. After 48 hours of refrigeration the result obtained were comparable with those observed after one week of cryopreservation. After 72 hours of refrigeration, a sharp and significant decrease of these values was noted. Our data underlined the fact that there is an individual variability in subject response to the method of preservation employed. Our findings show the possibility of using sperm refrigerated for up to 48 hours in AIH programs.

[A case of non-Hodgkin T-cell lymphoma with predominantly cutaneous manifestations].

There is a wealth of experiences concerning cancer and leukemia induced in human populations by radiation. The contribution of the nuclear industry to the radiation exposure of the general population is small, but there is the risk of catastrophic accidents causing a large number of deaths. The authors describe the case of a 48 year old black man accidentally exposed to the effects of radiations during the nuclear disaster of Chernobyl in 1986. The patient showed, many years later, a predominantly cutaneous high-grade T cell lymphoma, which was refractory to traditional treatments but sensitive to high doses of a recombinant interferon. Unluckily the patient died, six months later, because of autoinfection overwhelming. The goal of the authors is again to ask to reflect on the risk of the use of nuclear power and to debate the possible choice of the still experimental treatments.
