[Systematic prevention of deep vein thrombosis with subcutaneous heparin therapy following cesarean section. Advantages and disadvantages].

The study was carried out on 234 women who had undergone caesarean sections. Hematoma in the area of the incision represents a major obstacle to the prevention of deep vein thrombosis through systematic subcutaneous heparin therapy. Even though anticoagulant therapy is not 100% effective, complications in this particular treatment appear to be minor. In our opinion, this therapy should be reserved for certain favorable cases and should not be generally used for all women undergoing caesarean sections.

Folate and iron deficiencies in mothers and their newborn children.

Iron and folate status was assessed in 79 multiparous French women and 21 immigrant parturient women, previously defined as belonging to a high risk population for nutritional deficiencies, and their newborn infants. Maternal hemoglobin was correlated with ferritinemia and with serum folate values; weight gain during pregnancy appeared to be correlated with mother's iron status at start of labour and gestation duration with folate status. The need of iron and folic supplementation is emphasized for such groups. While a correlation appeared between maternal and newborn values for serum and red blood cell folates and for the RBC indices, no statistical difference was found between the infants born to iron or folate deficient mothers when compared with others.

The very low birth weight problem, an imperative necessity for prevention.

The very low birth weight births are a major problem in the perinatal field, a short review is presented on the increasing rates of admission of these new borns in neonatal intensive care units and on the results of mortality and long-term morbidity published by pediatricians. The obstetricians have to apply the established preventive methods. This paper presents the results of a single hospital at Clamart, France, showing the progressive reduction of the number of births with less than 1500 g birth weight (still-births and live-births), when a preventive policy is proposed since eight years.

Enzymes for epinephrine synthesis and metabolism in the myometrium, endometrium, red blood cells, and plasma of pregnant human subjects.

The enzyme phenylethanolamine-N-methyl transferase (PNMT) converts norepinephrine (NE) to epinephrine (E), and cathechol-O-methyl transferase (COMT) and monoamine oxidase (MAO) both metabolize NE and E. We were able to measure the activity of these enzymes in myometrial and endometrial samples obtained from 27 pregnant women between 32 and 40 weeks' gestation at the time of cesarean section. In addition, these enzymes were measured in red blood cells and plasma obtained from 45 normal pregnancies between 10 and 40 weeks' gestation. PNMT activity was significantly higher in myometrium than in endometrium. During labor myometrial and red blood cell PNMT activity is significantly lower than in specimens from patients at term not in labor, while myometrial PNMT activity is significantly elevated in patients with dystocia. Under these conditions no significant alterations in myometrial, red blood cell, or plasma COMT or myometrial MAO activities were observed. This study identifies for the first time the presence of PNMT in myometrium, endometrium, and red blood cells and provides the necessary step for the conversion of NE to E, a concept previously limited to the adrenal bland, brain, and heart. This could provide an important physiologic mechanism for the control of uterine activity during pregnancy and parturition.

[Spontaneous abortion after intra-uterine transfer of an ovum fecundated in vitro (author's transl)].

Spontaneous expulsion of a human ovum fecundated in vitro and transferred into the uterus occurred after 10 1/2 weeks of amenorrhoea. The oocyte had been collected after stimulation by human menopausal and chorionic gonadotrophins (hMG and hCG). The techniques of pre-ovulatory oocyte collection, fecundation, in vitro culture and of reimplantation of the embryo are briefly described.

Spontaneous or systematically induced labor for the termination of twin pregnancies.

The best time for twin delivery is at 38 or 39 weeks gestation. Systematic Caesarean section or induction is decided upon during these weeks. Thanks to this policy, the good results formerly obtained have been maintained and an important decrease in emergency. Caesarean sections has been noted thus reducing the risks inherent in this method. However, a randomized trial is necessary to confirm these results.

Influence of routine administration of folic acid and iron during pregnancy.

Haematological and folic acid status were assessed in 200 women in the 6th month of pregnancy. Folic acid deficiencies with no or little haematological impairment were found in one third of the cases, and their occurrence increased when the socioeconomic level was low. During the last trimester of pregnancy, the women were given either iron alone or iron and folic acid supplementation. In the mothers, the rise of folate values in serum and red blood cells, in the folic acid-supplemented group, had no obvious haematological consequences, showing that iron therapy alone can, in developed countries, prevent the anaemia in pregnancy. In the infants, there was no difference in the haematological indices, whatever the mothers' treatment had been. However, a significant difference appeared for the gestational age and, therefore, the height and weight. Folic acid supplementation during pregnancy increased its duration by virtually 1 week.

Inhibin-like activity and steroid content in human follicular fluid during the periovulatory period.

Inhibin-like activity as well as estradiol-17 beta and progesterone content were assayed in human follicular fluid obtained at different stages of the periovulatory period. Between the start of the midcycle estrogen surge in plasma and the subsequent gonadotrophin peak, follicular fluid was found to contain similar levels of estradiol-17 beta and progesterone (8.2 pmol/microliter) and to induce a marked (44%) inhibition of gonadotrophin-releasing hormone-stimulated release of FSH. The midcycle gonadotrophin peak was characterized by a drop in estradiol-17 beta levels and a tripling in progesterone levels, whereas inhibin-like activity fell to almost half the previous value.

[Lysozyme content of amniotic fluid (A.F.) (author's transl)].

Lysozyme content of 47 A.F. collected at different stages of pregnancy has been measured. At the same time creatinine concentrations of 33 of those A.F. have been measured. Lysozyme content increases with the progress of the pregnancy. A similar increase is apparent when lysozyme concentration is correlated with A.F. creatinine.
