Fat digestion and absorption in the adrenalectomized rat.

Malabsorption of fat in the adrenalectomized rat has been confirmed. The criteria for adrenal hypofunction were more rigorous than in previous studies. The malabsorption is not due to impaired intestinal lipolysis or inadequate intestinal intracellular glyceride synthesis. Delayed gastric emptying is probably secondary to the malabsorption rather than a cause. It is postulated that impaired intestinal fatty acid activation may be the key defect and that in some way this inhibits fatty acid transport across the mucosal membrane.

Studies on the sensory hairs of receptor cells in the inner ear.

The crista ampullaris of the semicircular canal in the frog can be isolated and mounted in a chamber so that the sensory hairs can be observed under high magnification in interference-contrast. The cupula is removed and the sensory hairs can be manipulated and their mechanical properties investigated by a microprobe held in a micromanipulator. The hairs appear quite stiff and pivot around their base. When subjected to force they break as if they are brittle. All the cilia within a bundle move together as if joined to one another. Labelling for electron-microscopy with polycationic ferritin reveals that the membrane surrounding the cilia has a surface coat of negatively charged molecules. When the organ is incubated with polycationic ferritin before fixation the sensory hairs agglutinate. Fusion of the membrane surrounding individual sensory hairs also occurs.

Up- and downshoot in Duane's retraction syndrome.

Up- and downshoot of the adducted eye in patients with Duane's retraction syndrome was surgically eliminated in five patients by posterior fixation of the horizontal rectus muscles. Retroequatorial fixation of these muscles reduces sideslip and thus a vertical effect of the co-contracting horizontal rectus muscles.

Evaluation of a new cutaneous topical anesthesia preparation.

Topical anesthetic agents are usually not effective on intact skin because of poor penetration. EMLA is a new topical anesthetic formulation consisting of a eutectic mixture of the local anesthetics lidocaine 5% and prilocaine 5%. We evaluated the ability of this preparation to prevent or ameliorate the pain associated with the percutaneous placement of large IV catheters. Under double-blind randomized conditions, either EMLA or placebo was applied to the dorsum of both hands and an occlusive bandage then placed over each application site. The creams were placed 30, 45 or 60 minutes prior to IV cannulation. A 16-gauge catheter was inserted through each application area. Patients were asked to evaluate the relative degree of pain at each venous puncture site. The EMLA site was preferred when the formulation was applied for 45 to 60 minutes prior to cannula placement (p less than 0.01, confidence limits for the binomial distribution). Cusum analysis confirmed this minimal effective application time. The results indicate that EMLA is an effective cutaneous anesthetic formulation when applied with an occlusive bandage for a minimal time period of 45 minutes.
