[Classification of acquired aortic stenosis (based on electrocardiographic study data)].

The article is based on the case records of 192 patients with acquired aortic stenosis and interpretation of the ECG of 187 patients. In this group, 143 patients underwent operation and 8 died in the period of their examination. The purpose of the examination consisted in determining whether or not the ECG could characterize the stage of the cardiac disease. Four types of electrocardiographic changes of the ventricular complex characteristic of left-ventricular hypertrophy were distinguished. On comparing clinical and electrocardiographic findings, no clear dependence was revealed between the clinical manifestations of the stenosis and the severity of the hypertrophic process in the muscle of the left ventricle according to the ECG data. The information gained by means of ECG is confirmed by the results of pathomorphological examination. The authors suggest a clinical classification of acquired aortic stenosis based on the function changes in the left-ventricular myocardium as determined from the ECG.

Aortic Valve Stenosis

[Evolution of lesser circulation hypertension in patients with congenital heart defects based on the example of an interventricular septal defect].

The results of direct pressure measurement in pulmonary circulation in 452 patients with ventricular septal defect (216 males and 236 females), whose age ranged from 12 months to 37 years, were analysed. The findings of diagnostic catheterization of the heart in 150 practically healthy persons 12 months to 40 years of age were used to elaborate the normal values of pressure in pulmonary circulation. The results of pressure measurement in pulmonary circulation repeated at intervals of 1 to 6 years in 46 patients with ventricular septal defect, 18 months to 23 years of age, were analysed. Reverse evolution of pressure in pulmonary circulation after surgical rehabilitation was followed up in 85 patients in 1, 2, 3, 5, and 7 years after the operation. At the age of 12 months to 7 years, systolic pressure in the right ventricle and pulmonary artery comes down to a possible minimum due to expenditure of the adaptational reserve. In large ventricular septal defects, however, the organism proves incapable of reducing the pressure in the early and first periods of childhood as a rule, and so the pressure increases throughout life.

[Course of the rheumatic process in mitral heart defects after primary and repeated courses of treatment with titrated gamma-globulin].

Titrated gamma-globulin, which the authors used in the preoperative management of patients with acquired valvular disease and minimum rheumatism activity, contained high titers of antibodies against the antigens of streptococcus (streptokinase, streptohyaluronidase, streptolysin 0) and Coxsackie A13, A18, and B3 viruses. Its use in combination with antirheumatic agents did not cause any complications and made it possible to reduce the activity of the process in most cases with grade I and after repeated courses also in grade II activity of rheumatism. Treatment with titrated gamma-globulin stimulates the organism's defence forces (factors of nonspecific immunity: complement, lysozyme, interferon) and leads to diminution of the isolation of cardiotropic viruses from blood and bioptic material of the atria of patients.

[Myocardial function in patients with congenital pulmonary valve stenosis].

The studies of metabolism and morphology of the myocardium, and of ECG parameters in 72 patients with valvular pulmonary stenosis conducted with biopsy and autopsy material has demonstrated that while the workload on the right ventricle grows and the severity of the pathological process increases, the oxidative capacity of the myocardium also increase, the indices of its energy supply decrease, the degree of myocytes hypertrophy grows, and the number of mitochondria and the coefficient of their energy efficacy decrease. A correlation was established between the degree of these changes and the severity of electrocardiographic signs of hypertrophy and disorders in the repolarization process in the right ventricle.

[Intravital study of energy metabolism in the liver in mitral valve stenosis].

In 42 patients with mitral stenosis an intravital study was conducted comparing the liver energy exchange with the biochemical blood serum parameters and morpho-histochemical characteristics of the biopsy material. A correlation was established between the energy exchange and the clinical stage of the disease, the pigment function of the liver and the severity of its structural changes.

[Electric counter shock therapy as a rehabilitation stage for patients with surgically treated acquired heart diseases].

A total of 910 case-reports on electropulse treatment of fibrillation arrhythmia are analyzed in the light of rehabilitating patients operated upon for acquired heart diseases. Experience has shown that re-establishment and retention of the sinus rhythm is of decisive importance in the restitution of physical activity and capacity for work. However, investigations have confirmed that the electropulse therapy helps achieve but good immediate results, whereas the remote ones remain unsatisfactory. The problem of stabilizing the rhythm still awaits its solution. The application of agents blocking the beta-adrenoreceptors has but insignificantly reduced the incidence of recurrent episodes of auricular fibrillation.
