Beginnings of the New York State Mental Hygiene Medical Review Board 1976-1980.

A review is provided of the organization and functions of the New York State Mental Hygiene Medical Review Board and the first 4 years of its investigative activities into sudden, unexpected, mysterious, or unnatural deaths among mental patients. In 4 years 384 such deaths have been investigated, among which natural causes, suicide, and accidents are the most common. Certain medical procedures, laws, and administrative methods have been changed due to findings of the Board.

Differential expression of gizzard actin genes during chick embryogenesis.

The iso forms of gizzard actin present during chicken embryogenesis were analyzed by two-dimensional electrophoresis. During chick embryogenesis there are conspicuous changes in the content of the iso forms of actin. Gizzards from 8-day-old embryos contain almost exclusively beta-actin. After 8 days of embryonic age, there is a continuous increase in the amount of gamma-actin and an apparent decrease in the amount of beta-actin. These changes in the content of beta- and gamma-actin in gizzard tissue are paralleled by changes in the content of the corresponding mRNAs, as detected by the ability of the RNAs to direct the synthesis of these proteins in micrococcal nuclease-digested reticulocyte lysates. The correlation of the in vivo and in vitro experiments indicates that during chick embryogenesis there is a differential expression of the genes coding for the iso forms of gizzard actin and that this expression is controlled at the transcriptional level.

Möbius syndrome. Neuropathologic observations.

Neuropathologic findings in an infant with congenital right facial and bilateral lateral rectus palsy (Möbius syndrome) are presented. Multiple microscopic foci of necrosis were found in the lower pontine tegmentum, involving the region of the intramedullary course of 6th and 7th cranial nerves. The significance of pathologic findings in this infant and 14 additional published cases of Möbius syndrome is discussed.

Reproductive management of mares without detection of oestrus.

The use of photoperiod, progestagen, prostaglandin and hCG treatments was investigated to obtain mating of mares at predetermined times. The objectives were: (1) synchronization of oestrus at an early time of the year, (2) simplification of treatment schedules by use of vaginal sponges, and (3) use of several controlled cycles by successive synchronization. The following conclusions were reached. First, after a 16 h photoperiod was applied beginning on 25 November, hormonal synchronization of oestrus and ovulation followed by cyclicity were obtained on 1 February; i.e. 2 months of light are essential as hormonal synchronization of ovulation was not obtained by 10 January. Second onset of oestrus was well synchronized after vaginal application of progestagens (3.75 days +/- 0.98 s.d. after withdrawal) and in spite of vaginal irritation, fertility was high (71%, N = 24) after mating every 48 h of the induced oestrus. Third, for synchronization of return to oestrus in mated non-conceiving mares, oral progestagens were given from Days 7 to 21 after mating. Predetermined mating (Days 27 and 29) and hCG injection (day 28) for non-pregnant animals were decided after a progesterone assay of Day 21 blood plasma. After 3 controlled mating periods, the cumulative fertility was 88% (N = 24) in non-lactating mares and 58% (N = 19) in lactating mares. Programmed reproductive management is possible in the horse.

Oestrogen pattern during early pregnancy in the mare.

Plasma total (conjugated + unconjugated) oestrogens were measured from Day 0 to 100 of pregnancy and compared with the levels found during the oestrous cycle. From Day 0 to 35 of gestation, the concentrations were similar to those during dioestrus. An increase in total oestrogens between Days 35 and 40 was followed by a plateau of 3 ng/ml between Days 40 and 60 which was slightly higher than preovulatory concentrations. This first increase in total oestrogen level was produced by the ovaries since values were suppressed after ovariectomy; stimulation may be due indirectly to PMSG causing follicular growth. After Day 60, a second increase was detected and considered to be of feto-placental origin as the levels at this time were not suppressed after ovariectomy. By Day 85, oestrogen concentrations exceeded thos detected in non-pregnant mares so that a direct measurement of total oestrogens in plasma by a simplified radioimmunoassay after Day 85 of gestation offers a reliable method of pregnancy diagnosis.

Mistranslation in a eucaryotic organism.

Previous work from our laboratory has demonstrated that a subclass of the aminoglycoside antibiotics, those containing the drug fragment paromamine, stimulates mistranslation in cell-free protein-synthesizing systems derived from eucaryotic cells. We report here experiments which show that the ciliate Tetrahymena thermophila (formerly T. pyriformis, syngen 1) is sensitive to the paromamine-containing aminoglycoside antibiotics. The drugs are active with respect to growth inhibition, inhibition of protein synthesis in the whole organism, inhibition of protein synthesis in vitro and the stimulation of mistranslation in cell-free protein-synthesizing systems. Because of their misreading properties, these drugs may be useful in isolating and propagating strains carrying mutations which can be translationally suppressed (that is, missense and nonsense mutations).

Contusion angle deformity in prizefighters.

Ophthalmic examinations of 55 retired boxers showed nine eyes with contusion angle deformities and no cases of secondary glaucoma due to the trauma, despite an average duration of 33 years since injury. The subjects with chamber angle recessions had more proptotic eyes (average, 19.7 mm) than those without recessions (average, 17.9 mm). The difference between the means was not significant (P less than .05).

Urinary oestrogen and plasma progesterone levels in non-pregnant mares.

Eight mares were observed clinically for 14 months to detect oestrus and ovarian activity, and assays of urinary oestrogens and plasma progesterone were performed. Cyclical ovarian activity occurred during 60% of the experimental period. Persistent luteal activity was found over 20% of the period and for the rest of the time activity was absent. Cyclical activity was characterized by the alternation of periods of low and high progesterone levels corresponding to the follicular and luteal phases of the ovarian cycle. During the follicular phase, urinary oestrogen levels increased to a maximum of 463 +/- 296 ng/ml 2 days before ovulation. The duration of the interval from complete luteolysis to ovulation (6-3 +/- 2-7 days) was correlated with oestrone peak values (r = 0-44) and with oestrone levels at the beginning of the low progesterone phase (r = -0-36). High oestrogen levels were recorded during persistent luteal activity which occurred more frequently in summer. Ovarian inactivity was more frequent in winter and was characterized by very low progesterone and low oestrogen (less than 50 ng/ml urine) levels. Some individual and seasonal effects were studied.

Synchronization of oestrus in mares with a prostaglandin analogue and HCG.

Following an initial observation period of 13 days when plasma progesterone levels were determined, synchronization of oestrus and ovulation was investigated in thirty-three mares treated with a prostaglandin analogue ('Equimate') and HCG. Two courses of treatment were given 8 days apart and each consisted of a single intramuscular injection of Equimate (250 mug) followed 6 days later by an intramuscular injection of HCG (2500 i.u.). The first course was designed to remove all luteal tissue and to induce ovulation of any follicles that developed in response to the withdrawal of progesterone. The second course had the effect of synchronizing oestrus and ovulation following luteolysis of the induced CL in all the mares. A high proportion of treated mares responded by ovulation within 24 and 72 hr after the second HCG injection and provided fertility was not disturbed, the prognosis for a high success rate by the double treatment was good. The plasma progesterone level is a reliable criterion of ovulation and would replace the need for the clinical procedures normally employed to determine the onset of oestrus and follicle rupture.
