[Clinical cardiac electrophysiological studies in carotid sinus syndrome].

The aim of the present study was to evaluate the impact of electrophysiological study in the diagnosis and treatment of carotid sinus syndrome. The indications for the study include (1) attempting to exclude other cardiogenic causes of syncope; block, tachyarrhythmia, (2) diagnosing a vasodepressor response in the presence of cardioinhibitory type of carotid sinus syndrome, (3) determination of the optimal pacing mode in patients who need pacemaker implantation for treatment. 51 patients, 40 male (78%) and 11 female (22%) with an average age of 62 years suffering from carotid sinus hyperaesthesia--3000 ms ventricular asystole on carotid sinus massage--were investigated with electrophysiological study. The A-H interval in 8 patients (16%), the H-V interval in 5 pts (10%) were prolonged in the His bundle electrocardiogram. In 39 patients (76%) sinoatrial block, in 12 patients (24%) A-H block was found during carotid sinus massage. Early A-H Wenckebach block occurred in 8 patients (16%). Retrograde (V-A) conduction was present in 36 patients (70%) at a frequency of 65-85 bpm, and 22 pts (43%) at a frequency of 120-180 bpm. Sinus node disease was found in 10 patients (20%) according to the sinus node recovery time and sinoatrial conduction time. In 8 patients (16%) supraventricular and in 4 patients (8%) ventricular tachyarrhythmia was induced during study. The atrial stimulation could not prevent the occurrence of A-V block during carotid sinus massage in any of the 51 patients. In 2 patients (4%) the vasodepressor reaction with ventricular stimulation was determined.(ABSTRACT TRUNCATED AT 250 WORDS)

[Value of the waist-hip ratio in adolescent obesity].

Interrelationship between the body mass index (BMI), the waist-hip ratio (WHR) and serum concentration of immunoreactive insulin (IRI), C-peptide and sex hormone binding globulin (SHBG) was investigated in adolescents with obesity. Significant positive correlations were found between BMI and IRI, between BMI and C-peptide and there was a negative correlation between BMI and SHBG. WHR did not correlate with IRI, C-peptide and SHBG. IRI, C-peptide and SHBG in the group of central and gluteal obesity differentiated by WHR did not differ significantly. These results suggest that use of WHR to differentiate central and gluteal obesity does not help in adolescent period, possibly because of somatic changes of puberty.

Target cells are sensitized for cytotoxic T-lymphocyte-mediated destruction by estradiol and tamoxifen.

The effect of estradiol (E2) and tamoxifen (TX) on cytotoxic T-lymphocyte (CTL)-mediated target-cell lysis was studied. CTL was generated in mixed cultures of rat spleen cells, using female Fischer 344 rat cells as responders and female Wistar rat cells or Nb2 rat lymphoma cells as stimulators. Concanavalin-A-stimulated Wistar lymphoblasts or Nb2 cells were used as targets. CTL harvested on day 5 exerted 16-25% cytotoxicity when used at 1:12-1:50 target:effector cell ratios. Day-6 CTL had no cytotoxic activity. Treatment of target cells with either TX or E2, or both, at 1-microM concentrations for 4 hr prior to cytotoxicity testing raised the target-cell killing to 100%. Highly significant enhancement of cytotoxicity was also observed when the drugs were used at 100-, 10-, or 1-nM concentrations. Treatment of effector cells under similar conditions led to inhibition of cytotoxicity at 1-microM concentration, some enhancement at 100 nM and no effect at 10 and 1 nM. When treated target and effector cells were combined, the amplification of target-cell lysis was similar in magnitude to that seen in tests with treated targets only. Drug treatment of target cells had no influence on their resistance to lysis in hypotonic solutions or on total and spontaneous 51Cr release. The inhibition of DNA and protein synthesis in target cells interfered with both basal cytolysis and drug-induced enhancement. The release of the nuclear label, [3H]-thymidine, from Nb2 targets by CTL, was also enhanced by target treatment with TX. These results illustrate that CTL-mediated cytotoxicity is amplified by physiological concentrations (1 and 10 nM) of E2 and equimolar concentrations of TX.

[Unrecognised acute myocardial infarct in a nine-year autopsy material].

The authors analysed in a retrospective study the clinical features, ECG and enzyme examinations of 97 patients who died in acute myocardial infarction proved by autopsy during 9 years in their department. They found the recognition rate of the acute myocardial infarction 73%, compared to the data of the literature good. There occurred 26 unrecognized myocardial infarctions, among which atypical clinical features were more frequent and typical ECG signs and enzyme values were less frequent, than among the recognized cases. In their earlier study 57% of the recognized acute myocardial infarctions occurred on the posterior wall, in the present one the rate of anterior and posterior cases was the same. In the earlier study from 95 posterior acute myocardial infarctions 35 (41%) were unrecognized, in present from 41 cases only 11 (27%). This 14% improvement in the recognition rate of the posterior myocardial infarctions should be attributed to the routine application of paravertebral ECG leads, but statistically was not significant.

