Three-dimensional description of pulmonary deposition of inhaled aerosol using data from multimodality imaging.

UNLABELLED: Three-dimensional assessment of pulmonary deposition of inhaled aerosol may be performed using SPECT. The use of aligned anatomical images enables improved accuracy of quantification and anatomical localization of deposition. METHODS: Techniques of analyzing these data and their application to deposition studies of two nebulizer-generated aerosols (mass median diameter 1.5 and 6.5 microM respectively) in 12 normal subjects are described. The deposition data were transformed to a standard hemispherical shape and the mean distribution pattern for each aerosol evaluated. Deposition by airway generation was then calculated using a spatial model of airway morphology. The results were compared to those from planar image analysis. RESULTS: The hemispherical transform yielded considerably more qualitative information on deposition pattern. The central-to-peripheral concentration ratio between conducting and alveolated airways was 5.27 for the coarser aerosol and 2.43 for the fine. The two-dimensional spatial estimates of the ratio were 2.61 and 2.03 respectively. CONCLUSION: Analysis of multimodality imaging data considerably enhanced information on deposition compared to planar imaging. It provides new data on aerosol deposition which will be of value to physicians involved in drug inhalation therapy.

Molecular cloning, expression and evaluation of phosphohydrolases for phytate-degrading activity.

Four acid phosphatase (phosphomonoesterase E.C.3.1.3.2) genes were cloned by polymerase chain reaction (PCR). These were pho3, pho5 and pho11 from Saccharomyces cerevisiae and the gene for a phosphate-respressible acid phosphatase from Aspergillus niger. The individual genes were subcloned into an A. oryzae expression vector downstream from a starch-inducible alpha-amylase promoter and the resulting expression constructs were transformed into a mutant strain of A. oryzae, AO7. Southern hybridization analysis confirmed that the acid phosphatase genes had been integrated into the host genome with estimates of integrated copy numbers ranging from 2 to 20 for individual transformants. Northern hybridization analysis of total RNA from individual transformants revealed the presence of a single transcript of the expected size of 1.8 kb. Production of recombinant protein was induced by the addition of 30 g L-1 of soluble starch in the fermentation media. Active acid phosphatases, not present in control cultures, were detected in the supernatant fractions of transformant cultures by acid phosphatase activity staining of non-denaturing polyacrylamide gels. The ability of the recombinant acid phosphatases to hydrolyze phytate was assessed by referenced phytase (myoinositol hexakisphosphate phosphohydrolase E.C. 3.1.3.8) activity assay procedures. A two- to six-fold increase in phytase activity was measured in transformants compared to control, untransformed A. oryzae. Sufficient quantities of A. niger and pho5 recombinant acid phosphatases were generated from large-scale fermentations to assess the efficacy of these enzymes as phytate-degrading enzymes when included in poultry diets.(ABSTRACT TRUNCATED AT 250 WORDS)

Linopirdine does not improve matching performance in the titrating matching-to-sample paradigm.

Linopirdine (DUP 996), a proposed cognitive enhancing agent, was studied in four squirrel monkeys (Saimiri sciureus) and six White Carneau pigeons responding under a titrating matching-to-sample paradigm (TMTS). Briefly, under this titration schedule, each trial began with the presentation of a sample stimulus on the center key of a three-key pigeon or squirrel monkey chamber. Completion of a fixed-ratio on the center key resulted in the termination of the stimulus presentation and the initiation of a delay period. The length of the delay changed as a function of the subject's performance. During the first five trials of each session, the delay was fixed at 3 s in length. On the sixth and all subsequent trials, the length of the delay increased, did not change, or decreased such that accuracy was maintained at approximately 80%. Following the delay, two of the three response keys were transilluminated with different colored lights. A single response on the key transilluminated with the same stimulus as the sample stimulus resulted in the presentation of food. A response on the key transilluminated with the stimulus that did not match the sample stimulus resulted in a timeout. Linopridine was administered in the pigeon (0.001-5.6 mg/kg) and squirrel monkey (0.01-1.0 mg/kg) 15 min before testing. Matching performance was not affected as measured by changes in mean delay values or percent accuracy even at doses that decreased rate of responding. These results suggest that the enhancement in cognitive function previously reported after administration of linopiridine may be limited to specific situations.

Proscar: five-year experience.

We assessed the long-term safety and efficacy of finasteride, an orally active 5 alpha-reductase inhibitor, in 2 previously reported groups of patients with symptomatic benign prostatic hyperplasia (BPH). Prostate volume was measured by magnetic resonance imaging, and the maximum urinary flow rate was assessed noninvasively. Symptoms were scored utilizing a patient self-administered symptom score questionnaire. Total symptom scores ranged from 0 (or asymptomatic) to 35 (severely symptomatic). After an initial double-blind period, the patients in study 1 were treated with 10 mg finasteride for 1 year and then switched to 5 mg finasteride for an additional 4 years, whereas patients in study 2 were treated with 5 mg finasteride for the entire 5 years. A total of 190 patients were randomized in the double-blind studies, 156 entered year 1 of the open extension and 70 patients completed 5 years of finasteride therapy. In both studies prostate volume was reduced from baseline by 30%, dihydrotestosterone was reduced by 72%, and the maximum urinary flow rate improved by approximately 1.5 ml/s. Prostate-specific antigen was decreased by approximately 50%. Finasteride was well tolerated; approximately 10% of patients reported sexual adverse experiences during the 5-year study period, which were considered drug related by the investigators. The incidence in reporting sexual adverse experiences did not increase with the increased duration of treatment: findings consistent with previous reports. In summary treatment of BPH with finasteride for 5 years inhibits the progression of the disease with an excellent safety profile and represents a low-risk medical option for the treatment of symptomatic BPH.

