Virtual reality computer simulation.

BACKGROUND: Objective assessment of psychomotor skills should be an essential component of a modern surgical training program. There are computer systems that can be used for this purpose, but their wide application is not yet generally accepted. The aim of this study was to validate the role of virtual reality computer simulation as a method for evaluating surgical laparoscopic skills. METHODS: The study included 14 surgical residents. On day 1, they performed two runs of all six tasks on the Minimally Invasive Surgical Trainer, Virtual Reality (MIST VR). On day 2, they performed a laparoscopic cholecystectomy on living pigs; afterward, they were tested again on the MIST VR. A group of experienced surgeons evaluated the trainees' performance on the animal operation, giving scores for total performance error and economy of motion. During the tasks on the MIST VR, errors and noneconomy of movements for the left and right hand were also recorded. RESULTS: There were significant correlations between error scores in vivo and three of the six in vitro tasks (p < 0.05). In vivo economy scores correlated significantly with non-economy right-hand scores for five of the six tasks and with non-economy left-hand scores for one of the six tasks (p < 0.05). CONCLUSION: In this study, laparoscopic performance in the animal model correlated significantly with performance on the computer simulator. Thus, the computer model seems to be a promising objective method for the assessment of laparoscopic psychomotor skills.

[Immediate postoperative course and late results after inguinal herniotomy in children].

Three hundred consecutive children less than 18 years of age were treated for inguinal hernia by the same surgeon from 1970 to 1985. The surgical procedure was the same in all cases, namely ligation of the tunica vaginalis and extirpation of the hernial sac. Hernioplasty was not performed. Twenty patients were excluded. Seven patients had a recurrent hernia, i.e. a cumulative recurrence rate of 2.4 per cent after a median follow up of 12 years (range 5-19). Five of seven (71%) recurred within five years after operation, while only two recurred from the fifth to the 15th year. We conclude that recurrence five years after operation is seldom and the recurrence rate in our work is acceptable compared with other publications.

Recurrences after inguinal herniotomy in children. Long time follow-up.

Three hundred consecutive children all under 18 years of age were treated for inguinal hernia by herniotomy by the same surgeon from 1970-1985. Hernioplasty was not performed. Seven patients had a recurrent hernia, i.e., a cumulative recurrence rate of three per cent after a median follow-up of 12 years (range 5-19). Five of seven (71%) recurred within five years after the operation, while only two recurred from the 5th to the 15th year. Long term follow-up is necessary to assess incidence of recurrence after inguinal herniotomy in children.

[Ultrasonic diagnosis of colonic invagination].

Intussusception is a rare condition in adults. In the large intestine, intussusception is most frequently caused by malignant tumours. The case history of a man aged 35 years is presented here. In connection with operation for colo-colic intussusception, familial polyposis was demonstrated. The diagnosis of colo-colic intussusception was established by ultrasonic scanning.

[Tarsometatarsal fracture dislocations].

The late results of 16 tarsometatarsal fracture-dislocations are presented. The median period of observation was 9.5 years. Closed or operative reduction of the dislocations was performed and, in 11 cases, internal fixation with Kirschner wires was employed. Re-dislocation occurred only in the cases where the joint was not stabilized with Kirschner wire. The results were good in nine out of 12 cases with normal positions in the tarsometatarsal joints. None of the patients with persistent dislocation in the joint had good results. Exact closed reduction or operative reduction with internal fixation with Kirschner wire are recommended in the treatment of tarsometatarsal fracture-dislocations.

[Subtalar luxation].

Fourteen cases of subtalar dislocation were reviewed after an average period of observation of 13 years (5-27 years). Ten dislocations were medial and four were lateral. Two dislocations were open and, in seven of the cases, dislocation fractures were present. No cases of avascular necrosis of the talus were found. Three patients developed severe arthrosis and arthrodesis was performed in two of these. In cases of subtalar dislocation without fractures or dislocation fractures which did not involve the joints surrounding the talus, the prognoses were good. In cases of open dislocations or dislocations with intra-articular fractures, the prognoses were less favorable.

Enterobius vermicularis and acute appendicitis.

The relationship between Enterobius vermicularis and acute appendicitis has been discussed for more than 80 years. The latest histological examinations show that the parasites do invade the wall of the vermiform appendix, and related to these are inflammatory reactions. This invasion causes the symptoms that lead to appendectomy. A case report of a patient with symptoms of acute appendicitis and a histological finding of intramural parasites is presented.

Transversalis fascia-Cooper ligament vs. ileopubic tract repair for medial inguinal hernia.

In a prospective randomized study of 171 patients, transversalis fascia-Cooper ligament (McVay) repair of medial inguinal hernia was compared with transversalis fascia-ileopubic tract (Ottsen) repair. After median follow-up of 5 years, the total recurrence rate was 9% (95% confidence interval 4-17) following McVay repair and 15% (9-25) following the Ottsen operation. The difference was not statistically significant, but the analysis showed that, whereas the recurrence rate associated with McVay herniorrhaphy was almost constant after 2 years, recurrences continued to appear after Ottsen repair.