Enhanced stability of interleukin-2 mRNA in MLA 144 cells. Possible role of cytoplasmic AU-rich sequence-binding proteins.

The MLA 144 gibbon T cell line is infected with a type C retrovirus and constitutively expresses interleukin-2 (IL-2) and granulocyte macrophage colony-stimulating factor (GM-CSF). IL-2 mRNA levels are 10-fold more abundant than GM-CSF in these cells. Comparable transcriptional rates for these lymphokines suggested the involvement of post-transcriptional mechanisms in selective IL-2 mRNA accumulation. IL-2 mRNA is exceptionally stable in MLA cells with a t1/2 of more than 8 h. The presence of reiterated AUUUA sequences in the 3'-untranslated region (UTR) has been shown to confer mRNA lability. The provirally altered MLA IL-2 allele encodes an mRNA in which three AUUUA motifs have been deleted. Six major cytoplasmic proteins bound in vitro transcribed RNA probes containing sequences from the 3'-UTR of normal human IL-2 (3'-IL-2), GM-CSF (delta 2R1), and the virally altered MLA IL-2 (3'-IL-2 PV) mRNA. Increased binding of these proteins to 3'-IL-2 PV was observed relative to 3'-IL-2 or delta 2R1. Northwestern blotting demonstrated similar differential ability of a 36- and 43-kDa protein to bind, as well as showed that these proteins colocalized by immunoblotting as hnRNP A1 and C, respectively. These findings suggest a direct correlation between differential binding of cytoplasmic proteins to AU-rich 3'-UTRs in vitro and lymphokine mRNA stability in vivo.

Selective modulation of IFN-gamma mRNA stability by IL-12/NKSF.

We investigated the role of IL-12 in regulating IL-2 and IFN-gamma production in primary culture of human T cells. Addition of neutralizing antiserum against the 40-kDa subunit of IL-12 to PHA-stimulated PBMC markedly reduced both IFN-gamma protein production and mRNA accumulation and stability. Moreover, concurrent treatment of partially purified T cells (> 90% CD3+) with PHA and rIL-12 selectively enhanced IFN-gamma mRNA stability and protein production, while IL-2 protein and mRNA levels were unaffected. These studies also show that IFN-gamma and IL-2 mRNA stability are temporally dissociated during the course of T cell activation, and we propose that this dissociation may be mediated through the production of IL-12. The effect of IL-12 on modulation of IFN-gamma mRNA turnover is not associated with detectable changes in either the levels or affinity of cytoplasmic RNA-binding proteins capable of recognizing AU-rich sequences in the 3'UTR of IFN-gamma mRNA.

Immunogenicity of the S protein of transmissible gastroenteritis virus expressed in baculovirus.

Seven fragments of the spike (S) gene cDNA of transmissible gastroenteritis virus (TGEV), as well as the full length cDNA, were cloned and expressed in baculovirus vectors. Piglets were immunized with cells infected with the recombinant viruses. Each of the recombinants induced TGEV-specific antibodies detected in a fixed cell enzyme immunoassay. The amino terminal half of the S protein, containing all four major antigenic sites (A, B, C and D), and encoded by a 2.2 kb fragment of the S gene, induced virus neutralizing (VN) antibody titers comparable with those induced by the complete S protein. Recombinant proteins lacking the A antigenic site, or with a deletion including the putative receptor binding sites and the D antigenic site, were not capable of inducing levels of VN antibodies similar to those induced by the whole S protein.

Studies of fowlpox virus recombination in the generation of recombinant vaccines.

A p7.5/beta-galactosidase (7.5 lacZ) gene construct, cloned adjacent to the fowlpox virus (FPV) thymidine kinase (tk) gene was used as a marker to identify the products of recombination as 'blue' FPV plaques. The rFPVs were detected as early as 4 h after the introduction of plasmid DNAs and by 72 h post-infection (p.i.) for one transfer vector comprised 0.48% of the viral population. The proportion of rFPV increased linearly from 0.073% to 0.62% as the cumulative length of homologous sequences in the transfer vector increased from 0.73 to 4.5 kb. Two approaches using a second reporter gene, the Newcastle disease virus haemagglutinin-neuraminidase (NDV HN) gene were tested to differentiate between single and double cross-over events. In one, the HN gene was cloned into the FPV tk gene and the 7.5 lacZ cloned outside of the homologous region. Progeny of a single cross-over with FPV DNA generated an unstable plaque containing the HN gene and subsequent intramolecular recombination resulted in excision of the 7.5 lacZ and the generation of a stable 'white' plaque. For virus grown in CEF cells (tk+) in the presence of 5-bromo-deoxyuridine, only those viruses which contained a tk gene disrupted by the HN gene formed plaques. This approach allowed us to easily identify rFPV containing the HN gene but lacking 7.5 lacZ or other bacterial sequences. In a second approach, a double cross-over between rFPV DNA containing a stably expressed beta-galactosidase gene cloned into the tk gene (blue plaque) and plasmid DNA containing the HN gene flanked by tk sequences would allow transplacement of the 7.5 lacZ gene with the HN gene, and generating a white plaque. We were unable to generate recombinant viruses with the HN gene and which generated a white plaque, indicating that double cross-over events do not occur at a sufficiently high frequency in FPV.