Neuropeptide Y: intrastriatal injections produce contralateral circling that is blocked by a dopamine antagonist in rats.

The brain is rich in neuropeptide Y (NPY) but its function is poorly understood. Previous studies have shown that intrastriatal injections of NPY stimulate dopamine (DA) release. In the present paper, behavioral studies evaluated the possibility that unilateral intrastriatal injections of NPY would produce contralateral circling that could be blocked by coinjection with a DA antagonist. Four experiments examined circling behavior in rats after unilateral intrastriatal microinjections (0.5 microliter) of: 1) amphetamine alone; 2) amphetamine with the DA antagonist cis-flupenthixol; 3) NPY alone; and 4) NPY with cis-flupenthixol. Each experiment consisted of seven test sessions; the first and seventh were preceded by no injection, the second and sixth by a control injection (saline or cis-flupenthixol with saline) and the third, fourth, and fifth by drug injections. Animals were scored during two 5-min intervals of a 20-min test session that began with the central injection and placement in a circular arena (30 cm diam.). Results indicated that the 25.0- but not the 6.0- or 12.0-micrograms doses of amphetamine and the 0.10- but not the 0.01- or 1.0-microgram doses of NPY produced contralateral circling. This directional bias was antagonized by cis-flupenthixol (20 micrograms in 0.5 microliter) in the case of amphetamine and fully blocked in the case of NPY. Results raise the intriguing possibility that contralateral circling induced by unilateral intrastriatal NPY may be mediated by DA.

Advances in the areas of molecular genetics and molecular biology provide a tremendous opportunity for development of custom-made microorganisms for human and animal use. This reviews discusses the current and future potentials of these organisms for use in the food and agricultural industries.

Antiretroviral therapy is associated with a decrease in unintegrated HIV-1 DNA in pediatric patients.

Good markers for monitoring the efficacy of antiretroviral therapy in children do not currently exist. This study examined the effect of antiretroviral therapy on human immunodeficiency virus (HIV-1) unintegrated DNA (uDNA), integrated DNA (iDNA), percent uDNA, immune complex dissociated (ICD) p24 antigenemia, and plasma viral titer. Seven children were followed at therapy initiation and at approximately 3- and 10-month intervals. HIV-1 uDNA was detected in all children prior to start of therapy (average percent uDNA, 43%). At 3 months, the percent HIV uDNA decreased in all patients to an average of 18% (p = 0.01) and at 10 months decreased to an average of 1%. In contrast, the amount of HIV iDNA was relatively constant after initiation of therapy. ICD HIV p24 antigen was detected in all patients prior to therapy (average, 538 pg/ml). Over the study period, the ICD p24 antigen level decreased in three patients and remained relatively unchanged in four patients. Plasma cultures of HIV-1 were positive in only one of the seven patients prior to therapy. Among the methods evaluated, measurement of uDNA was the only parameter which reliable decreased after initiation of nucleoside therapy.

Positive identification in a case of intentional extreme fragmentation.

The investigation of the first of the murders of young males to which Jeffrey Dahmer confessed led to systematic survey of two acres of semirural property in Bath Township, Ohio. The survey revealed the fragmentary skeleton of a young adult male, as well as bones of several species of animals. Through archaeobiological analysis, the animal bones were identified and taphonomic modifications documented. The human bones were dry and weathered, and many were rodent-gnawed, indicating that they had been exposed for many years. The human bones displayed an extreme degree of splintering, warping, bending, and spiral breakage. The only relatively complete bone was a cervical vertebra. Dismemberment and breakage had been accomplished by cutting, blunt force, and, in the case of the ribs, manual bending. Even in so extreme a case of intentional fragmentation, however, forensic anthropological analysis resulted in positive identification, with the primary criteria based on matching osteological features evident in premortem and postmortem radiographs of a cervical vertebra.

School nutrition survey.

Food we eat has an important influence on health and well-being. Many eating habits are established in childhood. 456 children aged eight to 12 years participated in this survey of food eaten at school. Of all the food items eaten as a snack, 48.6% were categorised as junk. 75.8% of the sandwiches brought to school for lunch were made with white bread. Of the remaining food items brought for lunch 63.5% were of the junk variety. Compared with those who brought a snack or lunch from home, those given money to buy their own were more likely to eat junk (p < 0.01). Food eaten at school reflects approximately one third of a child's daily food intake but health food practises for even a third of food intake may be of a value for health and long term eating habits. Nutritional education with the reinforcement of high nutritional standards in schools could improve the situation.

