[Surgeries of the orbit].

Krönlein was the first ophthalmologist to carry out orbital surgery. He described the temporal approach. The purpose of this paper is to report the earliest findings and results in tumors of the orbit in 101 patients who was treated with surgery in ORL CM UJ Clinic in Kraków. Retrobullar tumors of the orbit require a bone orbitotomy.

[The reasons for failures of laryngeal cancer surgeries].

In 4 ENT Clinics of Medical Academies in Poznań, Warszawa, Kraków, Lublin 2620 laryngeal cancer patients were operated upon during the years 1980-1987. The treatment failure occurred in 760 cases (29%). The following possible to discover factors were probably responsible for unsuccessful results: senility, other concomitant diseases (especially cardiac), prolonged diagnostic procedure, no up to date diagnostic methods, giving up the radiotherapy (38% of cases), upper laryngeal localization of tumors (87%), advanced extents T3 and T4 (83%), advanced clinical stages of cancer (III degrees and IV degrees 85%), lack of surgical radicality especially in neck dissection, unsuccessful neck dissection behind the accessory nerve, omittance of taking the specimens to the pathomorphologic examinations from the marginal part of the operational field in 25% of patients. The early recurrences in these places in apart of patients spoke for the presence of the neoplasmatic cells in this region.

[Studies of the changes in vascularization of the myocutaneous flaps of the pectoralis major. Preliminary report].

The research in the vascularisation of myocutaneous flaps of the pectoralis major muscle was conducted on the human cadavers. After the injection of the arteries 20 myocutaneous flaps were created. The main artery which is the source of vascularization of this muscle is pectoralis branch of thoraco-acromial artery. The objective of the research was to find out the direction of run of this artery. The direction variability was defined. The variability in the run of the branch of the thoraco-acromial artery refers to the place of departure from the thoraco-acromial artery, does not influence usefulness of the pectoral major muscle in creating the myocutaneous flap, considering the constant pathway of run of the distal part of this artery. The results of our research was compared with the classical description of the vascularisation given by the creator of this flap as well as with the works of authors dealing with the same issue.

[Malignant neoplasms of the middle ear in Krakow Otolaryngological Clinic in the years 1962-1994].

In the years 1962-1994, 20 patients were treated with a surgery because of malignant neoplasms of the middle ear. Pathomorphological examination showed rhabdomyosarcoma and squamous carcinoma. After surgery patients were treated supplementary Rtg-th. As a results of treatment 45% asymptomatic 5-years long survivals. A considerable advancement of the disease, late diagnosis and resulting from it nonradicality of the surgery influenced the results. Other reports and own observation advocate combined treatment: surgery and irradiation for radiosensitive malignant neoplasms of the middle ear.

[Tumors of the lacrimal gland].

Analysis of the primary lacrimal organ tumors was made. In 9 cases lacrimal gland and in 6 cases lacrimal sac was surgical treatment. Long-term results were good in all benign lesions.

[Efficiency of surgical treatment in patients with laryngeal cancer in four clinical centers].

A group of 578 patients with larynx cancer underwent surgical treatment in four Otolaryngological Clinics of Medical Universities in Poznań, Warsaw, Cracow and Lublin between 1986-1987. Within this group, 75% of operated patients had a three--year survival rate without symptoms, and 58% of patients had a five-year survival rate. A detailed evaluation of causes of failures in surgical treatment dealt with e.g. the initial placement of the cancer including entrance margins of larynx in a rich net of lymphatic vessels, extensiveness of organs, the degree of clinical progressions, general physical condition of a patient, radicality of treatment, regularity of check-ups after the operation, high degree of histological malignancy unfavourable configuration of TNM feature blood transfusions, the appearance of remote metastases low efficiency in the early diagnostics of larynx cancer, subjective conditions of patients, discontinuation of complementary irradiation.

[The development of the laryngeal cancer surgery].

We present historical review after treatment of laryngeal cancer. Complications after partial and total operations of laryngeal cancer was presented. Their results and complications were development of surgical treatment of laryngeal cancer.

[The suprahyoid approach for carcinoma of the base of the tongue].

From 1989 till 1993 10 patients with cancer of the base of the tongue were treated surgically. The surgical approach was suprahyoid pharyngotomy in 3 cases expand to intraoral approach and lateral pharyngotomy. Three patients died 3 years after surgery. All patients underwent adjunctive irradiation. The suprahyoid pharyngotomy is an ideal surgical approach in the management of T1 or T2 squamous cell carcinoma of the base of the tongue.

[The examination card for a child with suspected hearing loss or deafness].

An audiological examination card for a child has been worked out, allowing a detailed collection of anamnestic data concerning: family history, gestation, labour, neonatal period, infancy and early childhood. The items and possible answers contained in the card are so prepared that it can be used for computer analysis of the data obtained.

[Meningioma of the orbit in a thirteen-year-old boy].

Primary meningioma of the orbit is very rare. A radical surgical treatment of primary meningioma of the orbit by a thirteen years old boy was carried out. Main symptoms communicated by the patient were amblyopia, which terminated in blindness, as well as increasing exophtalmos of a bulbus oculi. After the operation a regression of exophtalmos has been achieved while the blindness stayed unchanged.

[Malignant fibrohistiocytoma of the larynx].

A patient in age 68 years with malignant fibrohistiocytoma of the larynx was surgically treated in the department of Otolaryngology, Medical University in Cracow. Fibrohistiocytoma is very rare tumor of the larynx. The diagnosis was supported by immunohistochemical examination. In the course of treatment were observed local relapses and neck metastases.

[Dermoid cyst of the nose].

Dermoid cyst of the nose may present difficult management problems if inadequately evaluated preoperatively or if incompletely excited. The most important is to differentiate them from nasal gliomas, meningocoeles et encephalocoeles. The Otolaryngological Clinic of Jagiellonian University in Cracow presented a review of the congenital midline nasal mass during 1945 and 1992 year. If a dermal fistule appears, it may be identified as a cutaneous defect leading from the midline of the nose. If this dermal fistule is connected with the skin of the nose it is called a dermal fistula dorsi nasi.

[Osteomas of frontal sinus].

In the years 1982-1992, 12 patients with frontal sinus osteoma were treated in the Department of Otolaryngology, Medical University in Cracow. The osteomas has been surgically removed by the brow incision. In the case of seven patients the tumor was removed as a whole but in five cases in parts because of their large size. In all patients the cosmetic result was satisfactory.

[Evaluation of elemental and mineral composition of larynx thyroid cartilage in the period of its involutional mineralization].

The research has been conducted on the material of 170 larynx thyroid cartilages of persons in the age range from 19 to 99 years, applying the radiological method, spectroscopy, electyronic scanning microskopy and rentgenological diffractometry. The distribution of certain elements has been determined in fragments of thyroid cartilage with different progression of mineralization. It has been proved that the elemental composition of the examined thyroid cartilages depends on the individual age and the progression of mineralization, resulting from the age.

[Development of reconstruction surgery of pharynx and esophagus].

We present historical review of pharyngooesophageal reconstruction after resection for carcinoma and pharynx and oesophagus stenosis. This paper presents development and complications after surgical treatment of pharynx and oesophagus. The most important of pharynx and oesophagus resection is reconstructive surgery.