Vaginal lactobacillosis.

OBJECTIVES: Our purpose was to study a cyclic condition characterized by vaginal discharge and discomfort occurring 7 to 10 days before menses and associated with the occurrence of long serpiginous rodlike organisms, visible in wet mount preparations. STUDY DESIGN: Thirty-seven patients and 30 controls were studied prospectively. The vaginal flora was analyzed, including isolation of anaerobic bacteria and fungi. Species of lactobacilli isolated from some of the women were identified and the antibiotic susceptibility was determined. Recent episodes of vaginitis and therapy were recorded. RESULTS: Ninety-two percent of the patients had a history of recent episodes of vaginosis lasting for an average of 22.2 months, and 57% had received triazole or imidazole antifungal therapy. Anaerobic lactobacilli were isolated from 97% of the vaginosis patients and from 40% of the controls. The average length of the organism seen was 60 microns in the patients and 10 microns in the comparison group. Amoxicillin and clavulanate potassium proved to be an effective therapy symptomatically and resulted in disappearance of the extremely long organisms in vaginal smears. CONCLUSIONS: Lactobacilli vaginosis is associated with extremely long lactobacilli. The cause of this morphologic transformation is unknown. The condition can be effectively treated with antibiotics.

Fibronectin and vitronectin binding of Bacteroides fragilis and eight other species of the genus.

The fibronectin and vitronectin bindings of 152 strains belonging in 9 Bacteroides species of different origins were tested by means of latex agglutination. 23% of the strains isolated from faeces exhibited fibronectin binding, as did 46% of the strains obtained from severe infections. Most of the strains displaying fibronectin binding belonged to the species Bacteroides fragilis or Bacteroides vulgatus. The binding could be inhibited by preincubation of the cells with an excess amount of fibronectin. Vitronectin binding was less common, but was always observed in parallel with fibronectin binding.

Vasopressin activates a chloride conductance in cultured cortical collecting duct cells.

In rabbit cortical collecting duct (CCD) cells, arginine vasopressin (AVP) causes a transient increase followed by a sustained depression of transepithelial potential difference (PDte). Mechanisms underlying the decrease in PDte are not well understood. In this study, we used primary cultures of rabbit CCD cells to study effects of AVP. Basolateral addition of AVP caused a dose-dependent increase in transepithelial conductance (Gte) and a corresponding decrease in PDte. A significant effect was observed at 1 pM AVP, and half-maximal response occurred at 30 pM AVP; 1 nM AVP increased Gte and decreased PDte. Replacement of apical Na+ with N-methyl-D-glucamine did not prevent the effect of AVP on Gte, suggesting that it is not mediated by an increase in apical Na+ conductance. Similarly, apical Ba2+ (1 mM) or 4,4'-diisothiocyanostilbene-2,2'-disulfonic acid (DIDS, 0.1 mM) failed to prevent the effect of AVP. On the other hand, 5-nitro-2(3-phenylpropylamino)benzoic acid (0.1 mM) caused partial inhibition, whereas substitution of apical Cl- with gluconate or cyclamate almost completely prevented the AVP-induced increase in Gte. In unidirectional ion-flux studies, 1 nM AVP caused only a modest increase in apical-to-basolateral (A-->BL) flux of 22Na and had no effect on transepithelial flux of 86Rb in either direction. On the other hand, AVP caused a pronounced increase in A-->BL flux and BL-->A flux of 36Cl, resulting in an increased net Cl- absorption. The effect of AVP on Gte could be mimicked by 8-bromo-adenosine 3',5'-cyclic monophosphate (8-bromo-cAMP) and isoproterenol, and effects of AVP and isoproterenol were not additive.(ABSTRACT TRUNCATED AT 250 WORDS)

The kinetics of aluminium-containing antacid absorption in man.

The kinetics of aluminium absorption were determined in patients with duodenal ulcer and in 2 normal males after taking the antacid Tisacid (Biogal, Hungary). A peak serum aluminium of 54.5 micrograms/l was reached at 30 min, returning to the initial value of 6.8 micrograms/l at 3 h after the oral intake. Urinary but not serum aluminium increased in parallel to the amount of drug taken. The serum Al values are considerably influenced by the time of sampling. Owing to the rapid absorption and excretion of aluminium, there appears to be no danger of aluminium deposits in the tissues or of neurological alterations, provided renal function is normal.