Behavioural correlates of expressed emotion in staff-patient interactions.

Staff members working in community facilities for patients with long-standing mental illness were assessed using a modification of the Camberwell Family Interview designed to measure their attitudes towards their key patients. This enabled an evaluation of their levels of expressed emotion (EE). Staff members and their patients took part in a direct interaction task 2-4 weeks later. This was deliberately focused on low-conflict issues, and each interaction was audiotaped. The presence of the nuclear symptoms of schizophrenia in the mixed diagnosis sample was the only patient attribute associated with high-EE attitudes. Staff rated high in EE were more likely than low-EE staff to make negative statments during the interaction, and less likely to make supportive ones. Patients with low-EE keyworkers were more likely to volunteer statements of self-affirmation during the interaction. Low-EE staff tended to focus on positive aspects of the patient's life. During interviews and interactions, low-EE keyworkers did not dwell on their own negative feelings (if any) or on those expressed by the patient. This characteristic is probably a salient feature of successful therapeutic relations with clients suffering from long-term mental illness.

Staff-patient relationships in the care of the long-term adult mentally ill. A content analysis of Expressed Emotion interviews.

Analysis of the content of 61 interviews with keyworkers supporting chronically disabled patients in long-term care settings revealed a range of EE ratings and associated characteristics. Low-EE interviews were prevalent (n = 46), a finding not unlikely given the experience and training of the staff group sampled. High-EE (n = 15) relationships were characterised by less tolerance, inappropriate expectations of patient progress and frustration in the key worker. Criticism in both high- and low-EE interviews was most frequently focused on socially embarrassing or difficult behaviour and, to a slightly lesser extent, the clinical poverty syndrome. It was rarely directed at positive symptomatology. High levels of criticism were significantly related to regarding the patient's difficulties as within their control and having negative rather than positive expectations of their ability to manage on their own. The factors identified by the EE interview that influence the nature of the staff-patient relationship are discussed, and the clinical implications of the findings briefly considered.

Expressed Emotion in community care staff. A comparison of patient outcome in a nine month follow-up of two hostels.

Expressed Emotion was measured in the staff of two similar hostels for the long term mentally ill in an Inner London Borough (Hostel A and Hostel B). Patients in the two hostels showed similar levels of disturbance as measured by the PSE and symptom and behaviour profile scores. Staff from Hostel B displayed higher levels of EE, and were more critical, and these attitudes generalised easily into hostility and rejection. Those from Hostel A showed lower EE with warmth and positive attitudes towards clients. At 9 month follow up, Hostel A and B had similar levels of relapse requiring hospital admission. However Hostel B had a 64% discharge rate compared with 12.5% for Hostel A. These results are discussed with reference to the literature.

Expressed emotion in staff working with the long-term adult mentally ill.

Staff-patient relationships in long-term settings were examined in 35 staff and 61 patients. Measures were also taken of the staff's general health, their coping style in relation to work events, and job satisfaction. A range of ratings of EE was evident in staff descriptions of patients under their care. Strain and criticism in the relationship were not associated with identified stressors in the workplace, or the general health of the carer. When patients were grouped according to high-EE and low-EE interviews, there were no significant differences in their symptoms. Criticism was associated with other patient characteristics, including aggressive and attention-seeking behaviour, underactivity, and limited social interaction. The findings have implications for staff training and for the maintenance of optimal staff-patient relationships in services supporting severely disabled patients.

Effect of finasteride, a 5 alpha-reductase inhibitor, on serum gonadotropins in normal men.

Testosterone exerts negative feedback control on gonadotropin secretion either directly, after aromatization to estradiol, or after 5 alpha-reduction to dihydrotestosterone (DHT). Conflicting data exist as to the role of DHT in the modulation of this negative feedback. To determine whether suppression of endogenous DHT alters gonadotropin secretion, we gave the selective 5 alpha-reductase inhibitor finasteride (5 mg daily), or placebo, to 20 healthy men for 28 days. Basal and GnRH-stimulated LH, bioactive LH, FSH, testosterone, and DHT levels were measured before and after 14 and 28 days of treatment. Basal DHT fell from 1.1 +/- 0.2 to 0.15 +/- 0.04 nmol/L after 28 days of finasteride treatment. A significant rise in baseline testosterone from 17.6 +/- 2.0 to 18.3 +/- 2.3 nmol/L was seen at 14 days (P = 0.046), but not at 28 days. No significant changes were seen in either basal or GnRH-stimulated gonadotropin levels on any day. We conclude that suppression of serum DHT levels with 5 mg finasteride daily in healthy young men has no discernible effect on serum gonadotropin levels.

Implications of the Labrador Dust Study.

Joint medical and technical investigations of an iron ore mining and processing operation, over a 2 year period, has confirmed the presence of pneumoconiosis associated with respirable dust exposure above recognized threshold limit values. This study achieved short-term objectives in quantifying dust levels and the extent of pneumoconiosis, and in identifying the need for revised surveillance programs. Definitive conclusions on a variety of dust-related questions are possible by correlating the baseline data generated with a continuous monitoring program.
