Replacement of the anterior cruciate ligament. a comparative study of four different methods of reconstruction.

Eighty-seven patients have been examined 2 years on average after knee ligament reconstruction for a torn anterior cruciate ligament. The patients were divided into four groups according to the type of operation that had been carried out. In the first group an extra-articular lateral repair ("MacIntosh tenodesis") had been performed, in the second group an intra-articular "over-the-top" repair using the quadriceps and the patellar tendon, in the third group a modified "Eriksson" procedure using the patellar tendon, and in the fourth group a combined intra- and extra-articular repair using carbon fibres as a graft. The results of the operations in the different groups are compared. The best results were obtained with the "Eriksson" procedure, closely followed by the "over-the-top" repair. Limited range of motion and retropatellar pain resulting from changes in the alignment of the patella were the main problems. The results after the use of carbon fibres were less good. In two cases the graft tore without further trauma, and there were also problems because of restricted range of motion and retropatellar pain. The worst results were found after extra-articular lateral repair, due to insufficient stability in many cases. However, the best results with regard to the range of motion were found in this group.

[Operative therapy of benign and semimalignant bone tumors].

The difficulties in treating bone tumors involve mainly: (1) assurance of benignity, (2) possibility of malignant degeneration, (3) tendency to recur locally, (4) problems arising from topography (spine tumors, joint destruction, growth disturbances, deformities, marked vascularization, pathological fractures), (5) malignant degeneration (mainly after radiation therapy, which is generally not indicated) and (6) tendency to recur (50% and more), particularly after inadequate treatment. The "therapy" of benign bone tumors ranges from simple observation to "en bloc" resection with extensive replacement by bone grafts and corresponding osteosynthetic techniques.

Operative stabilisation of dislocations, fractures and fracture-dislocations of the cervical spine.

A retrospective study of 76 patients with surgically treated cervical spine injuries from 1971 to 1980 is presented. The indications for operative treatment, the technical procedures and the preliminary results are analysed. The conclusions are as follows: 1) A rational classification of the cervical spine injuries must precede the decision for operative or conservative management. 2) The intra- and postoperative complications are low with trained staff. 3) Unsatisfactory results from an operation are due to inappropriate indication for an anterior, posterior or combined fusion. 4) A laminectomy should never be done without a fusion, and a decompression of the spinal canal is better done as a rule by an anterior approach. 5) In children an anterior fashion should be avoided in order to prevent deformity from growth arrest of the end plates of the vertebrae. 6) In contrast to the degenerative disc disease the anterior fusion should always be secured by a plate.

The areas of contact pressure in the patello-femoral joint.

The development of damage to the articular cartilage of the patello-femoral joint depends on many factors. Pathological distribution of pressure in the joint may be one such cause. Direct measurement is required to clarify the question of low and high pressure in this joint. A pressure measuring plate has been made in the form of an air cushion which enables the measurement both of pressure and contact area in the knee joint. Tests using three cadaver knee joints and the results are described.

Giant cell tumors in combination with other primary bone tumors.

Three cases featuring the coincidence of 2 different primary bone tumors at the same site are presented. In 2 cases, the combination was not one of giant cell tumor and enchondroma, in the 3rd one, of giant cell tumor and non-ossifying fibroma. The patho-genetical and prognostical problems of these cases are discussed.

The treatment of femoral neck fractures with an isoelastic endoprosthesis implanted without bone cement.

We report about our first experiences inserting an "isoelastic" femoral endoprosthesis in cases of a fractured femoral neck. 28 patients have been observed for a period of more than 1 year after surgery. We did not find loosening or incompatibility of the polyacetal-resin material. We believe the implantation of this prosthesis type without cement fixation is especially advantageous. The histologic result of a patient who died 6 weeks after surgery did not reveal any tissue incompatibility. We observed an excellent fixation of the prosthesis by bone growth into the surface indentations of the stem.