Vaccination of chickens with a recombinant fowlpox virus containing the hemagglutinin-neuraminidase gene of Newcastle disease virus under the control of the fowlpox virus thymidine kinase promoter.

When chickens were vaccinated with a recombinant fowlpox virus (FPV) containing the Newcastle disease virus (NDV) hemagglutinin-neuraminidase (HN) cDNA under the control of the thymidine kinase (TK) promoter and inserted into the FPV TK gene, the FPV antibody response to the recombinant virus was similar to the response to vaccination with standard FPV, and the recombinant virus protected chickens against challenge with virulent FPV. While the presence of the NDV HN cDNA was demonstrated in the recombinant virus, which was stable on serial passage, expression of HN was not detected by hemagglutination, Western blot analysis or immunoprecipitation of infected cell lysate. Chickens vaccinated with the recombinant virus failed to mount an NDV hemagglutination-inhibition antibody response, and they did not resist challenge with velogenic NDV. It was concluded that the TK promoter was too weak to drive the HN gene, but that the insertion into the FPV TK gene did not reduce the immunogenicity of the virus.

[Incidence of transient bacteremia following tooth extraction and antibiotic sensitivity of isolated bacteria].

In 12 patients (26%) no bacteria where found in any of the blood culture systems inoculated with blood taken 2-5 minutes after extraction. In the blood from 20 patients however, bot Oxoid and Bio-Merieux (aerobic/anaerobic) cultures proved positive, while in 13 cases only Bio-Merieux and in 2 cases only Oxoid cultures showed bacterial growth. Positive cultures were found only in patients who displayed poor or moderate oral hygiene. The strains most often isolated were Gram-positive; aerobic facultative and obligate anaerobic bacteria were found with similar frequency. Most bacteria proved sensitive to Amoxicillin, while 25% of the isolated bacteria were resistant to Penicillin-G, and 6% of them to cefuroxime and cefaclor.

[A point system (Trauma Score) in the classification of injuries in emergency care].

essence of the modified variation of AIS scheme made suitable for data processing, for the application of which the so called "AIS vocabulary" is necessary. In their Department the severity of injury of all patients has been assessed in a routine way, on the basis of scoring systems since 01.07.1987. Their experience obtained in the field of scoring poly- and multi-traumatized patients is reported. In their opinion knowing and applying scoring systems give valuable information not only from the point of view of the practice of casualty attendance and prognosis to be predicted but for objective financial assessment too.

[Interferon (Egiferon) therapy of chronic active hepatitis].

Human leukocyta interferon (Egiferon) has been administered in 10 cases having HBsAg positive chronic active hepatitis. The treatment was started with 3 x 3 MU week for one month and either with 2 x 3 MU week or 3 x 3 MU week for 2 subsequent months. HBsAg negativity was found in cases of 4 and histological improvement in cases of 3 patients. The treatment was ineffective in 3 cases.

Association of heterogeneous nuclear ribonucleoprotein A1 and C proteins with reiterated AUUUA sequences.

Post-transcriptional regulatory mechanisms have been shown to play a major role in gene expression in eukaryotic cells. The presence of a reiterated pentamer (AUUUA) in the 3'-untranslated region (UTR) of mRNAs encoding lymphokines, cytokines, transcription factors, and proto-oncogenes has been shown to be associated with rapid turnover and translation attenuation. Cytoplasmic proteins (70, 50, 43, 36, and 25 kDa) capable of specifically binding to RNAs containing these AU-rich sequences were identified in human peripheral blood T lymphocytes. Levels of the 36-kDa protein were markedly increased following transcriptional, but not translational inhibition, a feature recently reported for hnRNP A1, a protein of comparable mass. Antibodies directed against heterogeneous nuclear ribonucleoproteins (hnRNPs) A1 and C immunoprecipitated 36- and 43-kDa proteins that had bound the AUUUA-rich region contained in the 3'-UTR of granulocyte-macrophage colony-stimulating factor mRNA. Recombinant hnRNP A1 was shown to preferentially bind to RNAs containing AUUUA sequences in a specific manner, and displayed comparable patterns to the 36-kDa AU-specific binding proteins following partial proteolysis. These data identify for the first time hnRNP A1 and C as cytoplasmic proteins in human lymphocytes that are capable of specifically associating with reiterated AUUUA sequences present in the 3'-UTR of labile mRNAs. As such, they may play a role as trans-acting factors in the modulation of cytoplasmic mRNA turnover and translation, in addition to their previously characterized roles as pre-mRNA binding proteins involved in nuclear mRNA processing.
