'Cold lesions' on bone scan. A case of metastatic squamous cell carcinoma of the penis.

Photopenic osseous lesions are a well-recognized but unusual manifestation of metastatic disease on bone scintigraphy. Common primary tumors giving rise to such lesions include lung, breast, renal, and thyroid. This case report illustrates multiple "cold" osseous metastases from aggressive penile squamous cell carcinoma without radiographic destruction.

Gallbladder visualization during post-therapy iodine-131 imaging of thyroid carcinoma.

A persistent, extra-thyroidal focus of 131I on whole-body imaging of thyroid carcinoma usually represents a functioning metastasis. We report a case of 131I localization within a septated gallbladder, initially mimicking an isolated hepatic metastasis. Adjunctive liver/spleen and hepatobiliary scintigraphy helped to elucidate the true nature of this 131I activity.

Imaging of intrathoracic disease with indium 111-labeled leukocytes.

Indium 111-labeled leukocyte scintigraphy provides an excellent noninvasive means for identifying and localizing infectious and inflammatory sites within the body. This article first compares gallium 67 citrate, the long-established radiopharmaceutical for infection imaging, to 111In-labeled leukocytes, particularly as applied to the chest. 111In-labeled leukocytes require time-consuming preparation, and the technical and practical aspects of cell labeling and imaging are reviewed in detail. The main focus of this article is the application of labeled leukocyte imaging to the evaluation of pleural, mediastinal, cardiovascular, and pulmonary disease.

Parathyroid adenoma associated with neurofibromatosis: correlative scintigraphic and magnetic resonance imaging.

Correlative imaging by dual-isotope thallium/technetium subtraction scintigraphy, computed tomography, and magnetic resonance imaging demonstrated a pathologically proven parathyroid adenoma in a 62-year-old man with known neurofibromatosis, who presented with hypercalcemia and an elevated parathormone level. The association between neurofibromatosis and primary hyperparathyroidism is discussed.

Significance of nodal uptake on indium 111 labeled leukocyte scans.

A retrospective study of 132 indium 111 labeled leukocyte scans on 129 patients was undertaken to determine the incidence and significance of activity in the neck, axilla, and groin suggestive of regional lymph nodes. Possible nodal uptake was observed in 35 out of 108 (32%) adults and 11 out of 21 (52%) children. Activity was characterized as symmetric or asymmetric. Based on medical record review, positive scans were divided into two groups: with or without clinical correlation. Nodal visualization was common in children and showed a high degree of clinical correlation. In adults, asymmetric uptake in the axilla and groin was frequently associated with disease. Cervical activity, however, often remained unexplained and may represent false-positive uptake in mandibular marrow or submandibular glands. In summary, nodal uptake may represent a significant finding, reflecting known disease or may signal subclinical disease.
