HBV infection in chronic alcoholics. A survey on 287 cases.

Out of 287 chronic alcoholics, 21 of whom (7.32%) resulted HBsAg positive, 106 were tested for other serum HBV markers. An overall HBV prevalence of 55.66% was found. Cirrhotic patients were slightly older than non-cirrhotics (median age: 51 vs. 47), but the prevalence of both HBsAg and other HBV markers did not differ significantly between the two groups, neither among age classes. A relevant number of subjects (18.87%) had anti-HBc alone (or associated with anti-HBe) as the unique marker of HBV infection; this leads to some uncertainties in the evaluation of active viral disease. HBV positivity rate resulted significantly higher (p less than 0.01) in alcoholics than in two control groups, composed by health-care staff and open population (respectively 34% and 32.81% of positivity). Even if hepatitis B virus did not seem to affect directly the progress to liver cirrhosis in chronic alcoholics, a large number of patients seemed unable to clear the virus, whose importance in the development of chronic liver disease and hepatocellular carcinoma is well established. For these reasons, it seems advisable to include chronic alcoholics in high-risk groups that should be vaccinated against hepatitis B.

An evaluation of the effects of isosorbide 5-mononitrate in patients with angina on effort using dynamic electrocardiography and echocardiography.

We studied 14 patients, 9 males and 5 females, with an average age of 53.6 +/- 9.2 years, having been suffering from effort angina for over 6 months, with positive ergometer test for angina and ST-segment depression greater than 1 mm. All the patients performed a washout period of 7 days at the end of which, before and after acute administration (90 min) of isosorbide 5-mononitrate or placebo some echocardiographic parameters were controlled according to a double-blind, randomized, crossover design. Moreover, after 2 months of open treatment with isosorbide 5-mononitrate (20 mg three times daily), echocardiographic parameters were investigated again. Continuous ECG monitoring (24 h) was performed after the washout period and after 2 months of treatment. After acute administration of isosorbide 5-mononitrate, we observed a reduction in pre- and afterload, whereas after 2 months of treatment the reduction in preload seems to play a more important role. Together with other mechanisms these haemodynamic changes are responsible for the substantial diminution of myocardial oxygen consumption, and consequently for the reduction of transient ischaemic episodes, both symptomatic and asymptomatic with heart rate increase. These results suggest that isosorbide 5-mononitrate exhibits a definite nitrate-typical vascular activity that is evident both after single administration and after 2 months of treatment.

Prolactin and relaxin: antagonism on the spontaneous motility of the uterus.

Prolactin (PRL) and Relaxin (RXL) are present in human decidua at term. In previous experiments we observed that the inhibition of spontaneous uterine contractions produced by decidual RLX was masked or absent if the samples were contaminated with PRL. The present study reports the effect of purified human PRL and of sera from hyperprolactinemic patients on the motility of the uterus in vitro. Also, we studied the effect of RLX on the uterus after exposure to PRL. High-PRL sera and purified human PRL produced prompt increases in the frequency and the amplitude of the spontaneous contractions of the rat uterus. PRL antagonized RLX both when the uterus was exposed first to RLX and then to PRL and when it was exposed first to PRL and then to RLX.

[Esterification of cholesterol in patients with renal transplants. Determination of lecithin:cholesterol-acyl-transferase (LCAT) activity].

In man, cholesterol esterification is controlled by lecithin:cholesterol-acyl-transferase (LCAT), which is a key-role enzyme of lipid metabolism. Though LCAT is synthesized by the hepatocytes, its activity seems to be linked to the kidney: in fact, LCAT activity is lowered in uremic and anephric subjects, and probably also in long-term haemodialysis patients. On the contrary, in successfull renal allografts we found fully restored LCAT activity. But physiopathological mechanisms underlying relationships between LCAT and kidney are still unclarified.

Relaxin in human decidua.

Relaxin (RLX) has been purified from the ovary of the pregnant pig and rat but not from human tissues. Our study shows that tissue extracts and incubation media of in vitro culture of human decidua contain a substance with relaxin bioactivities: the inhibition of spontaneous uterine contractions and the elongation of the interpubic ligament. After chromatography on Sephadex G-50 the bioactivity was retained in a protein fraction of approximately 6000 daltons mol wt. The yield of RLX from decidua was 15--33.5 GPU/g fresh tissue. This opens the possibility of the isolation and purification of RLX in the human species.

Is human decidua a specialized endocrine organ?

The production of protein hormones by human placenta, amnion, chorion, and decidua capsularis was studied in in vitro experiments to establish whether the decidua could be considered a specialized structure for the release of PRL. The tissues were incubated in the incubation medium, we found a highly significant rise of PRL during culture of the decidua, while no increase was noticed during culture of the placenta or amnion. Conversely, chorionic somatomammotropin and CG increased greatly and quickly during culture of placenta but not during culture of other tissues. No significant change was found in GH. The total PRL released into the medium from decidua was 3 times higher than the initial PRL content of this sittue; addition of puromycin to the incubation medium reduced both the tissue content and the release of PRL to almost 50% of the control values. This result raises the possibility of a specific endocrine activity of decidua capsularis. The PRL-secreint cells of the decidua probably do not possess dopamine receptors, since bromocriptine, when added to the medium, did not influence the release of PRL, confirming our previous in vivo observations.
