Evaluation of plasma iodothyronines in various stages of pregnancy.

Evaluation of thyroid status during the whole normal pregnancy was carried out by measuring serum total thyroxine (TT4), total 3,5,3' triiodothyronine (TT3), T3 resin uptake (T3(U)), 3,3',5' triiodothyronine (rT3), free thyroxine index (FTI), free thyroxine (FT4) and thyrotrophin (TSH) in 70 serum samples from 42 pregnant women being in a variable period from the 6th to the 40th week of pregnancy and in 14 serum samples from 14 non pregnant healthy women. The serum samples were divided in 7 groups, according to the gestational age of the subjects. Every group was formed by 10 samples collected within the same five weeks period. The levels of TT4, TT3, T3 (U) and rT3 were found significantly elevated during pregnancy, above the levels of the control group (p less than 0.01); the increase is very early. FTI remained in the range of normal non pregnant subjects without any significant change in all groups studied. FT4 concentration showed a progressive downward trend with gestational age. This fall became significant in the second group (11th to 15th weeks), when compared to the levels of non pregnant women (p less than 0.01) TSH mean values remained in the normal range within the three trimesters, though they showed some fluctuations.

Amniotic fluid content of glucose, C-peptide and insulin in normal and diabetic pregnancies.

Glucose, C-peptide and insulin concentrations in amniotic fluid were measured in 63 women, 19 of whom had insulin-dependent diabetes. The amniotic fluid glucose concentration was higher in the diabetic (37.4 +/- 4.1) than in the normal women (19.4 +/- 5.1 mg/dl; P less than 0.01). The amniotic fluid insulin concentration was higher in diabetic than non diabetic women (15.9 +/- 3.1 versus 8.9 +/- 2.1 microU/ml; P less than 0.01). The C-peptide concentration was also higher in the amniotic fluid of diabetic women than normal (1.60 +/- 0.66 vs 0.26 +/- 0.15 nmol/l; P less than 0.001). The lecithin/sphingomyelin (L/S) ratio in the amniotic fluid was greater than 2 in 61 mothers; only two diabetic patients had an L/S ratio lower than 2. Five neonates of diabetic mothers were macrosomic and three of these were also hypoglycaemic. A negative correlation was observed in diabetic women between insulin and C-peptide concentrations in the amniotic fluid and Apgar score of the newborn infants both at 1 and 5 minutes; no correlation was found on the contrary with the amniotic glucose. In the prognostic evaluation of the fetus with a diabetic mother one must consider many parameters. Among these more consideration must be given to the levels of C-peptide and insulin in amniotic fluid than to glucose level.

Respiratory function during pregnancy.

We studied the respiratory mechanics, acid-base status, alveolar ventilation and ventilatory response to hypercapnia in pregnant women at the first and third trimesters and postpartum. We found alveolar hyperventilation and hypersensitivity of the respiratory centers to carbon dioxide with no significant differences between the first and third trimesters. After delivery we found a sudden decrease in these parameters. The changes seem to be due to a direct primary (probably hormonal) stimulation on the medulla oblongata and cannot be correlated to the metabolic needs of the fetus and are aimed at favoring placental gas exchanges, especially at the beginning of the pregnancy, when the fetal cardiovascular system is not yet developed.

Effect of estrogens on luteinizing hormone release in testicular feminization syndrome before and after gonadectomy.

The effect of exogenous estrogens on luteinizing hormone release was studied in three siblings with complete testicular feminization syndrome. Two subjects, 21 and 20 years old, were postpubertal. The third, 15 years old, was in the early pubertal stage. An estrogen provocation test was performed in which 20 mg of conjugated estrogens were administered intravenously and serum follicle-stimulating hormone and luteinizing hormone levels were assessed every 12 hours for 96 hours under basal conditions, on day 5 of an eight-day treatment with 0.2 mg/day ethinyl estradiol orally, and on day 5 of a subsequent eight-day treatment with 0.2 mg/day ethinyl estradiol and 120 mg/day cyproterone acetate orally. The first two tests were repeated one month after gonadectomy. During pregonadectomy treatments there was an overall luteinizing hormone fall. After gonadectomy, the two postpubertal subjects exhibited luteinizing hormone surges during ethinyl estradiol treatment -in one as a single peak and in the other as multiple peaks. A positive feedback effect was not induced in the youngest patient either before or after gonadectomy as in normal prepubertal and early pubertal females. The data suggest that testosterone or some other testicular factor inhibits estrogen induced positive feedback for luteinizing hormone. This inhibition mechanism acts independently of the testosterone cytosol receptor.

Canalization of human cervical mucus.

Cervical mucus forms channels when dried under a coverslip. The aim of the present work was: 1) to prove mucus canalization both in spontaneous ovulatory cycles and during ovulation induction with gonadotropins; 2) to prove the estrogen dependence of this phenomenon; 3) to check the importance of the proteidic and electrolytic concentration on chaneling; and 4) to use this phenomenon clinically, shortening the time in which it occurs. The number and arrangement of channels vary during the cycle. The phenomenon is estrogen-dependent. The comparison between estradiol values and the number of channels during spontaneous ovulatory cycles and treatment with gonadotropins showed a linear relationship. Treatment with estradiol 17 beta-valerate and ethinyl estradiol induced channel formation in women with primary amenorrhea. Canalization and ferning disappeared after dialysis or treatment with proteolytic enzymes. It follows that the two phenomena have similar characteristics. Canalization increases daily, as does estradiol, whereas ferning maintains the same grade for a longer period, and when a grade of + + + is reached, it provides no further indications. With the use of a thermostat, canalization occurred in only a few hours. Chaneling, a more precise index, could therefore substitute for ferning, particularly when monitoring the induction of ovulation.

[Gaucher's disease: hemorrhagic diathesis and term pregnancy with the birth of a malformed fetus].

A patient suffering from Gaucher's disease observed for the first time five years age while in pregnancy and observed again during a second pregnancy ended with the delivery of a malformed female infant is presented. Laboratory findings of this case and their correlation with the clinical manifestations of this disease are underlined. Occurrence of foetal malformations in this disease is also discussed.

Sertoli cell adenoma in Morris syndrome.

An unusually large Sertoli cell adenoma was detected in a patient with Morris syndrome and there is a description of the uncommon hormonal data found in this particular case which are not typical for this kind of neoplasia. The patient is alive and well at the present time which is now three years after surgical intervention.

[Lipid metabolism in newborn infants at birth. Obstetrical-pediatric aspects].

Lipid parameters in the cord blood of neonates and that of their mothers at birth were studied in a series of 67 subjects divides in accordance with prior disease or diseases arising during pregnancy. From the results obtained in this albeit restricted study, agreement is expressed with the literature view that there is a common genetic control of lipid metabolism in the foetus, which is unusual and independent of that in the mother. An interesting, though unexplained finding was the absence of prebetalipoproteins in the cord blood of 71.5% of the control group.
